The value of short-term peripheral parenteral nutrition after colorectal surgery: a comparative study with conventional postoperative intravenous fluid.

The potential beneficial effect of postoperative peripheral parenteral nutrition (PPN) was assessed in ten patients undergoing colorectal surgery and compared with the conventional use of intravenous fluids in ten controls. Clinical and biological parameters were checked. A significant improvement in the nitrogen-balance, which was not positive during the first postoperative days was observed. All other daily measured parameters (lipid metabolism, electrolytes, glucose levels and plasma pH) remained within normal ranges. In our opinion, short-term postoperative PPN may lessen protein losses an…

Association Between Fecal Contamination and Outcomes After Emergent General Surgery Colorectal Resection: A Post Hoc Analysis of an Eastern Association for the Surgery of Trauma (EAST) Multicenter Study.

Background: The impact of fecal contamination on clinical outcomes in patients undergoing emergent colorectal resection is unclear. We hypothesized that fecal contamination is associated with worse clinical outcomes regardless of operative technique. Patients and Methods: This is a post hoc analysis for an Eastern Association for the Surgery of Trauma-sponsored multicenter study that prospectively enrolled emergency general surgery patients undergoing urgent/emergent colorectal resection. Subjects were categorized according to presence versus absence of intra-operative fecal contamination. Pro

Short-term outcomes of minimally invasive surgery in older colorectal cancer patients in the era of enhanced recovery after surgery: is a "one-size-fits-all" strategy sufficient?

BACKGROUND: An enhanced recovery protocol (ERP) comprises a series of elements aimed at optimizing and standardizing perioperative care. Therefore, in this study, we aimed to evaluate the safety and feasibility of a modified enhanced recovery after surgery (ERAS) protocol following colorectal surgery in older adults aged ≥ 65 years. MATERIALS AND METHODS: Patients aged ≥ 65 years who underwent minimally invasive colorectal cancer surgery at a tertiary referral hospital in Taiwan between 2018 and 2022 were reviewed retrospectively. Patients were divided into ERAS and traditional care groups ac…

Self-expandable metal stents as a bridge to surgery for obstructive colorectal cancer: a propensity score-matched retrospective cohort study.

PURPOSE: Acute colorectal obstruction constitutes a surgical emergency in patients with colorectal cancer (CRC) and is associated with high morbidity and mortality when managed with emergency surgery. Although self-expandable metal stents (SEMS) may optimize surgical conditions, concerns remain regarding their long-term oncologic safety. METHODS: This retrospective cohort study included 355 patients with obstructive CRC treated between 2014 and 2023, including 65 patients in the SEMS group and 290 in the surgery-alone group. Perioperative outcomes, postoperative complications, disease-free su

Spinal anesthesia in laparoscopic colorectal surgery: analgesia and recovery outcomes - a cohort study and systematic review of randomized controlled studies.

INTRODUCTION: Spinal anesthesia has been increasingly discussed as an adjunct to general anesthesia in laparoscopic colorectal surgery due to its potential to enhance early analgesia and reduce opioid exposure. Whether these benefits translate into improved postoperative bowel recovery remains unclear, and existing evidence is limited by heterogeneous study designs and small sample sizes. This study combines prospective cohort data with a systematic review and meta-analysis of randomized controlled trials to evaluate the effects of spinal anesthesia on pain, opioid use, and bowel function. Institution-level framework to estimate the impact of BMI on operative decision-making in patients undergoing colorectal surgery in a provincial cohort.

BACKGROUND: In colorectal surgery, a higher body mass index (BMI) is associated with increased technical difficulty, reduced use of minimally invasive surgery (MIS), and higher rates of conversion to open. While these associations are well established, less is known about how they vary across systems and providers. The study presents a methodological framework that evaluates the impact of obesity on operative approach and conversion risk in colorectal surgery and compares these associations between a provincial Canadian cohort (Nova Scotia (NS)) and National-Surgical-Quality-Improvement-Progra…

Reduction of surgical site infection rates in elective colorectal surgery by means of a nationwide interventional surveillance programme. A cohort study.

INTRODUCTION: Colorectal surgery has the highest surgical site infection (SSI) rates of all abdominal surgeries. Epidemiological surveillance is an excellent instrument to reduce SSI rates, but its effects may be time-limited and need to be monitored periodically. This study analyses the effectiveness of an interventional surveillance programme with regard to reducing SSI rates after elective colorectal surgery. METHODS: Cohort study analysing a SSI surveillance programme in elective colorectal surgery over a 15-year period. Prospectively collected data were stratified by 5-year periods (Peri…

Effect of oral antibiotic bowel preparation versus no preparation on surgical site infections in elective colorectal surgery: a randomized trial.

PURPOSE: Surgical site infections (SSIs) represent a significant cause of morbidity following colorectal surgery. While mechanical bowel preparation combined with oral antibiotics is known to reduce SSIs, the independent effect of oral antibiotics alone remains unclear. This study compared the efficacy of oral antibiotic bowel preparation (OABP) versus no bowel preparation (NBP) in reducing SSIs among patients undergoing elective colorectal surgery. METHODS: A prospective, randomized, double-blinded trial was performed at a tertiary care center in India. Eighty-six patients scheduled for elec…
