Hypomanic symptoms in early pregnancy: prevalence and associated factors.

Given that the perinatal period is a time of increased risk for pregnant women to manifest mental health problems, the identification of antenatal hypomanic symptoms is particularly important. However, data on antenatal hypomanic symptoms is lacking. The present study was aimed at filling this research gap by investigating the prevalence of hypomanic symptoms, including the "active-elated" and "irritable/risk-taking" sides of hypomanic symptoms at the first trimester, and examining their associations with anxiety and depressive symptoms at the following time points: the first trimester, the se

Cross-lagged analysis of interdependence and psychological symptom clusters in postpartum women: A prospective study.

The aim of this study is to investigate the longitudinal interaction mechanism between interdependence and clusters of psychological emotional symptoms in postpartum women. Three waves of follow-up assessments were conducted on 389 postpartum women at T1 (before discharge), T2 (42 days post-delivery), and T3 (3 months post-delivery). The assessments utilized the general information questionnaire, the 10-item Connor-Davidson resilience scale (CD-RISC-10), the Edinburgh Postnatal depression scale (EPDS), the post-traumatic stress disorder (PTSD) Checklist-Civilian version (PCL-C-7), and the mutu

Repetitive negative thinking in the perinatal period and its relationship with anxiety and depression.

BACKGROUND: Rumination and worry represent two types of repetitive negative thinking (RNT), and their predictive and maintaining roles are well-established in depression and anxiety, respectively. Furthermore, there is an emerging literature on the link between RNT and psychological wellbeing in the perinatal period. METHODS: We conducted a scoping review of studies that have investigated the relationship between RNT and perinatal depression and anxiety. We identified 87 papers eligible for inclusion in the review; they included cross-sectional and longitudinal studies, as well as treatment e

Inflammatory markers of perinatal depression in women with and without history of trauma.

PURPOSE: Increased inflammation has been described as consistently associated with depression. Moreover, the pro-inflammatory pattern was found in women with a history of trauma irrespective of major depression diagnosis. In this study, we explored the possible association of inflammatory markers with perinatal depression (PND), measuring serum levels of cytokines (IL-6, TNF-a, IFN-γ), acute phase proteins (CRP), erythrocyte sedimentation rate (ESR), cortisol and brain-derived neurotrophic factor (BDNF) in women at the second trimester of pregnancy. Moreover, we tested whether the biological m

Dietary interventions for perinatal depression and anxiety: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Dietary interventions are a widely available mediation for depression and anxiety among pregnant and/or postpartum (i.e., perinatal) persons, but their effectiveness is not well known. OBJECTIVES: We performed a systematic review and meta-analysis to assess the effectiveness of dietary interventions for the treatment of perinatal depression and/or anxiety. METHODS: We searched MEDLINE, EMBASE, PsycINFO, CINAHL, and Web of Science from their inception to 2 November, 2022. Studies were included if they were available in English and examined the effectiveness of a dietary interventi

Longitudinal mediation analysis of the factors associated with trajectories of posttraumatic stress disorder symptoms among postpartum women in Northwest Ethiopia: Application of the Karlson-Holm-Breen (KHB) method.

INTRODUCTION: In recent years, literatures identified childbirth as a potentially traumatic experience resulting in posttraumatic stress disorder (PTSD), with 19.7 to 45.5% of women perceiving their childbirth as traumatic. A substantial variation in PTSD symptoms has been also indicated among women who experience a traumatic childbirth. However, there has been no research that has systematically investigated these patterns and their underlying determinants in postpartum women in Ethiopia. OBJECTIVE: The aim of this study was to investigate the trajectories of PTSD symptoms and mediating rela

Traumatic Childbirth Experiences and their Association with Postpartum Psychiatric Disorders: A Systematic Review.

Traumatic childbirth experiences, characterized by intense fear, helplessness, or loss of control during labor or delivery, may increase the risk of postpartum psychiatric disorders. Events such as emergency cesarean sections, preterm births, and neonatal intensive care unit admissions have been associated with elevated psychological distress. This systematic review aimed to synthesize current evidence on the association between traumatic childbirth experiences and postpartum psychiatric disorders, including depression, post-traumatic stress disorder, and anxiety. A systematic search of PubMed

Postpartum psychiatric disorders.

Postpartum blues, postpartum neurotic depression and puerperal psychoses have distinct clinical features; they affect women in all social classes and in all cultures, and despite numerous studies they have not been linked definitively with any biologic or psychosocial variables. The only possible exception is puerperal psychosis, which emerges much more often in women with a personal or family history of a bipolar affective disorder than in women without, a finding that probably explains the reluctance of some researchers to recognize puerperal psychotic episodes as distinct from psychotic epi

Seeking help for anxiety and depression after childbirth: results of the Maternal Health Study.

Access to treatment for postnatal anxiety and depression is dependant on women seeking help for psychological symptoms. The aim of this paper was to investigate what women themselves say about seeking help for emotional difficulties after childbirth. The Maternal Health Study is a prospective pregnancy cohort study investigating the physical and psychological health of 1,507 nulliparous women during pregnancy and after birth. One thousand, three hundred eighty-five women completed a computer-assisted telephone interview at 9 months postpartum; 8.5% of women reported intense anxiety or panic at

Assessment of the factor structure of the EPDS in Chinese perinatal women: a longitudinal study using multigroup confirmatory factor analysis.

INTRODUCTION: Perinatal depression poses significant risks to the mental health of perinatal women, affecting both their well-being and their ability to care for themselves and infants. This study investigates the factor structure and reliability of the Edinburgh Postnatal Depression Scale (EPDS) across multiple time points in perinatal women in China. METHODS: A total of 849 perinatal women participated in this study, with data collected at six time points: early, mid, and late pregnancy, as well as 1, 3, and 6 months postpartum. Parallel analysis and exploratory factor analysis were conduct

Associations between maternal psychological distress and mother-infant bonding: a systematic review and meta-analysis.

PURPOSE: Maternal psychological distress and mother-infant bonding problems each predict poorer offspring outcomes. They are also related to each other, yet the extensive literature reporting their association has not been meta-analysed. METHODS: We searched MEDLINE, PsycINFO, CINAHL, Embase, ProQuest DTG, and OATD for English-language peer-reviewed and grey literature reporting an association between mother-infant bonding, and multiple indicators of maternal psychological distress. Prevalence of neurotoxicity symptoms among postpartum women on isoniazid preventive therapy and efavirenz-based treatment for HIV: an exploratory objective of the IMPAACT P1078 randomized trial.

BACKGROUND: This exploratory analysis investigates the prevalence and risk factors of neurocognitive toxicity in postpartum women on HIV treatment in response to a concern of an Isoniazid Preventive Therapy (IPT)/Efavirenz interaction. TRIAL DESIGN: Pregnant women on HIV treatment from countries with high TB prevalence were randomized in IMPAACT P1078 to 28 weeks of IPT started either during pregnancy or at 12 weeks postpartum. Partway through study implementation, the Patient Health Questionnaire 9, the cognitive complaint questionnaire, and the Pittsburg Sleep Quality Index were added to ev

Perinatal depression at the intersection of race/ethnicity and disability.

PURPOSE: Perinatal depression disparately impacts diverse groups, with marginalized populations often facing greater vulnerability. While previous research has highlighted disparities in perinatal depression by race/ethnicity and by disability status independently, there is a lack of research examining the intersectionality of these social identities and their combined association with perinatal depression. Therefore, this study adopts an intersectional lens to explore variations in perinatal depressive symptoms associated with the combination of race/ethnicity and disability status in a natio

Patterns of disease-modifying therapy utilization before, during, and after pregnancy and postpartum relapses in women with multiple sclerosis.

BACKGROUND: Pregnancy is a common consideration for people with multiple sclerosis (pwMS); MS onset is typically between 20 and 45 years of age, during potential child-bearing years. Pregnancy and postpartum care are a significant factor influencing disease-modifying therapy (DMT) selection for many pwMS. To date, few DMTs are considered safe to continue during pregnancy and real-world treatment patterns before, during, and after pregnancy remain uncharacterized. Evolving guidance is needed regarding how to optimize management of the pregnancy and postpartum periods considering the changing DM

Management of Bipolar Disorder During the Perinatal Period.

Bipolar disorder (BPD) is a lifelong mental health condition characterized by symptoms of mania, depression, and often anxiety. BPD can have detrimental consequences for individuals during pregnancy and the postpartum period, as well as for their offspring. This is often due to underdiagnosis and/or misdiagnosis as unipolar depression. There is a high incidence of first episodes of BPD in pregnant and postpartum persons. Perinatal care providers need to routinely screen for BPD and assess for relapse among those with a previous diagnosis during the pregnancy and postpartum periods. Medication

An evaluation of symptom domains in the 2 years before pregnancy as predictors of relapse in the perinatal period in women with severe mental illness.

BACKGROUND: Symptoms may be more useful prognostic markers for mental illness than diagnoses. We sought to investigate symptom domains in women with pre-existing severe mental illness (SMI; psychotic and bipolar disorder) as predictors of relapse risk during the perinatal period. METHODS: Data were obtained from electronic health records of 399 pregnant women with SMI diagnoses from a large south London mental healthcare provider. Symptoms within six domains characteristically associated with SMI (positive, negative, disorganization, mania, depression, and catatonia) recorded in clinical note
