OBJECTIVE: To evaluate the surgical and functional outcome of congenital ptosis repair and the anisometropic changes after surgical repair of ptosis to determine the potential contribution of anisometropia to the development of refractive amblyopia.
METHODS: The clinical records of 162 children with congenital ptosis that had been surgically repaired between 1995 and 2006 at the Goldschleger Eye Institute were reviewed and analyzed for functional and cosmetic outcome, visual acuity status, and presence of amblyopia.
RESULTS: A total of 162 patients (mean age, 10 months) underwent surgical pt…

RESULTS: Significant clinical findings included a high incidence of refractive error in 94%, amblyopia in 60%, and strabismus in 40% of BPES cases. Orbital radiologic indices on CT scan in BPES were found to be comparable to the control group. On karyotyping, 8 out of 33 (24%) cases harbored chromosomal abnormalities. These abnormalities included 46, …

CONTEXT: Screening could identify preschool-aged children with vision problems at a critical period of visual development and lead to treatm … ge, although testability rates were generally lower in children 1 to 3 years of age. Treatments for amblyopia or unilateral refractive error were associated with mild improvements in visual acuity compared with no treatment. AIM: To evaluate the role of fixation in causing pterygium and determining its laterality and location.
METHODS: This is a prospective, observational, case-control study. Cases were defined as patients with primary pterygium who had unilateral amblyopia with eccentric fixation. Controls were age-matched patients with primary pterygium, but without amblyopia and eccentric fixation. All patients underwent complete ocular, orthoptic, and systemic examination and a detailed risk-factor assessment (latitude of residence, exposure to sunlight, sand, and a high-reflectance environment). The role of …

PURPOSE: Children with dense superficial opacities of the cornea are at risk for developing amblyopia. This study evaluated the efficacy of phototherapeutic keratectomy (PTK) in severe cases of anterior stromal scarring.
METHODS: Five eyes of five patients, aged 6 to 8 years, were included. Three children suffered from uveitis-associated band keratopathy, one child had anterior corneal stromal scarring due to viral infection, and one child had anterior basement membrane dystrophy. Phototherapeutic keratectomy was performed under general anesthesia using the 200 Hz ALLEGRETTO excimer laser. Op…

BACKGROUND: The management of children with oculomotor nerve palsy is complicated by their variable presentation, amblyopia, potential loss of binocularity, and associated neurological disease. Our purpose was to evaluate the causes, neurological associations, treatment, and sensorimotor outcomes of a group of children who developed oculomotor nerve palsy.
METHODS: We identified 18 children aged 14 years or younger, of whom 13 (72%) were less than 8 years old, through a retrospective record review of all children with oculomotor nerve palsy seen between January 1995 and January 2001 by one of…

PURPOSE: To analyze graft survival and visual outcome after penetrating keratoplasty in infants with congenital corneal opacity.
METHODS: W … CONCLUSION: Early penetrating keratoplasty for congenital corneal opacity may prevent deprivation amblyopia. Although this procedure carries a high risk of failure, particularly in those patients with Peter's anomaly, careful case selection, optical correction, and management of postoperative amblyopia may result in a successful visual outcome.

Multifocal intraocular lenses (MIOLs) have demonstrated efficacy and safety in adult cataract surgery, yet they encounter many challenges in pediatric applications. This article elaborates on the difficulties in biometric measurements in children, the unpredictability of postoperative refraction outcomes, the lack of long-term spectacle independence in children with MIOLs, the absence of significant advantages in correcting childhood amblyopia, and the potential increase in the rate of secondary surgeries. Due to the insufficient clinical evidence supporting the use of MIOLs in children at pre…

The rapid development of computer science over the past few decades has led to unprecedented progress in the field of artificial intelligence (AI). Its wide application in ophthalmology, especially image processing and data analysis, is particularly extensive and its performance excellent. In recent years, AI has been increasingly applied in optometry with remarkable results. This review is a summary of the application progress of different AI models and algorithms used in optometry (for problems such as myopia, strabismus, amblyopia, keratoconus, and intraocular lens) and includes a discussio…

The aim of this article is to report two cases of Straatsma syndrome, a rare disease characterized by the traditional triad of unilateral myelinated retinal nerve fibres, axial myopia, and amblyopia. The cases were a five-year-old and a three-year-old girl, both with unilateral myopia, visual acuity of 0.1 (decimal), and myelinated retinal nerve fibres in fundoscopy. Prescription of cycloplegic refraction findings and occlusion of the involved eye was attempted in both cases, but visual acuity did not improve. Poor-prognostic factors of Straatsma syndrome include a high degree of anisometropia…

Simple dacryops is a cystic lesion of the ductal structures of the palpebral lobe of the lacrimal gland and is the commonest lacrimal ductal cystic lesion in adults. We report the case of an 18-month-old girl with a painless swelling of the right temporal upper eyelid that was first noted at 4 months of age. An early surgical excision was planned to avoid deprivational amblyopia. We describe clinicopathological and immunohistochemical correlations in our case, highlighting the immuno-characterization of adjacent lacrimal gland duct and acinar structures present within the cyst wall and managem…

Torticollis is an abnormal head posture caused by ocular or non-ocular factors. Ocular torticollis (OT) is a compensatory head posture adopted to avoid double vision or dampen nystagmus. In this review, we argue, that causes for OT comprise a wide range of congenital or acquired ocular conditions. These patients are at risk of being mis-cat-egorised as having non-ocular torticollis, which may delay treatment and cause amblyopia in children. OT can be symptomatically relieved by prismatic correction incorporated in the patient's glasses or treated by strabismus surgery, which also improve ocula…

BACKGROUND: Congenital cataract is one of the major causes of blindness and amblyopia in children. About one-third of the cases are inherited.
CASE PRESENTATION: We applied whole exome sequencing for a pediatric patient with congenital cataract, small penis, baldness and absence of eyebrows and detected a compound heterozygous mutation in the lanosterol synthase (LSS) gene. These two mutations were inherited from the patient's parents. Both mutations altered the amino acid coding, at highly conserved amino acid residues.
CONCLUSIONS: We concluded that the mutations affect the structural stab…

The management of a monocular congenital cataract is dominated by the problems of amblyopia, always present. The paper shows the case of an eight years old child, who was operated when he was 6 month of age for monocular congenital cataract. After YAG-Nd treatment of secondary cataract, aerian correction and intense pleoptic and orthoptic treatment, the visual acuity rise up to 1/4, when was possible to made an IOL implantation. We consider that is important to try to solve the monocular congenital cataract, even at higher ages, because the results is possible to be satisfactorily.

This study evaluated the ophthalmological outcome following sixth nerve palsy or paresis in 64 children 7 years of age and younger. The outcomes considered were vision, residual strabismus and the need for strabismus surgery. Etiologies included tumor, hydrocephalus, trauma, infection, malformation, and idiopathic and miscellaneous causes. Strabismus surgery was performed on 24% of the patients, with residual strabismus present in 66% of the patients. Neurologists and ophthalmologists should monitor visual acuity in these young children at frequent intervals be prepared to institute amblyopia …
