Brachialis muscle contracture as a cause of lameness in cats: A study of four cases.

Three cats (four cases) were presented to two hospitals in New Zealand with limited extension of the elbow, and a nonpainful, firm mass on the craniolateral aspect of the distal humerus. Findings in all cases were consistent with brachialis muscle contracture. In one case, there was also contracture of the biceps brachii muscle. Ultrasound imaging (used in three cases) showed a heterogeneous, increased echogenicity of the muscle. The histopathology results were obtained in three cases; two were diagnosed as fibrosis of the muscle, one as a soft tissue sarcoma. All cases were treated surgically…

A New Technique of Brachioradialis Tendon Rerouting Combined With Osteotomy in Contractures of the Forearm.

Rotational deformities of the forearm occur due to neurological imbalance of pro/supinating forces, commonly seen in brachial plexus birth injury. Brachioradialis rerouting has been described for the correction of active pronation or supination deficits. Fixed deformities cannot be addressed by soft tissue procedures alone, and need derotational osteotomies. More recently, techniques combining biceps rerouting and forearm osteotomy have been described. We present an alternative technique to correct fixed rotational forearm deformities by derotational forearm osteotomy and brachioradialis tendo

FBN2 pathogenic variants in congenital contractural arachnodactyly with severe cardiovascular manifestations.

PURPOSE: Congenital contractural arachnodactyly (CCA) is an extremely rare autosomal dominant connective tissue genetic disorder caused by pathogenic variants in FBN2. CCA is characterized by arachnodactyly, camptodactyly, contracture of major joints, scoliosis, pectus deformities, and crumpled ears, but rarely with lethal cardiovascular manifestations as in Marfan syndrome. It is imperative to conduct a comprehensive analysis and review of the pathogenesis of CCA resulting from pathogenic variants in FBN2 gene. MATERIALS AND METHODS: Using whole-exome sequencing and Sanger sequencing, we ide…

A 10-Year Review of Collagenase Versus Fasciectomy in the Treatment of Dupuytren Contracture.

BACKGROUND: Dupuytren disease (DD) is one of the most common disorders of the hand, affecting 5.7% to 11.7% of the global population. This study seeks to evaluate the 10-year efficacy of the 2 most prominent treatment modalities for DD in Veterans Affairs hospitals, injectable collagenase Clostridium histolyticum versus open fasciectomy. METHODS: A retrospective review was conducted of all electronic medical records of patients who underwent open fasciectomy or collagenase injection to treat their persistent Dupuytren contracture between April 2011 and April 2021. All procedures were performe…

Pediatric Spastic Wrist Contractures Can Be Well Managed With Wrist Arthrodesis.

BACKGROUND: Severe spastic wrist contractures secondary to cerebral palsy (CP) or alike can have significant implications for patient hand function, hygiene, skin breakdown, and cosmesis. When these contractures become rigid, soft tissue procedures alone are unable to obtain or maintain the desired correction. In these patients' wrist arthrodesis is an option-enabling the hand to be stabilized in a more functional position for hygiene, dressing, and general cosmesis, though are patients satisfied? METHODS: All children who had undergone a wrist arthrodesis for the management of a severe wrist…

Considerations in Determining the Severity of Burn Scar Contractures With Focus on the Knee Joint.

Scar contractures are a common complication after burn injuries. These contractures are characterized by impairment of joint mobility, leading to a risk for limitations during daily activities, and restrictions in participation in society. Qualifying its severity is not well established in burn care. This study, therefore, examined different approaches to determine the severity of limited mobility in the knee joint due to scar contracture. To determine the severity of burn scar contractures development of the knee over time, the following approaches were analyzed: prevalence, the degree of lim…

Glenohumeral abduction contractures after residual neonatal brachial plexus injury.

Glenohumeral abduction contractures are common in patients with neonatal brachial plexus injury, but little has been previously published about them. We conducted a retrospective analysis of data prospectively collected from 205 consecutive children (108 female) of mean age 9.6 years with neonatal brachial plexus injury (C5-C6, 58%; C5-C7, 29%; C5-T1, 14%). Most children (69%) showed a glenohumeral abduction contracture, it being more common in those with upper neonatal brachial plexus injury.Level of evidence: III.

Congenital contractural arachnodactyly suspected by abnormally long extremities by fetal ultrasound.

Congenital contractural arachnodactyly (CCA) is a rare disease with the clinical features of limited extension of multiple joints, arachnodactyly, camptodactyly, thin and long extremities, and so on. In the point of long extremities, CCA resembles Marfan syndrome (MFS). CCA is easily differentiated from MFS after birth due to the flexion of multiple joints, including elbows, knees, hips and fingers. During the fetal period, observation of arachnodactyly and folded fingers by fetal ultrasound is the means of differential diagnosis between these two diseases. We report on a case of CCA diagnosed

Relationship between Severity of Varus Osteoarthritis of the Knee and Contracture of Medial Structures.

BACKGROUND: Knees with severe varus osteoarthritis can develop medial structure contracture. However, there is no report on the relationship between severity of varus deformity and contracture of the medial structure. We aimed to determine the threshold angle that could be corrected in proportion to the width of medial osteophyte removal and to examine correction differences between angles larger and smaller than the threshold angle in total knee arthroplasty. METHODS: This study included 27 varus osteoarthritic knees scheduled for total knee arthroplasty (TKA). A navigation system was used t…

Dystonia and Contractures are Potential Early Signs of CACNA1E-Related Epileptic Encephalopathy.

Epileptic encephalopathy related to CACNA1E has been described as a severe neurodevelopmental disorder presenting with early-onset refractory seizures, hypotonia, macrocephaly, hyperkinetic movements, and contractures and is associated with an autosomal dominant inheritance pattern. Most pathogenic variants described to date are missense variants with a gain of function effect, and the role of haploinsufficiency has yet to be clarified. We describe 2 cases of CACNA1E encephalopathy. Notable findings include congenital contractures and movement disorders predating onset of epilepsy, particularl…

An unusual case of eosinophilia, myalgia and skin contractures: Shulman's disease revisited.

Shulman's disease (eosinophilic fasciitis) is a very rare autoimmune disorder with an unknown etiopathogenesis. During the initial period of … s, fever, and peripheral blood eosinophilia followed by rapidly evolving skin tightening with joint contractures and muscle stiffness, which misled the treating team toward Scleroderma and its overlap syndromes. The diagnosis was finally clinched by a full-thickness skin biopsy along with underlying fascia and muscle tissue from an effected area, with a grati …

The Collagenase of the Bacterium Clostridium histolyticum in the Treatment of Irradiation-Induced Capsular Contracture.

BACKGROUND: Irradiation therapy is an important pillar in the treatment of breast cancer. However, it can trigger capsular fibrosis, the most significant complication of implant-based breast reconstruction. As collagen is the main component of fibrotic capsules, the collagenase of the bacterium Clostridium histolyticum poses a potential treatment option for this pathological condition. METHODS: Thirty-six rats received miniature silicone implants on their backs. On day 1, the implant sites of two groups were irradiated with 10 Gy. On day 120, one irradiated group received collagenase injectio

Contact pressure of the coracoacromial arch in shoulders with joint contracture: A cadaveric study.

BACKGROUNDS: There have been many reports describing that the capsular fibrosis of the shoulder joint is the main cause of frozen shoulder, whereas others reported the significance of subacromial impingement as an etiological factor. The purpose of this study was to investigate the contact pressure between the coracoacromial arch and the rotator cuff tendons to clarify the contact phenomenon in shoulders with joint contracture. METHODS: Fourteen fresh-frozen cadaveric shoulders were used. Specimens were divided into two groups: normal group (8 shoulders, definition: more than 61° of flexion a…
