BACKGROUND: The optimal treatment of femoral neck fracture in the elderly patient is still under debate. In patients aged 60-80 years, the decision between internal fixation and arthroplasty remains controversial. The primary aim of the present study is to evaluate the functional outcome of patients aged 60-80 years with femoral neck fracture treated with total hip arthroplasty or closed reduction and internal fixation. The secondary aim is to evaluate the incidence of nonunion and avascular necrosis in femoral neck fracture in different age groups.
MATERIALS AND METHODS: We studied 100 patie

A 56-year-old man, right-hand-dominant office worker, complained of pain and swelling at the base of his right thumb after using his hand to press onto the front passenger seat during an emergency brake. X-ray showed a dorsal dislocation of the first carpometacarpal joint of his right hand. Closed reduction of the joint was performed. As there were no clinical signs of instability post-reduction and X-ray confirmed that the joint was congruent, the joint was immobilised in a thumb spica splint for 6 weeks. His pain subsided and the range of motion of his first carpometacarpal joint was full at

Forty-eight of fifty-four patients who had had closed reduction and percutaneous pinning of an unstable fracture of the proximal end of the humerus were available for clinical and roentgenographic follow-up at an average of three years (range, two to seven years) after the operation. According to the point-scale of Saillant et al., the result was good or excellent in thirty-four patients, fair in ten, and poor in four. Four patients had loss of fixation and had repeat fixation with percutaneous pinning after a second closed reduction. Only one of them had a poor result because of malunion. Fou

AIMS: This study describes and compares the operative management and outcomes in a consecutive case series of patients with dislocated hemiarthroplasties of the hip, and compares outcomes with those of patients not sustaining a dislocation.
PATIENTS AND METHODS: Of 3326 consecutive patients treated with hemiarthroplasty for fractured neck of femur, 46 (1.4%) sustained dislocations. Of the 46 dislocations, there were 37 female patients (80.4%) and nine male patients (19.6%) with a mean age of 83.8 years (66 to 100). Operative intervention for each, and subsequent dislocations, were recorded. T

Repairing Pilon fractures remains challenging. ORIF allows direct anatomical reduction, but at the expense of soft tissues dissection which are associated with recovery. On the other hand, External Fixation allows indirect reduction and causes less soft tissue damage. However, a few studies conclude that External Fixation is associated with high rates of malunion. The objectives were to evaluate and compare: primary outcome measure: ankle hindfoot function (AOFAS at 9 months) and secondary outcome measures: quality of reduction, bone union, arthritic changes, other potential complications and u

BACKGROUND: Pediatric radial neck fractures, though rare, pose significant management challenges with several treatment options. Open reduction is typically used for more complex cases, where greater fracture severity may contribute to higher complication rates and poorer outcomes. This study aims to evaluate functional outcomes, complications, and risk factors in a large cohort of pediatric patients treated with open reduction, performed after unsuccessful closed reduction.
METHODS: A retrospective cohort study was conducted on 53 pediatric patients (mean age 8 years) with radial neck fractu

BACKGROUND: Ultrasound-guided supraclavicular block (UGSCB) is an emerging technique gaining interest amongst emergency physicians that provides regional anaesthesia to the upper limb to tolerate painful procedures. It offers an alternative to the more traditional technique of a Bier block (BB). However, the effectiveness or safety of UGSCB when performed in the emergency department (ED) is unclear.
METHODS: SUPERB (SUPraclavicular block for Emergency Reduction versus Bier block) is a prospective open-label non-inferiority randomised controlled trial comparing the effectiveness of UGSCB versu

Forearm fractures are common injuries in the pediatric population. Successful treatment of these fractures should result in complication-free functional pronosupination. Traditionally, these have been treated with closed reduction and casting, but the last several decades have seen a dramatic increase in the operative treatment of these fractures. However, little high-level evidence exists to guide management. The data from the limited set of studies available suggest that closed treatment does usually result in satisfactory outcomes, particularly in younger patients; operative fixation is usu

The Galeazzi fracture is an unstable fracture-dislocation of the forearm. There have been reports of non-union of the radius despite rigid internal fixation with a plate. We present the case of a 25-year-old male who fell from his bicycle, sustaining a closed Galeazzi fracture-dislocation. Definitive surgical fixation involved internal fixation using a six-hole dynamic compression plate. Post-operatively, the patient noted a significant reduction in pronation. The fracture had united but with approximately 5 mm of radial shortening. The operating consultant formulated a surgical plan to resolv

RATIONALE: Lunate fractures are very rare, accounting for only 0.5% to 6.5% of all carpal fractures. They are mostly caused by high-energy trauma and commonly occur with fractures and dislocations of other carpal bones. It is rarely combined with Colles' fracture and the underlying pathogenesis and prognostic significance still remain controversial.
PATIENT CONCERNS: Here we report a case of a fresh lunate fracture combined with Colles' fracture, without dislocations of other carpal bones caused by a low-energy fall injury.
DIAGNOSES: Lunate fracture, Colles' fracture.

Limb fractures are a large part of pediatric trauma activity. Conservative treatment is possible because of children's bone remodeling potential. In case of displaced fractures, when a closed reduction can be done in the emergency room (ER), this avoids general anesthesia, hospitalization and the associated costs. In well-defined situations, there is a consensus about the indication for fracture reduction in the ER. Some complex fracture cases require immediate treatment in the operating room: intra-articular fractures, pathological fractures, fractures with associated skin, nerve or vascular

UNLABELLED: Salter-Harris type III (SH-III) distal femur fractures are challenging to manage given their intra-articular nature and physeal involvement. Inadequate reduction of these injuries can potentiate growth disturbances, including premature physeal closure and subsequent deformity. Existing literature lacks a description of a standardized reduction technique for this fracture pattern. In this report, we use a cadaver knee to demonstrate a structured reduction maneuver for the SH-III medial femoral condyle fracture, designed to facilitate anatomic alignment of the epiphyseal fragment via

OBJECTIVE: Thoracolumbar traumatic spondylolisthesis is a relatively rare phenomenon and has poor prognosis due to serious spinal cord or cauda equina injuries. In such cases, closed reduction is a method for restoring the vertebral sequence and may play an important role in the treatment process, although whether it is actually feasible for patients with this condition requires further investigation. The present study included 9 patients with serious thoracolumbar traumatic spondylolisthesis to determine the advantages of closed reduction over total reduction through open surgery.

OBJECTIVE: The optimal timing of pin removal following closed reduction and percutaneous pinning (CRPP) for pediatric Gartland type 3 supracondylar humerus fractures remains controversial. Although pins are commonly removed between the third and fifth postoperative weeks, high-level evidence comparing clinical and radiological outcomes between these time points is limited. This study aimed to evaluate whether pin removal at the third vs. the fifth postoperative week influences radiographic healing or functional outcomes.
METHODS: This retrospective observational study included 95 pediatric pa

Dorsolateral dislocation of the proximal interphalangeal (PIP) joint is a common injury to the hand. Closed reduction of the dislocation anatomically realigns the avulsed ligaments; thus patients may be managed non-operatively. Standard treatment involves placement of a dorsal splint to prevent hyperextension and lateral stresses. This allows early active motion of the PIP joint while preventing a flexion contracture. In this fresh cadaver study, the PIP joint in 24 fingers was dorsolaterally dislocated. Four digits had to be excluded from the investigation due to a fracture dislocation with a
