A case report describing diverticulosis of the appendix presenting as acute appendicitis.

INTRODUCTION: Diverticulosis of the appendix (DA) is a rare clinical finding which is often confused with acute or chronic appendicitis and is usually only identified during or after appendectomy. The symptoms of DA can last for up to two weeks and laboratory studies tend to reflect a more chronic inflammation. Distinguishing the two entities is important as DA has a higher risk for perforation and may be associated with an underlying malignancy. PRESENTATION OF CASE: A 54--year old African--American male presented with three--days of right sided abdominal pain, nausea, and vomiting. Physical…

Oral antibiotics in the management of perforated appendicitis in children.

After appendectomy for perforated appendicitis children have traditionally been managed with intravenous broad-spectrum antibiotics for 5 to 10 days and then until fever and leukocytosis have resolved. We prospectively evaluated a protocol of hospital discharge on oral antibiotics when oral intake is tolerated-regardless of fever or leukocytosis-in a consecutive series of 80 children between one and 15 years of age who underwent appendectomy (38 open and 42 laparoscopic) for perforated appendicitis. At discharge subjects began a 7-day course of oral trimethoprim/sulfamethoxazole and metronidaz…

Barium Appendicitis 6 Weeks After Upper Gastrointestinal Imaging.

Barium sulfate is widely used for gastroenterology imaging. Retention of barium in the appendix, where it acts as an appendicolith, thereby leading to obstruction and inflammation. Barium-associated appendicitis is a very rare complication of upper gastrointestinal imaging (UGI), especially in children. We present a case involving an 8-year-old girl who was diagnosed with acute appendicitis due to a barolith and required a laparoscopic appendectomy 6 weeks after UGI. After UGI, patients should be informed regarding possible retention of barium in the appendix, which can cause acute appendiciti…

Is the laparoscopic approach appropriate for pediatric subhepatic appendicitis?

INTRODUCTION: Subhepatic appendicitis in children is an unusual condition that can be challenging for the pediatric surgeons to treat. The aim of our study was to compare the outcomes of laparoscopic appendectomies based on the position of the appendix. METHODS: The data of 1736 patients who had undergone laparoscopic appendectomy in our tertiary center were retrospectively reviewed. We compared two groups: subhepatic location (n = 56) and non-subhepatic location (n = 1680). A P-value of less than 0.05 was considered statistically significant. RESULTS: There was no statistically significant …

Incidental Amyand's Hernia During Enhanced-View Totally Extraperitoneal (eTEP) Repair of Recurrent Inguinal Hernia: A Case Report and Technical Rationale for a Posterior Approach.

Incidental Amyand's hernia is a rare intraoperative finding during inguinal hernia repair and may pose a management dilemma, particularly in recurrent cases where prior anterior repair limits surgical options. In these settings, careful consideration is required for optimal plane selection and the safety of mesh placement after appendectomy. A 67-year-old man with a history of prior anterior Lichtenstein repair presented with a recurrent right inguinal hernia. A posterior laparo-endoscopic repair using the enhanced-view totally extraperitoneal (eTEP) technique was performed in accordance with …
