Subjective visual vertical in peripheral unilateral vestibular diseases.

In humans, the perception of vertical is provided by input from various sensorineural organs and pathways: vision, eye-movements, and proprioceptive and vestibular cues, particularly from the otolithic organs and graviceptive pathways. Well known in several types of brainstem lesions, subjective visual vertical (SVV) abnormalities may also be observed after peripheral vestibular lesions, such as surgical deafferentation, with a deviation directed toward the operated ear. Subjective visual vertical abnormalities are presumably related to a lesion of the otolithic organs and/or to changes in the

A review on screening tests for vestibular disorders.

Although many studies have reported on tests of the vestibular system a valid and reliable, evidence-based screening battery for easy clinical use remains elusive. Many screening tests attempt to assess the vestibulo-ocular reflex. Therefore, head shaking, the Dix-Hallpike maneuver, the supine roll test, and head impulse tests are discussed. Other tests address the spatial orientation functions of the vestibular system, such as the Bucket Test and the Fukuda Stepping test. Still, other tests are based on the known correlates with balance skills, both static and dynamic, such as tandem walking

Association of posterior semicircular canal hypofunction on video-head-impulse testing with other vestibulo-cochlear deficits.

OBJECTIVES: The video-head-impulse test (vHIT) provides a functional assessment of all six semicircular canals (SCC). Occasionally isolated loss of the posterior canal(s) (ILPC) is diagnosed, though this finding is poorly characterized. Here we assessed how accurate that diagnosis is by measuring the co-occurrence of abnormalities on caloric irrigation, vestibular-evoked myogenic-potentials and audiometry. METHODS: We identified 52 patients with ILPC (unilateral=40, bilateral=12). We determined vHIT-gains and saccade-amplitudes and correlated vHIT-findings with other vestibulo-cochlear tests.

Inferior vestibular neuritis.

Vestibular neuritis (VN) mostly involves the superior portion of the vestibular nerve and labyrinth. This study aimed to describe the clinical features of VN involving the inferior vestibular labyrinth and its afferents only. Of the 703 patients with a diagnosis of VN or labyrinthitis at Seoul National University Bundang Hospital from 2004 to 2010, we retrospectively recruited 9 patients (6 women, age range 15-75) with a diagnosis of isolated inferior VN. Diagnosis of isolated inferior VN was based on torsional downbeating spontaneous nystagmus, abnormal head-impulse test (HIT) for the posteri

Medical fitness examination of commercial pilots: new criteria for evaluation of vestibular tests.

In medical fitness examinations of commercial pilots, extensive vestibular investigations should be conducted. In particular, the pendular test, as a weak rotational stimulation method, can disclose central vestibular disorders. Thus, the central nystagmus tracing can be considered as a sign of irritation of vestibular centers, most frequently as a consequence of head trauma, a finding that may influence the decision in the enlistment of the pilot. In contrast, a small nystagmus amplitude tracing is observed in cases of insufficient blood supply, occurring mostly in elderly persons. Therefore,

Validation of a comprehensive diagnostic algorithm for patients with acute vertigo and dizziness.

BACKGROUND AND PURPOSE: Vertigo and dizziness are common complaints in emergency departments and primary care, and pose major diagnostic challenges due to their various underlying etiologies. Most supportive diagnostic algorithms concentrate on either identifying cerebrovascular events (CVEs) or diagnosing specific vestibular disorders or are restricted to specific patient subgroups. The aim of the present study was to develop and validate a comprehenisve algorithm for identifying patients with CVE and classifying the most common vestibular disorders. METHODS: The study was conducted within t

Computerised dynamic posturography: its place in the evaluation of patients with dizziness and imbalance.

As well as vestibular information, computerised dynamic posturography (CDP) measures visual and proprioceptive contributions to the balance system. Three hundred and seventy-five patients were evaluated by computerised dynamic posturography and the results were compared to electronystagmography (ENG) results. CDP demonstrated abnormalities in the balance system in many of the patients who had normal ENGs. It can hone the physician's skill in recognising subtle balance disease; it does not provide localization to one side; it does not offer disease-specific information; however, taken in conjun

Central Auditory and Vestibular Dysfunction Are Key Features of Autism Spectrum Disorder.

Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by repetitive behaviors, poor social skills, and difficulties with communication. Beyond these core signs and symptoms, the majority of subjects with ASD have some degree of auditory and vestibular dysfunction. Dysfunction in these sensory modalities is significant as normal cognitive development depends on an accurate representation of our environment. The hearing difficulties in ASD range from deafness to hypersensitivity and subjects with ASD have abnormal sound-evoked brainstem reflexes and brainstem auditory evo

Application of multivariate statistics to vestibular testing: discriminating between Menière's disease and migraine associated dizziness.

Menière's disease (MD) and migraine associated dizziness (MAD) are two disorders that can have similar symptomatologies, but differ vastly in treatment. Vestibular testing is sometimes used to help differentiate between these disorders, but the inefficiency of a human interpreter analyzing a multitude of variables independently decreases its utility. Our hypothesis was that we could objectively discriminate between patients with MD and those with MAD using select variables from the vestibular test battery. Sinusoidal harmonic acceleration test variables were reduced to three vestibulo-ocular r

The Aging Vestibular System: Dizziness and Imbalance in the Elderly.

Aging in vestibular structures with loss of hair cells in the inner ear starts early in life, but vestibular function usually remains relatively unimpaired up to advanced ages. However, dizziness and imbalance are common in the elderly and have substantial impact on the quality of life. Dizziness interferes with the everyday activities of 30% of persons over age 70. As causes can be multiple, including vestibular and non-vestibular components, it is crucial to detect the factors leading to dizziness and imbalance to maintain mobility and avoid secondary complications like falls and anxiety. Peripheral Downbeat Positional Nystagmus: Apogeotropic Posterior Canal or Anterior Canal BPPV.

BACKGROUND AND PURPOSE: Downbeat nystagmus (DBN) during the Dix-Hallpike test (DHT) suggests excitation of the anterior canal (AC) or inhibition of the posterior canal (PC) underlying benign paroxysmal positional vertigo (BPPV). This case series describes 2 individuals presenting with DBN in positional testing suggestive of a PC BPPV variant termed apogeotropic PC-BPPV and due to inhibition of the PC. CASE DESCRIPTIONS: Case 1 illustrates a DBN during positional testing (PC inhibition) that changes to an upbeating nystagmus (PC excitation) representing the otoconial material changing location

An Overview of Central Vertigo Disorders.

Head motion recorded by the vestibular labyrinths is conveyed to specific brainstem and cerebellar structures that relay velocity information to eye muscles to stabilize vision, and to the axial and limb muscles necessary to stabilize balance. Neural networks enhance and extend the primary vestibular signal, and create adaptation to movement when appropriate. Pathological lesions to one or more of these structures may cause central vertigo and imbalance, and may be localized by specific forms of nystagmus and other abnormal neurological signs. Vertigo treatment may be directed to the underlyin

Subclinical vestibular pathology in patients with noise-induced hearing loss from intense impulse noise.

Sixty patients with varying degrees of noise-induced hearing loss (NIHL) after long-term exposure to intense impulse noise from firearms, but without manifest clinical symptoms of vestibular pathology, were tested for body sway using a stable platform. The results were compared with those from 115 healthy referents examined in the same way. Subjects with NIHL showed significantly more body sway, estimated as movement of the centre of gravity in the horizontal plane, than did the referents. Subjects with more severe NIHL showed more sway than subjects with milder acoustic trauma. The results sh

Visual suppression test and ocular dysmetria: some electronystagmographic findings relevant to the assessment of size of cerebellopontine angle tumors.

Patients with cerebellopontine angle tumors underwent vestibular examination, testing of voluntary fast and slow conjugate eye movements and of visual-vestibular interaction (visual suppression test--VST) by electronystagmography (ENG). Both spontaneous and evoked nystagmus (Ny) and dysmetric alterations of the voluntary conjugate eye movements are relevant to cerebellopontine pathology. This study shows that ENG may reveal damage to the vestibular brain-stem and archicerebellar structures, the ENG signs of which very often precede the clinical signs of compression.
