Acute and chronic megacolon.

Megacolon, defined as dilation of the abdominal colon, may occur acutely or in a chronic form. Acute megacolon that occurs in association with severe inflammation of the colon is known as toxic megacolon, whereas acute megacolon without obvious colonic disease is known as Ogilvie's syndrome. The pathophysiology and management of toxic megacolon, Ogilvie's syndrome, and chronic megacolon in adults differ significantly, and it is critically important to distinguish among these entities. Toxic megacolon is a medical emergency that requires coordinated intensive medical and surgical management. In

A novel endoscopic posterior cervical decompression and interbody fusion technique: Feasibility and biomechanical analysis.

BACKGROUND AND OBJECTIVE: Cervical decompression and fusion, the primary surgical techniques for treating degenerative cervical myelopathy, is traditionally performed using interbody fusion through an anterior approach. There are no reported cases of cage placement performed via a posterior cervical approach under endoscopy. This study investigates a novel posterior interbody fusion technique using a newly designed split cage and validates its feasibility through computer simulations. METHODS: Anatomical parameters of the posterior cervical safe area (PCSA) were analyzed, and a split interbod

Posterior Cervical Foraminotomy Using Preoperative Magnetic Resonance Imaging Combined with Intraoperative Real-Time Ultrasonography: Technical Note and Initial Experience.

BACKGROUND: Posterior cervical foraminotomy for cervical spondylotic radiculopathy requires precise neural decompression while maintaining facet joint integrity. Ideally, the achievement of appropriate nerve root decompression should be confirmed using intraoperative computed tomography or other imaging modalities, although this is not generally done because of the need to avoid radiation exposure and/or because the procedure is very widely performed, even at facilities without expensive equipment. Magnetic resonance imaging (MRI)-ultrasound (US) fusion imaging has the potential to provide a s

Role of Decompressive Craniectomy in Ischemic Stroke.

Ischemic stroke is one of the leading causes for death and disability worldwide. In patients with large space-occupying infarction, the subsequent edema complicated by transtentorial herniation poses a lethal threat. Especially in patients with malignant middle cerebral artery infarction, brain swelling secondary to the vessel occlusion is associated with high mortality. By decompressive craniectomy, a significant proportion of the skull is surgically removed, allowing the ischemic tissue to shift through the surgical defect rather than to the unaffected regions of the brain, thus avoiding sec

Osteonecrosis of the Femoral Head: Core Decompression Systems.

Osteonecrosis of the femoral head (ONFH) is a chronic progressive debilitating disease that often affects young and active patients. It results from vascular interruption to the femoral head and can be caused by trauma, chronic corticosteroid use, chronic alcoholism, and coagulopathies. Treatment includes core decompression, a surgical procedure that may help delay or forestall disease progression if performed at the early stages of the disease. Technological advances have made the procedure more efficient and effective. There are several core decompression systems available in the market, giv

Interventional EUS guided cholangiography. First description in Mexico of a novel, secure and feasible technique. A case report.

Endoscopic retrograde cholangio pancreatography (ERCP) is the procedure of choice for biliary decompression in individuals with obstructive jaundice. In those cases when ERCP fails, the alternatives are to refer the case to a highly specialized center and repeat the procedure, or decompress the bile ducts either percutaneously or surgically. The proximity of the endoscopic ultrasound (EUS) probe from the stomach and duodenum to the hepatobiliar system and pancreas allows ultrasound-guided access into the bile and pancreatic duct. We present what to our knowledge is the first description in Mex

The Hidden Dangers of Decompressing Ameloblastomas: Evaluating Risks, Recurrence, and Treatment Outcomes.

BACKGROUND: Decompression of conventional (previously the multicystic/solid type) ameloblastoma remains controversial. Ameloblastomas are locally aggressive and have high recurrence rates after decompression, a treatment approach that requires critical evaluation. PURPOSE: To compare time to definitive treatment between subjects with conventional ameloblastoma initially managed with decompression and those treated with immediate resection. STUDY DESIGN, SETTING, SAMPLE: Retrospective cohort study conducted at the University of Illinois Chicago from 2007 to 2024. The study sample included sub

Comparison between Decompressive Craniectomy with Durotomy and Conservative Treatment in Spontaneous Supratentorial Intracerebral Hemorrhage.

Large Intracerebral hematoma (ICH), compounded by perihematomal edema can produce severe elevations of intracranial pressure (ICP). Decompressive craniectomy (DC) beneficially addresses mass effect. Therefore this study is aimed to prove that decompressive craniectomy with durotomy has better outcome in patients with spontaneous supratentorial ICH than conservatively treated patients. This Quasi-Experimental study was carried out in the Department of Neurosurgery, Dhaka Medical College & Hospital (DMCH), Dhaka, Bangladesh from July 2012 to December 2013 using purposive sampling procedure. Stereological Lung Parameters After Fetoscopic Abdominal Decompression of Congenital Diaphragmatic Hernia in an Ovine Model: A Pilot Study.

Background: Fetoscopic endoluminal tracheal occlusion (FETO) improves lung maturation in severe cases of congenital diaphragmatic hernia (CDH) but it does not ameliorate lung compression by herniated abdominal organs. Surgically opening the fetal abdomen (abdominal decompression [AD]) reduces the intrathoracic pressure by diverting the abdominal organs into the amniotic cavity-a probable causal therapy for lung hypoplasia and pulmonary hypertension in CDH. Open surgical abdominal decompression has been reported: we describe a minimally invasive approach in an ovine model of CDH as a probable f

Analysis of Surgical Cases of Tarsal Tunnel Syndrome in Our Department: Case Series and Literature Review.

BACKGROUND: Tarsal tunnel syndrome (TTS) is an entrapment neuropathy in which the tibial nerve is compressed within the tarsal tunnel and causes sensory disturbance in the sole of the foot. In this manuscript, we summarized our early surgical cases of TTS. MATERIALS AND METHODS: Six feet in five patients with TTS were treated surgically. The patients were aged 31-70 years (mean 53.1 years), and all of them complained of pain or dysesthesia of the sole of the foot sparing the heel. Magnetic resonance imaging (MRI) and nerve conduction test were performed preoperatively. In surgery, flexor reti

Biomechanical considerations in the surgical management of cervical spondylotic myelopathy.

Based on a review of previous studies and our opinion, biomechanical considerations suggest the following guidelines for the surgical management of CSM. It is not recommended that the dura mater, the pia mater, and the dentate ligaments be transected in the surgical treatment of CSM. Anterior decompression and fusion, preferably with the Smith-Robinson technique, is recommended for patients with anterior impingement of the spinal cord at one or two levels in the absence of a narrow spinal canal. This procedure is also advantageous when there is significant radiculopathy associated with the lev

Early Decompression in Acute Spinal Cord Injury : Review and Update.

Spinal cord injury (SCI) has a significant negative effect on the quality of life due to permanent neurologic damage and economic burden by continuous treatment and rehabilitation. However, determining the correct approach to ensure optimal clinical outcomes can be challenging and remains highly controversial. In particular, with the introduction of the concept of early decompression in brain pathology, the discussion of the timing of decompression in SCI has emerged. In addition to that, the concept of "time is spine" has been added recently, and the mortality and complications caused by SCI

Conservative Approach of a Dentigerous Cyst.

Dentigerous cysts (DC) are the most common odontogenic cystic lesions of inflammatory origin occurring in children. These lesions can be treated by enucleation with or without related impacted teeth or marsupialization/decompression. The latter procedures have been used successfully for many years, but decompression is increasingly recommended in children because of its good outcomes and the preservation of the developing tooth. This conservative surgical technic allows simultaneously the normal eruption of the involved permanent teeth and the ossification of the bony defect. The present repor

Pulmonary overcirculation following left atrial decompression in hypoplastic left heart syndrome with intact/restrictive atrial septum.

Patients with hypoplastic left heart syndrome (HLHS) with intact or severely restrictive atrial septum have slow resolution of their elevated pulmonary vascular resistance after left atrial decompression. We report two neonates who required urgent surgical intervention because of unusually rapid fall in their pulmonary vascular resistance following left atrial decompression leading to pulmonary overcirculation and severe congestive heart failure. HLHS with intact or severely restrictive atrial septum is a predictor of poor outcome. The need for left atrial decompression can be predicted by wel

Endogenous-lesioned cervical disc herniation: a retrospective review of 9 cases.

The purpose of this study was to analyze the pathogenic mechanisms, clinical presentation, and surgical treatment of cervical disc herniation without external trauma. Between 2004 and 2008, 9 patients with cervical disc herniation and no antecedent history of trauma were diagnosed with cervical disc herniation and underwent surgical decompression. Pathogenic mechanisms, clinical presentation, surgical treatment, and prognosis were analyzed retrospectively. In 6 patients, herniation resulted from excessive neck motion rather than from external trauma. An injury from this source is termed an end
