Imanaga's First Method for Reconstruction with Preservation of Mesojejunal Autonomic Nerves During Pylorus-Preserving Pancreatoduodenectomy.

BACKGROUND Pancreatic surgeries have undergone substantial development. Pancreaticoduodenectomy and pylorus-preserving pancreatoduodenectomy inherently require reconstruction. In 1960, Professor Imanaga introduced a reconstructive technique performed in the order of the gastric remnant, pancreatic duct, and biliary tree from the viewpoint of physiologic function after pancreaticoduodenectomy. We herein report our experience with Imanaga's first method during pylorus-preserving pancreatoduodenectomy and retrospectively evaluate the short- and long-term outcomes. Technicalities and pitfalls are

Surgery for Type III-IV hiatal hernia: anatomical recurrence and global results after elective treatment of short oesophagus with open and minimally invasive surgery.

OBJECTIVES: Type III-IV hiatal hernia (HH) is associated with a true short oesophagus in more than 50% of cases; dedicated treatment of this condition might be appropriate to reduce the recurrence rate after surgery. A case series of patients receiving surgery for Type III-IV hernia was examined for short oesophagus, and the results were analysed. METHODS: From 1980 to 1994, 60 patients underwent an open surgical approach, and the position of the oesophago-gastric junction was visually localized; from 1995 to 2013, 48 patients underwent a minimally invasive approach, and the oesophago-gastric

Pathogenesis and treatment of neoplastic diseases of the papilla of Vater: Kausch-Whipple procedure with lymph node dissection in cancer of the papilla of Vater.

Cancer of the papilla or the ampulla of Vater appears, from a clinical point of view, to be an intraduodenal or ampullary cancer. An adenoma-dysplasia-carcinoma sequence has been established. In 20%-40% of the patients with an adenoma of the papilla, a cancerous lesion in the adenoma is additionally observed. Oncological resection using a Kausch-Whipple technique or a pylorus-preserving partial pancreatico-duodenectomy (PPPD) offers a 5-year survival probability of between 45% and 65%. The hospital mortality after oncological resection at experienced centers is below 5%. The most frequent trea

Perioperative Immunonutrition in Gastrointestinal Oncology: A Comprehensive Umbrella Review and Meta-Analysis on Behalf of TROGSS-The Robotic Global Surgical Society.

INTRODUCTION: Gastrointestinal (GI) cancers are associated with high morbidity and mortality. Surgical resection, the primary treatment, often induces immunosuppression and increases the risk of postoperative complications. Perioperative immunonutrition (IMN), comprising formulations enriched with omega-3 fatty acids, arginine, nucleotides, and antioxidants, has emerged as a potential strategy to improve surgical outcomes by reducing complications, enhancing immune function, and promoting recovery. METHODS: A systematic search of PubMed, Scopus, and the Cochrane Library was conducted on 28 Oc

[Surgery in Crohn’s disease].

BACKGROUND: Prior to the COVID-19 pandemic, TB treatment in Peru was delivered almost exclusively through facility-based directly observed therapy (DOT). During the pandemic, selected adolescents were allowed home-based TB treatment under caregiver supervision, along with video-supported treatment (VST) for adolescents with access to a smartphone. METHODS: We conducted 16 focus groups, each with 4-10 participants of adolescents with rifampicin-susceptible TB, their caregivers, and health care workers (HCWs). We used semi-structured guides to gather perspectives on facility-based DOT versus ho

Transanal total mesorectal excision (TaTME): current status and future perspectives.

Total mesorectal excision (TME) is the gold standard surgical treatment for mid- and low rectal cancer; however, it is associated with specific technical hurdles. Transanal TME (TaTME) is a new procedure developed to overcome these difficulties, through an enhanced visualization of the dissection plane. This potentially could result in a more accurate distal dissection with a lower rate of positive circumferential resection margins, increasing the rate of sphincter-saving procedures. The indications for TaTME are currently expanding, despite not being yet standardized, and structured training

Outcomes of colorectal cancer surgery in the nonagenarians: 20-year result from a tertiary center.

BACKGROUND: There is a foreseeable trend that life expectancy is on the rise in many parts of the world. More and more patients will present with colorectal cancer at extreme old age and advanced age is a well-known risk factor for adverse outcomes after surgery. The aim of this study is to evaluate the outcomes of colorectal cancer surgery in patients aged 90 or above. METHOD: A retrospective analysis of consecutive patients aged 90 or above who underwent operations for colorectal cancer between January 1996 and December 2015 was performed. The primary outcomes were the complications rate, 3

Two-stage totally minimally invasive approach for acute complicated diverticulitis.

OBJECTIVES: Surgical options for acute diverticulitis with peritonitis include Hartmann's procedure or resection and primary anastomosis with or without a stoma. Initial laparoscopic lavage and drainage can control the acute intra-abdominal sepsis to allow for a delayed definitive procedure in nonemergency conditions. Potential advantages include the avoidance of a laparotomy, stoma and local infections at the origin of dehiscence and incisional hernias. We evaluated this approach in a selected group of patients. METHODS: Patients with intra-abdominal pus secondary to diverticular perforation

Accuracy and Clinical Utility of Spot-Check Noninvasive Hemoglobin Monitoring: A Preliminary Study.

INTRODUCTION: Hemoglobin (Hb) monitoring is essential for perioperative management, particularly for detecting anemia and guiding transfusion decisions. Conventional invasive methods provide accurate results but are limited by delays, intermittent sampling, and iatrogenic blood loss. Noninvasive spot-check Hb (SpHb) monitoring offers rapid assessment, but its accuracy is unclear. This study thus evaluated agreement between SpHb obtained with the Rad-67 and invasive Hb measurements in anesthetized patients. METHODS: This single-center, prospective, observational study enrolled 25 adults who un

From STEP to MAGIC: Patient-Inspired Surgical Innovation.

INTRODUCTION: Much of the current standard of care in surgery was, at some time, thought by many to be radical ideas that would "never work". This paper describes my personal journey in surgical innovation that was largely inspired by the pioneering parents of very sick children who pushed me to consider why there were not better solutions to difficult problems. INNOVATIONS: My path in surgical innovation began with the development of serial transverse enteroplasty, a novel approach for treating children with short bowel syndrome by repurposing traditional surgical methods. Through my clinica

Predictive Factors for Bowel Resection in Childhood Intussusception.

BACKGROUND: Intestinal intussusception is the most common cause of intestinal obstruction in infants and children under 3 years of age. Any delay in diagnosis or management can lead to intestinal ischaemia and perforation. The aim of this study is to determine the sociodemographic and clinical risk factors associated with bowel resection in infants and children with intussusception. MATERIALS AND METHODS: This is a retrospective analytical study of 118 patients operated at the Hassan II Hospital and University of Fez between 1 January 2008 and 1 January 2018. A comparison of risk factors betw

Predictors of urinary tract infection after lower gastrointestinal surgery.

BACKGROUND: Urinary tract infections (UTIs) cause postoperative morbidity in patients undergoing lower gastrointestinal (GI) surgery that can prolong postoperative hospital stays. In patients with a fever of unknown origin (FUO), clinicians ignore what to do while waiting for the results of the urine culture test. This study aimed to develop a nomogram predicting UTI in the case of postoperative FUO. METHODS: This observational, retrospective study included all consecutive patients from 1 November 2020 to 1 November 2021 undergoing lower-GI surgery at the Chirurgia Generale 3, University Hosp
