Hindfoot Fusion Nail with Adjuvant Hybrid External Fixation used as Limb-Salvage Procedure after Failed Open Reduction Internal Fixation of Ankle Fracture in a Charcot Joint: A Case Report.

The failure of the internal fixation of an ankle fracture due to Charcot joint/ neuroarthropathy is rare; such occurred in the case of a 52-year-old woman with uncontrolled diabetes who sustained a right ankle fracture after falling from standing height. The patient was treated with the internal fixation of both malleoli, which procedure failed as the patient progressed, until she eventually was diagnosed with Charcot arthropathy. The post operative images were reviewed and showed a catastrophic fixation failure with a diagnosis of Charcot neuroarthropathy. At that point, we decided to treat t

Comparison of Techniques Utilising External Fixation to Motorised Intramedullary Nails in the Treatment of Lower Limb Discrepancy: Systematic Review and Meta-analysis.

AIM: This systematic review and meta-analysis compares the clinical outcomes of motorised lengthening intramedullary nails (MLNs) with techn … gnificant (95% CI: -1.77, -0.04). CONCLUSION: This study suggests that techniques that incorporate external fixators have a greater potential for the amount of total lengthening over the use of motorised lengthening MLN but may increase complications requiring reoperation. There is no significant difference in time to union. CLINICAL SIGNIFICANCE: This is the fir …

Submuscular Plating, Titanium Elastic Nailing, and External Fixation for Pediatric Femoral Shaft Fractures: A Comparative Systematic Review and Meta-Analysis.

Pediatric femoral shaft fractures are commonly treated with titanium elastic nailing (TENS), submuscular plating (SMP), or external fixation (EF), but the optimal fixation strategy remains controversial. This systematic review and meta-analysis compared perioperative metrics, union, complications, and functional outcomes between SMP, TENS, and EF in skeletally immature patients. A comprehensive search of PubMed, Scopus, Web of Science, Embase, and Google Scholar (up to November 2025) identified comparative studies of diaphyseal pediatric femoral shaft fractures treated with at least two of the

Letter to the Editor regarding "Quantitative assessment of pelvic reduction using a pretensioned external fixator in unstable pelvic injuries: a retrospective study".

The pretensioned pelvic external fixator (PPEF) is a promising option for unstable pelvic injuries, but its radiological assessment remains unstandardized. Queipo-de-Llano et al. reported favorable reduction in 28 patients, with no significant difference between Tile C and A/B fractures. We highlight three methodological issues: inconsistent labeling of the Matta-Tornetta criteria, reduction CT performed several days after frame application, and inadequate control of baseline balance in subgroup comparisons. Clarifying these points is essential for interpreting the true efficacy of PPEF.

Primary closure versus secondary healing of pin sites after the removal of provisional external fixator: a systematic review and meta-analysis.

BACKGROUND: External fixators are commonly used for managing complex fractures, but they are associated with the risk of pin site infections. Pin site management following external fixator removal can be performed using either primary closure or secondary healing. The relative efficacy and safety of these approaches, particularly in terms of infection rates, remain uncertain. OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the infection rates of pin entry sites following the removal of the temporary external fixator, comparing primary closure and secondary healing method…

Intramedullary nailing versus external ring fixation for tibial shaft fractures: an explorative analysis of muscle strength from the IMVEX trial.

BACKGROUND: To explore the effect of intramedullary nailing versus external ring fixation for patients with tibial shaft fractures on 12-month maximal isometric muscle strength for knee extension and knee flexion. We hypothesise that patients treated with external ring fixation will show significantly better maximal isometric muscle strength for knee extension and knee flexion 12 months after surgery compared to treatment with intramedullary nailing. Moreover, we aim to explore the association between muscle strength and the Knee Injury and Osteoarthritis Outcome Score (KOOS) at 12 months foll

Open elbow arthrolysis with hinged external fixation for postburn elbow stiffness: a case series with an averaged 4-year follow-up.

OBJECTIVE: Patients with severe burns may develop heterotopic ossification (HO), articular cartilage loss, and soft tissue contracture in the elbow, significantly limiting elbow function and diminishing quality of life. Open elbow arthrolysis (OEA) combined with hinged external fixation (HEF) has demonstrated efficacy in managing severe elbow stiffness. However, no recent studies have specifically investigated its application in postburn elbow stiffness. This study evaluates the clinical outcomes of OEA with HEF in patients with postburn elbow stiffness, with an average follow-up period of fou

Quantitative assessment of fracture healing using patient-specific CT finite element analysis: A novel approach to external fixation management.

BACKGROUND: Posttreatment decisions regarding fracture care primarily rely on physicians' subjective judgment based on clinical and imaging findings. This study aimed to evaluate whether patient-specific computed tomography (CT) finite element analysis could allow for the quantitative assessment of fracture site strength as an objective indicator of treatment decisions. METHODS: Our patient was a 37-year-old male who underwent external fixation for an open comminuted tibial fracture of the right lower leg. Monthly CT scans were taken starting 2 months postinjury. Utility of external fixation for traumatic periprosthetic fracture after total ankle arthroplasty in patients with rheumatoid arthritis: A report of two cases.

This report describes two cases of implant-stable traumatic periprosthetic fractures after total ankle arthroplasty in patients with rheumatoid arthritis. One case with a low body mass index (14 kg/m2) achieved complete bone union with the use of an external fixator, while the other case with a high body mass index (32.83 kg/m2) failed to achieve bone union with the external fixator; however, complete union was achieved utilising secondary internal plate fixation. Although open reduction and internal fixation using a plate is the standard procedure in implant-stable periprosthetic fracture cas

Quality of Life in Children With Achondroplasia Undergoing Paired Limb Lengthening With an External Fixator and Modified Distraction Control: Observational Nonrandomized Study.

BACKGROUND: Transosseous distraction osteosynthesis is prioritized in orthopedic care for children with achondroplasia. However, difficulties encountered during treatment and rehabilitation directly impact patients' quality of life. Using rod external fixators within a semicircular frame for osteosynthesis is less traumatic compared to spoke circular devices. Their straightforward assembly and mounting on the limb segment can help significantly reduce treatment duration, thereby improving children's quality of life during treatment and rehabilitation. OBJECTIVE: This study aimed to conduct a …

Clinical outcomes of an olive wire and external fixator versus a metallic screw in the treatment of inferior tibiofibular syndesmosis injuries in ankle fractures.

BACKGROUND: Metal screws are the most widely used in treating syndesmotic injuries, but failure and the rigidity of the screws can threaten the success of the treatment and increase the cost of care. We have provided an alternative with an olive wire and external fixator(OWEF) used for syndesmotic fixation. METHODS: A retrospective longitudinal follow-up study was conducted. From February 2011 to January 2018, 58 of 72 patients with ankle fractures and associated syndesmotic disruption were treated with either screw or OWEF fixation. The costs, complications, and clinical outcomes using Oleru

A Case of Closed Pilon Fracture Resulting in Soft-Tissue Necrosis and Treated with Reverse Sural Artery Flap and Circular External Fixation.

Tibial pilon fractures are difficult to treat due to articular comminution and soft-tissue injuries caused by high-energy trauma. Open reduction and internal fixation is a commonly used method of treatment. However, it has a high risk of infection and soft-tissue complications due to the extensive detachment of soft tissue. We report on a case with a tibial pilon fracture and soft-tissue necrosis that we treated using limited internal fixation combined with a circular external fixator (LIFCEF) and reverse sural artery flap (RSAF) as part of an orthoplastic approach within the orthopedic surger

External Fixation Prior to Fasciectomy Leads to Substantial Improvement of Advanced Dupuytren's PIP Contractures at Mean Follow-up 15 Months.

BACKGROUND: The Digit Widget is an external fixation device that can be used to reverse proximal interphalangeal joint (PIPJ) contractures in the hand. Our hypothesis is that usage of the Digit Widget prior to fasciectomy in patients with severe Dupuytren's proximal interphalangeal (PIP) contractures will result in short-term improvement and maintenance of the PIPJ contracture after fasciectomy. METHODS: Patients who underwent placement of the Digit Widget soft tissue distractor prior to fasciectomy for Dupuytren's disease were identified between January 2015 and December 2018. Multiple finge

Comparison between external fixation and pelvic binder in patients with pelvic fracture and haemodynamic instability who underwent various haemostatic procedures.

Haemostatic procedures such as preperitoneal pelvic packing (PPP), pelvic angiography (PA), and internal iliac artery ligation are used for haemorrhage control in pelvic fracture patients with haemodynamic instability. Pelvic external fixation (PEF) and pelvic binder (PB) are usually applied with haemostatic procedures to reduce the pelvic volume. This study aimed to compare the clinical outcomes between patients who underwent PEF and PB. Among 173 patients with pelvic fracture admitted to the emergency room of three regional trauma centres between January 2015 and December 2018, the electroni
