Seventy-six in- and outpatients seeking help for complaints of dizziness in a neurological clinic were assessed by the Structured Clinical Interview for DSM III (SCID). Neurological assessment included electrophysiological and otological examination. We established criteria to differentiate between dizziness as a symptom of panic disorder and dizziness as a symptom of neurological illness. Criteria for dizziness as a symptom of panic disorder are: adverse life events before the onset of dizziness, current comorbidity with depression, a high number of vegetative symptoms typical for panic attac

The long-term efficacy of endolymphatic sac procedures for control of vertigo in Menière's disease has been controversial. We evaluated results of sac shunt surgery for 234 patients having at least 10 years followup (mean, 13.5 years). All patients had persistent vestibular symptoms despite medical therapy. All underwent endolymphatic subarachnoid shunt as their original operation. Data were collected by chart review and questionnaire regarding: (1) the number of additional surgical procedures to control vertigo, (2) remaining dizziness, and (3) level of disability. One hundred forty-seven of …

Questionnaires assessing symptoms, anxiety and handicap were completed by 127 vertiginous patients. Factor analysis identified four distinct symptom clusters which formed the basis for the construction of scales quantifying the number and frequency of symptoms of: (a) vertigo (of long and short duration); (b) autonomic sensations and anxiety arousal; and (c) somatization. Scores on the vertigo severity scale were significantly related to clinical diagnosis and had near-zero correlations with measures of anxiety. Vertigo severity, autonomic signs and depressed mood each independently contribute…

Two randomized studies to test the efficacy of a combination of Piracetam and Ergotoxin on the destabilized brainstem are reported. In the first study, a pharmacological model using Minocycline is employed. A follow-up clinical study analysed the effect of the preparation in 5 patients suffering from vertigo and other related complaints. It was seen that there was a significant improvement in the nystagmus profiles of the pharmacological model volunteers. Similarly, the patient group showed a marked improvement in symptoms, and in orientation capability as tested by Cranio-Corpo-Graphy.

Recent investigations comparing the postural responses of normals and patients with bilateral peripheral vestibular deficit to a sudden back … e used to monitor VSR responses of 10 patients with suspected simulated, aggravated, or psychogenic vertigo--vertigo of functional origin. Two cases, presented in detail, illustrate the identification of weak reflex responses for patients aggravating an organic VSR pathology during the Romberg test and the identification voluntarily controlled destabilizi …

A case of a rare arteriovenous malformation in the internal auditory canal is reported. It caused unilateral tinnitus, facial weakness, trigeminal hypesthesia, and vertigo with lateropulsion. The audiological and otoneurological findings together with air-cisternography a CT scan had indicated an intrameatal tumor. An extended trans-temporal exposure of the internal auditory canal demonstrated an angiomatous lesion compressing the adjacent seventh and eighth cranial nerves. It could be removed safely by a second-stage lateral suboccipital approach to the cerebello-pontine angle.

The authors report a case of neurovascular compression of the eighth cranial nerve in a 49-year-old businessman. The patient was admitted to the University of Tokyo Hospital because of progressive vertigo and tinnitus on the right without hearing loss over the seven years prior. There were no other symptoms. The general examination was normal. He was neurologically intact except for loss of caloric response. Audiometric studies and brain stem response were normal. The findings of routine hematology, biochemistry, and serology were within normal limits. Tomogram showed that right internal audit…

The essential features of the posturing and positional mechanisms in man are defined and the causes of their dysfunction categorized to provide a scheme of surgical management appropriate for each. The authors' long-term results (two to twelve years) with (1) twenty-seven operations designed to decompress the endolymphatic system, (2) twenty-eight selective eighth nerve sections are documented and compared, by means of statistical analysis, with those reported by others during the past fifteen years. Although the latter procedure provided better control of vertigo, there was no difference betw…

We draw the following conclusions from clinical observations on traumatized patients with vertigo and lumbar pain. 1) Hypertonicity of the lumbar soft supporting tissues, and especially the lumbar erector muscles (i.e., over-excitment of the lumbar proprioceptors) is a cause of vertigo due to whiplash injury. Equilibrium tests after fixing the waist with a corset are useful for detecting ataxia of lumbar origin. 2) Over-excitement of the lumbar porpiroceptors and dysfunction of the brain stem and the cerebellum are closely related in bringing about vertigo due to whiplash injury. The lumbar pr…

Posterior circulation strokes are frequently underrecognized in emergency departments, especially when dizziness is the presenting complaint without overt motor deficits. Diagnostic delays often occur when providers anchor on benign causes such as benign paroxysmal positional vertigo. Early computed tomography and magnetic resonance imaging may miss acute infarcts in the posterior fossa, whereas the bedside head-impulse, nystagmus, and test of skew plus hearing examination has demonstrated greater sensitivity for detecting stroke in acute vestibular syndrome. A 76-year-old male with vascular r…

Subclavian steal syndrome (SSS) is characterized by retrograde blood flow in the vertebral artery caused by proximal stenosis or occlusion o … nting. Furthermore, we present an illustrative case of a 68-year-old male with medically refractory vertigo and right arm claudication. Clinical examination revealed a 40 mmHg inter-arm systolic blood pressure gradient. Angiography confirmed severe proximal right subclavian artery stenosis with retrograde steal. The patient was successfully treated with a …

Benign paroxysmal positional vertigo (BPPV) is the most common peripheral vestibular disorder, characterized by brief episodes of vertigo triggered by changes in head position. While idiopathic cases are frequent, post-traumatic BPPV has been increasingly recognized, particularly in individuals who have experienced traumatic brain injury (TBI). TBI, ranging from mild concussions to severe head trauma, is a significant cause of neurological morbidity and is often associated with vestibular dysfunction. The pathophysiology of post-traumatic BPPV is thought to involve direct mechanical disruption…

Diabetes has been suggested as a potential risk factor for benign paroxysmal positional vertigo (BPPV), but whether a genetic-level causal relationship exists between the 2 remains unclear. A 2-sample Mendelian randomization (TSMR) analysis was conducted using genome-wide association study data from the FinnGen database (T1DM: n = 408,210; T2DM: n = 486,367; BPPV: n = 486,845). The inverse variance weighted method was the primary analytical approach, supplemented by weighted median, MR-Egger, and other methods to assess causal effects. Pleiotropy and heterogeneity tests were also performed. In…

OBJECTIVE: The aim was to evaluate the value of otolith-associated protein otoconin-90 (OC90) and otolin-1 in the pathogenesis research and clinical treatment of benign paroxysmal positional vertigo (BPPV).
MATERIAL AND METHOD: The study included 50 patients with BPPV admitted to neurology and otorhinolaryngology departments and 30 healthy subjects with no history of dizziness as a control group.
RESULTS: BPPV and controls were similar in terms of gender and age. Otolin-1 concentration was significantly greater in the BPPV group than in the controls (710.44 [584.35-837.39] vs 280.45 [212.7-4…
