Diverticular disease represents the most common disease affecting the colon in the Western world. Most cases remain asymptomatic, but some others will have symptoms or develop complications. The aims of treatment in symptomatic uncomplicated diverticular disease are to prevent complications and reduce the frequency and intensity of symptoms. Fibre, probiotics, mesalazine, rifaximin and their combinations seem to be usually an effective therapy. In the uncomplicated diverticulitis, outpatient management is considered the optimal approach in the majority of patients, and oral antibiotics remain …

Diverticular disease is generally benign but may be serious in case of septic complications. The most common complication of acute diverticulitis is development of an abscess which can be located around the colon or in the pelvis. The diagnosis can be clinically suspected in case of non-response to medical management of severe acute diverticulitis. Confirmation is obtained by conventional radiographic examinations and computerized tomography. This condition can be safely treated by percutaneous catheter drainage associated to antibiotics followed by an elective delayed single-stage operation w…

PURPOSE: In the context of neoplasia, acellular mucin in lower gastrointestinal (GI) tract implies occult mucin-producing tumor and warrants additional workup. The clinical significance of acellular mucin in benign conditions remains unclear.
METHODS: Lower GI tract surgical specimens with acellular mucin without documented neoplastic conditions (colonic diverticulitis (n = 16), appendicitis (n = 14), and others (n = 8)) were retrieved. Low grade appendiceal mucinous neoplasm (LAMN) (n = 24) and diverticulitis without acellular mucin (n = 28) were used as controls for appendicitis and diverti…

Diverticular disease is common condition globally, especially in Western countries. Diverticulitis, Symptomatic uncomplicated Diverticular disease and Segmental Colitis associated with diverticula constitute diverticular disease. Although most patients with diverticula are asymptomatic, around 25% of patients will experience symptoms whilst 5% of patients have an episode of acute diverticulitis. The prevalence increases with age with more than one theory being put forward to explain its pathogenesis. Faecolith entrapment in diverticula results in colonic mucosal damage and oedema, bacterial pr…

Crohn's disease and diverticulitis of the sigmoid may be combined in old patients, this association could lead to difficulties in diagnosis and treatment. Three cases of Crohn's colitis associated with diverticula, two females and one male, aged between 66 and 80 years are reported by the authors. In a review of the literature, some clinical, radiological and endoscopic findings may suggest the diagnosis, which guides subsequent treatment. A high morbidity is observed after surgery. In our series, sigmoidectomy cured both Crohn's colitis and diverticulitis.

BACKGROUND: Gas extravasation complications arising from perforated diverticulitis are common but manifestations such as pneumoperitoneum, pneumoretroperitoneum, and pneumomediastinum happening at the same time are exceedingly rare. This case report explores the unique presentation of these 3 complications occurring simultaneously, their diagnosis and their management, emphasizing the importance of interdisciplinary collaboration for accurate diagnosis and effective management.
CASE PRESENTATION: A 74-year-old North African female, with a medical history including hypertension, dyslipidemia, …

Colonic diverticular disease is a benign condition typical of the Western world, but it is not rare for even the 1st episode of diverticulitis to carry potentially fatal complications. The evolution of a peridiverticular process generally poses problems for medical treatment and exposes patients to repeated episodes of diverticulitis, making surgical treatment necessary in approximately 30% of symptomatic patients. One of the most worrying complications of diverticulosis is internal fistula. The most common types of fistula are colovesical and colovaginal, against which the uterus can act as a…

PURPOSE: To report our experience in the study of acute left-sided colonic diverticulitis and to investigate the semiology and the possible diagnostic role of Computed Tomography (CT).
MATERIAL AND METHODS: In the last 5 years, we examined with CT 52 patients with acute sigmoid diverticulitis. The examinations were performed with contiguous slices from the diaphragm to the perineum. An iodized contrast agent was administered orally in 32 cases and intravenously in 38. The diagnostic parameters we considered were: diverticula, pericolonic inflammation, thickening, intraparietal tracts, fistula…

PURPOSE OF REVIEW: While societal guidelines help direct management of diverticulitis and diverticular bleeding broadly, our review focuses on the latest data for nuanced care of older patients affected by these conditions.
RECENT FINDINGS: Diverticulitis in older patients can present with non-specific symptoms so a broad work up is recommended. Once diagnosed, those with uncomplicated disease (Hinchey Class 0 or 1a) can be safely managed without antibiotics or admission depending on frailty and comorbidities. Most older patients with complicated diverticulitis (abscess, perforation or obstru…

PURPOSE: To assess the effectiveness of and complications associated with metallic stent placement for treatment of benign diseases of the colon.
MATERIALS AND METHODS: With radiologic guidance, the authors placed metallic stents in 10 patients with benign diseases of the colon: Nine stents were placed in the rectosigmoid colon, and one was placed in the descending colon. The stents were placed in two cases of diverticulitis complicated by pelvic abscess, four cases of colonic fistula following surgery, and four cases of postsurgical anastomotic stricture. Coated stents were placed in the cas…

This is a case of a 53-year-old African American woman with newly diagnosed concomitant acute cholecystitis, diverticulitis, and SARS-COV-2 infection. She underwent treatment for COVID-19 with antibiotics and supportive treatment, and on day 32 after initial symptoms presented, she had an elective laparoscopic cholecystectomy. Our patient was one of the first cases to present with acute cholecystitis and diverticulitis complicated by COVID-19, and serves as a template for surgical management of non-emergent abdominal pathologies in the midst of the COVID-19 pandemic.

We present the case of a 50-year-old man with situs inversus who underwent laprascopic colonic resection for diverticulitis. The patient, who had right-sided pain due to inversion of the viscera mimicking appendicitis, initially presented a diagnostic challenge. A barium meal confirmed situs inversus and the diagnosis of diverticulitis. We present an overview of the operative technique of what is the first documented laparoscopic colectemy in a patient with situs inversus. We then discuss the benefits of the laproscopic approach to colectomy, with reference to this interesting case with unusua…

Natural history of diverticular disease (DD) through its different subgroups (diverticulosis, symptomatic uncomplicated diverticular disease (SUDD) previous diverticulitis (PD)) is poorly defined. Few studies, mainly retrospective, have analysed its rate of progression and risk factors associated. We assessed the potential progression rate of different spectrum of DD and associated risk factors. Data from the REMAD Registry, a 5-year Italian longitudinal prospective multicentre study, were analysed. A total of 1217 patients were included that were divided into three subgroups (diverticulosis, …

INTRODUCTION: Colorectal perforations can arise from various causes. The most common cause of the colorectal perforations is diverticulitis, while tumors account for only a small part. Perforations frequently occur in older patients, particularly in the sigmoid colon, and distinguishing between those caused by diverticulitis and colorectal cancer can be clinically challenging. Gastrointestinal stromal tumors (GISTs) account for a small percentage of gastrointestinal malignancies, and rectal perforation due to GISTs is rare but can lead to severe complications such as peritonitis.

INTRODUCTION: Fistulizing diverticulitis occurs in only 2% of diverticular disease cases, but its symptoms, such as urinary tract infections (UTI), pneumaturia, fecaluria, or vaginal discharge, are highly disruptive to patients. Therefore, surgery is commonly recommended. Laparoscopy has been proven feasible and safe for fistulizing diverticulitis, although revealing a conversion rate of 36%. Robotic surgery might reduce the conversion rate due to advanced instrumentation and improved optics.
METHODS: All consecutive patients diagnosed with diverticulitis complicated by a colovesical or/and c…

INTRODUCTION: Diverticulitis can be complicated by fistulas between the colon and neighboring structures, which predispose to significant morbidity and mortality. Fistulas involving the female urogenital tract often present with urogynecologic symptoms, such as vaginal discharge or recurrent urinary tract infections. While colo-vaginal fistulas, a more common variant, often present with vaginal flatulence, colo-salpingeal fistulas are exceedingly rare and have not been reported with this symptomatology. We describe a case of colo-saplingeal fistula presenting with vaginal flatulence, requiring…

BACKGROUND: A PubMed search of the biomedical literature was carried out to systematically review the role of laparoscopy in colonic diverticular disease. All original reports comparing elective laparoscopic, hand-assisted, and open colon resection for diverticular disease of the colon, as well as original reports evaluating outcomes after laparoscopic lavage for acute diverticulitis, were considered. Of the 21 articles chosen for final review, nine evaluated laparoscopic versus open elective resection, six compared hand-assisted colon resection versus conventional laparoscopic resection, and …
