United States Military Parachute Injuries: Part 2: Interventions Reducing Military Parachute Injuries in Training and Operations.

This is part 2 of an article detailing the reduction in airbornerelated injuries over time. Part 1 examined the early history of airborne operations and provided evidence for the reduction in injuries over time; part 2 discusses interventions associated with the decline in injury rates. In 1943 at the United States (US) Army Airborne School, data showed that injuries were substantially reduced from 120 to 18 injuries/1000 trainees. Credit for the reduction was given to development of the parachute landing fall (PLF), better supervision of students while in initial airborne training, intensive

Transport of extremely low birth weight neonates for persistent ductus arteriosus closure in the catheterization lab.

OBJECTIVE: The objective of this article is to describe the elements involved with transporting extremely low birth weight (ELBW) infants from referring centers to our center's neonatal intensive care unit (NICU) and then from the NICU to the catheterization lab for transcatheter closure of patent ductus arteriosus (PDA). SETTING: Several referring centers are over 300 miles away. ELBW infants are transferred in to our NICU safely for the procedure and transferred back following the procedure. A multidisciplinary team approach is necessary in order to achieve a safe transport of these fragile

Impacts of transportation sector emissions on future U.S. air quality in a changing climate. Part I: Projected emissions, simulation design, and model evaluation.

Emissions from the transportation sector are rapidly changing worldwide; however, the interplay of such emission changes in the face of climate change are not as well understood. This two-part study examines the impact of projected emissions from the U.S. transportation sector (Part I) on ambient air quality in the face of climate change (Part II). In Part I of this study, we describe the methodology and results of a novel Technology Driver Model (see graphical abstract) that includes 1) transportation emission projections (including on-road vehicles, non-road engines, aircraft, rail, and ship

Blood-Injection-Injury (B-I-I) Specific Phobia Affects the Outcome of Hypoxic Challenge Testing.

BACKGROUND: Blood-injection-injury (B-I-I) phobia is capable of producing inaccurate hypoxic challenge testing results due to anxiety-induced hyperventilation. CASE REPORT: A 69-yr-old woman with a history of hypersensitivity pneumonitis, restrictive spirometry, exercise desaturation requiring supplementary oxygen on mobilizing, reduced DLco, and B-I-I phobia was referred for hypoxic challenge testing (HCT) to assess in-flight oxygen requirements. HCT was performed by breathing a 15% FIo2 gas mixture, simulating the available oxygen in ambient air onboard aircraft pressurized to an equivalent

Railway noise annoyance and the importance of number of trains, ground vibration, and building situational factors.

Internationally accepted exposure-response relationships show that railway noise causes less annoyance than road traffic and aircraft noise. Railway transport, both passenger and freight transport, is increasing, and new railway lines are planned for environmental reasons. The combination of more frequent railway traffic and faster and heavier trains will, most probably, lead to more disturbances from railway traffic in the near future. To effectively plan for mitigations against noise and vibration from railway traffic, new studies are needed to obtain a better basis of knowledge. The main ob

Forces applied to the maxillary incisors by video laryngoscopes and the Macintosh laryngoscope.

BACKGROUND: Modern video laryngoscopes (VLSs) provide a superior view of the glottis, facilitating easier intubations. This study evaluates the forces applied to the maxillary incisors when using various VLSs and a Macintosh blade. METHODS: Fifty consecutive surgery patients were randomly assigned to receive laryngoscopy from a pair of four blades investigated in the study - the VLS GlideScope(®) (Verathon Inc., Bothell, WA, USA), V-Mac™ Storz(®) (Karl Storz, Tuttlingen, Germany), and McGrath™ (Aircraft Medical, Edinburgh, United Kingdom); and the classic Macintosh blade also from Storz(®) (K

A mathematical model of human respiration at altitude.

We developed a mathematical model of human respiration in the awake state that can be used to predict changes in ventilation, blood gases, and other critical variables during conditions of hypocapnia, hypercapnia and these conditions combined with hypoxia. Hence, the model is capable of describing ventilation changes due to the hypocapnic-hypoxia of high altitude. The basic model is that of Grodins et al. [Grodins, F. S., J. Buell, and A. J. Bart. J. Appl. Physiol. 22:260-276, 1967]. We updated the descriptions of (1) the effects of blood gases on cardiac output and cerebral blood flow, (2) ac

A drop-tower experiment to determine the threshold of gravity for inducing motion sickness in fish.

It has been repeatedly shown earlier that some fish of a given batch reveal motion sickness (a kinetosis) at the transition from 1 g to microgravity. In the course of parabolic aircraft flight experiments, it has been demonstrated that kinetosis susceptibility is correlated with asymmetric inner ear otoliths (i.e., differently weighed statoliths on the right and the left side of the head) or with genetically predispositioned malformed cells within the sensory epithelia of the inner ear. Hitherto, the threshold of gravity perception for inducing kinetotic behavior as well as the relative import

Retrieval of stratospheric aerosol size and composition information from solar infrared transmission spectra.

Infrared transmission spectra were recorded by the Jet Propulsion Laboratory MkIV interferometer during flights aboard the NASA DC-8 aircraft as part of the Airborne Arctic Stratospheric Expedition II (AASE II) mission in the early months of 1992. In our research, we infer the properties of the stratospheric aerosols from these spectra. The instrument employs two different detectors, a HgCdTe photoconductor for 650-1850 cm(-1) and an InSb photodiode for 1850-5650 cm(-1), to simultaneously record the solar intensity throughout the mid-infrared. These spectra have been used to retrieve the conce

Birds, migration and emerging zoonoses: west nile virus, lyme disease, influenza A and enteropathogens.

Wild birds are important to public health because they carry emerging zoonotic pathogens, either as a reservoir host or by dispersing infected arthropod vectors. In addition, bird migration provides a mechanism for the establishment of new endemic foci of disease at great distances from where an infection was acquired. Birds are central to the epidemiology of West Nile virus (WNV) because they are the main amplifying host of the virus in nature. The initial spread of WNV in the U.S. along the eastern seaboard coincided with a major bird migration corridor. The subsequent rapid movement of the

Behavioral sensitization following exposure to low doses of trimethylolpropane phosphate.

Behavioral sensitization is commonly studied within the context of drugs known to directly increase activity in the brain's dopamine system, particularly drugs of abuse. However, the present research suggests such behavioral changes can also be observed following exposure to other compounds that indirectly affect the dopamine system. One such compound is trimethylolpropane phosphate (TMPP), a bridged organophosphate that can be produced by the partial pyrolysis of certain synthetic lubricants used on military ships and aircraft. Although TMPP is a potent convulsant, it has been demonstrated th

Motion sickness in amphibians.

We explored the question of whether amphibians get motion sickness by exposing anurans (frogs) and urodeles (salamanders) to the provocative stimulus of parabolic aircraft flight. Animals were fed before flight, and the presence of vomitus in their containers after flight was used to indicate motion-induced emesis. None of the species that we studied vomited during the 8 to 10 parabolas of each flight. However, at least one specimen from each of the anuran species Rana rugosa, Rana nigromaculata, Hyla japonica, and Rhacophorus schlegelii vomited in a period of 0.5 to 42 h after flight. Some sp

Early Aeromedical Evacuation for Orbital Emphysema After Zygoma and Orbital Floor Fractures.

INTRODUCTION: Aeromedical evacuation of patients with post-traumatic orbital emphysema following zygoma and orbital floor fractures has often been viewed with considerable concern. The expansion of intra-orbital air with increasing altitude can theoretically result in orbital tension, which can lead to blindness. This concern is mainly based on extremely rare case reports and theoretical models rather than significant clinical experience. The purpose of this work is to evaluate hazards and complications associated with aeromedical evacuation of combat casualties with post-traumatic orbital emp

Risk of complications using a sedation protocol for aeromedical retrieval of acutely unwell mental health patients: a retrospective cohort study in Outback Australia.

BACKGROUND: Aeromedical transfer of acutely unwell mental health (AMH) patients presents potential risks to patient, staff and aircraft. Pharmacological options to reduce risk can impair consciousness, risking airway compromise and management challenges in-flight. Pre-emptive intubation carries associated patient risks and requires a receiving intensive care unit bed. This study aimed to assess the risk of complications using a protocolised approach to sedation of AMH patients undergoing retrieval in New South Wales, Australia. METHODS: This retrospective cohort study included all aeromedical

Experimental and Spectral Analysis of the Wake Velocity Effect in a 3D Falcon Prototype with Oscillating Feathers and Its Application in HAWT with Biomimetic Vortex Generators Using CFD.

The peregrine falcon, known as the fastest bird in the world, has been studied for its ability to stabilize during high-speed dives, a capability attributed to the configuration of its dorsal feathers. These feathers have inspired the design of vortex generators devices that promote controlled turbulence to delay boundary layer separation on aircraft wings and turbine blades. This study presents an experimental wind tunnel investigation of a bio-inspired peregrine falcon prototype, equipped with movable artificial feathers, a hot-wire anemometer, and a 3D accelerometer. Wake velocity profiles

Estimating CO2 emissions from international medical electives: a literature review and quantitative analysis.

INTRODUCTION: Electives are short placements during medical school lasting 2-8 weeks, serving as an opportunity to engage with different healthcare systems and cultures and to travel overseas. However, amid increasing alarm about climate change, interest in the sustainability of electives and alternative elective formats are gaining attention. METHODS: A scoping review of MEDLINE, Embase, ERIC, Web of Science SCOPUS, WHO Globus Index Medicus and Scielo was conducted with double-blind screening to identify previous efforts to quantify carbon costs of electives. To quantify the carbon dioxide (
