Pediatric Intracranial Abscesses in a Tertiary Care Center: Antimicrobial Management, Diagnostic Yield of Molecular Testing and Radiologic Follow-up.

BACKGROUND: Brain abscesses and intracranial empyemas are rare but serious infections associated with substantial morbidity and long-term neurologic sequelae. Microbiologic confirmation is often limited by prior empirical therapy, and the diagnostic yield of real-time polymerase chain reaction in real-world settings remains uncertain. Evidence guiding antimicrobial duration, microbiologic work-up and radiologic follow-up in children is limited. We aimed to describe a pediatric cohort, focusing on management strategies, molecular diagnostics and radiologic follow-up. METHODS: In this single-ce…

Intracranial complications of frontal sinusitis.

Frontal sinus infections can spread to the intracranial space so fast that the clinical situation often becomes far advanced before a complication is recognized. Retrograde septic thrombophlebitis is the most common pathway of extension. A review of recent experiences with fulminating frontal sinusitis and its intracranial complications such as subdural empyema, brain abscess, epidural abscess, and meningitis is presented. Neurologic features of intracranial invasion are interpreted. Good results have been achieved by immediate and aggressive surgical and medical measures.

Diffusion-weighted and conventional magnetic resonance imaging in cerebral cryptococcoma.

A case of cryptococcoma involving the right cerebral hemisphere is reported. The diffusion-weighted image shows hypointensity in the central cavity of the cryptococcoma, while apparent diffusion coefficient maps show hyperintensity. The imaging features of an intracerebral cryptococcoma mimic that of a central necrotic brain tumor, rather than a pyogenic brain abscess.

Intercostal Muscle Abscesses in Infective Endocarditis Associated With Migratory Deposition of Calcium Pyrophosphate.

Infective endocarditis (IE) is caused by vegetations, consisting of platelets, fibrin, inflammatory cells, and microcolonies of bacteria, fungi, rickettsia, chlamydia, and viruses, that form in the heart valves, endocardium, and large vessel intima. Staphylococcus aureus endocarditis is highly tissue destructive, usually follows an acute course, and tends to become severe due to valve destruction, surrounding abscesses, and distant seeding. The main complications of IE due to S. aureus are heart failure due to destruction of tendon cords and valves, perivalvular abscesses and fistulas, and the

[A case of cerebral abscess due to methicillin-resistant Staphylococcus aureus which is treated with linezolid + rifampin combination].

Methicillin-resistant Staphylococcus aureus (MRSA) is a rare cause of cerebral abscesses, however it is a relatively more common etiologic agent in post-neurosurgical abscesses and the main antibacterial therapy option is vancomycin. In this report, a case of brain abscess due to MRSA which did not respond neither to moxifloxacin + vancomycin nor vancomycin + rifampin combination therapies, and merely treated by linezolid + rifampin combination, has been presented. Fifty-one years old female patient who was operated 40 days ago for subarachnoid bleeding and aneurysm in middle cerebral artery b…

Cerebral thrombosis and abscess: A child with 2 rare complications of tetralogy of Fallot.

Tetralogy of Fallot is the most common cyanotic congenital heart disease. Rare cerebrovascular complications include brain abscess and cerebral thrombosis. Herein, we discuss a case of uncorrected tetralogy of Fallot presenting with cerebral thrombosis and multiple brain abscesses in a 1-year-and-8-month-old toddler. This patient, who was a known cardiac patient (uncorrected tetralogy of Fallot on follow-up since the age of 4 months of age), presented with a change in mentation, high-grade intermittent fever to the level of 39°C and generalized tonic-clonic type of abnormal body movement. On i…

Optic Pathway Abscess Caused by Clostridium septicum Endophthalmitis From Colon Adenocarcinoma Requiring Incision and Drainage of Optic Nerve and Chiasm.

BACKGROUND AND IMPORTANCE: Optic pathway abscess formation is a rare clinical entity. To date, there are no reports of optic pathway abscess formation due to Clostridium septicum, secondary to colon adenocarcinoma. We present a video case report of a unique patient found to have C. septicum optic pathway abscess requiring craniotomy for incision and drainage of optic nerve/chiasm. CLINICAL PRESENTATION: A 62-year-old woman presented with acute-onset right eye vision loss and rapidly progressive proptosis, chemosis, elevated intraocular pressure, and ophthalmoplegia requiring emergent canthoto

Delayed cerebellar abscess developed 38 years after craniocerebral gunshot wound: A case report.

BACKGROUND: Brain abscesses are recognized complications of penetrating craniocerebral trauma, typically developing within 6-8 weeks following the initial injury. Delayed onset abscesses have been occasionally reported, with extremely few cases occurring decades after the trauma. The pathophysiological mechanisms behind such scenarios remain poorly understood, highlighting the need for heightened clinical suspicion in patients with a history of head injury who present with new neurological symptoms, regardless of the time elapsed. CASE DESCRIPTION: We present the case of a 68-year-old male wi…

Subdural Abscess Suspected to Have Developed From Acute Sinusitis via the Foramen Cecum: A Case Report.

The foramen cecum connects the intracranial space to the sinus cavities. However, it is usually blind-ended and typically closed by surround … cum. Endoscopic sinus surgery was performed in our department to control the infection, followed by brain abscess drainage in the neurosurgery department. The foramen cecum is an anatomical feature that can connect the inside and outside of the skull, increasing the risk of infections spreading to the brain.

Pulmonary arteriovenous malformation associated with necrotizing fasciitis of the thigh.

Pulmonary arteriovenous malformation (PAVM) is a rare condition often associated with hereditary hemorrhagic telangiectasia. The association between PAVMs and brain abscesses has been previously reported, but abscesses at other locations are extremely rare. We present a 51-year-old woman with PAVM (without signs of hereditary hemorrhagic telangiectasia) and necrotizing fasciitis of her thigh and discuss the possible relationship between the 2 disorders.

Into the unknown: Diagnosing mysterious brain lesions.

In this inaugural clinicopathological conference, the invited experts discussed the diagnostic approach to central nervous system infections in immunocompromised hosts. The case presented involved a pancreas-kidney transplant recipient with multiple brain abscesses caused by Bartonella henselae. CSF metagenomic next-generation sequencing played a significant role in the diagnosis. Bartonella henselae is a gram-negative zoonotic pathogen that causes cat-scratch disease, which can be transmitted to humans through cat bites or scratches. Symptoms can vary in severity, correlating with the patient…

Abscesses due to Melioidosis: A case-based review.

Melioidosis is caused by percutaneous inoculation or inhalation of Burkholderia pseudomallei, predominantly among individuals with risk factors (diabetes mellitus, immunosuppression, etc.) from endemic areas of South Asia, Southeast Asia and Northern Australia. While some patients present acutely with sepsis and multi-organ failure, others present with a subacute to chronic course characterised by abscess formation. We present nine representative cases, each with an abscess at a separate site (lung, skin, bone/ joint, prostate, parotid gland, liver, spleen, brain and orbits). Using these cases
