Predictors of intracranial lesions in patients with traumatic facial fractures: findings derived from the National Trauma Registry of Iran.

OBJECTIVE: To use data from the National Trauma Registry of Iran (NTRI) to explore whether the type of traumatic facial fractures predicts intracranial lesions. METHODS: This retrospective registry-based study analyzed 6 years of data from four NTRI trauma centers, focusing on patients with facial fractures. Patients with at least one facial fracture were included, and data on demographics, injury mechanisms, fracture patterns, and intracranial lesions were analyzed. A multiple logistic regression model demonstrated the associations between clinical variables and intracranial lesions. RESULT

Is Psychiatric Illness Associated With the Clinical Decision to Treat Facial Fractures?

PURPOSE: Underlying psychiatric conditions are thought to influence the presentation, management, and outcomes of facially injured patients. Our study sought to determine if psychiatric diagnoses were associated with the decision to repair facial fractures during the index hospitalization. METHODS: This was a cross-sectional review of the 2014 Nationwide Emergency Department Sample. All patients with the primary diagnosis of a facial fracture were included in the study. The primary study predictor was the presence of a documented psychiatric illness. Covariates included patient age, insurance

Schwannoma of the Zygomatic Branch of the Facial Nerve.

SIGNIFICANCE STATEMENT: Facial nerve schwannoma is extremely uncommon. Despite its rarity, it is considered the most common facial nerve tumor and potentially affects any segment of the nerve. Presenting symptoms vary depending on the location of the neoplasm. Tumors pertaining to the extratemporal course of the nerve mainly appear as an asymptomatic parotid mass. We present a rare case of schwannoma of the zygomatic branch of the right facial nerve that was surgically resected, without facial nerve injury.

Safety Profile of Thread Lifts on the Face and Neck: An Evidence-Based Systematic Review.

BACKGROUND: "Thread lifting" has quickly gained popularity as a minimally invasive treatment for facial rejuvenation. However, the effectiveness is questionable, and the safety and adverse effects are often not discussed. OBJECTIVE: To identify and discuss the adverse effects associated with various types of threads. MATERIALS AND METHODS: Studies describing the use of thread lifts were identified using a PubMed search. Inclusion criteria included studies in which barbed and nonbarbed threads were used for the face and neck. RESULTS: Fifty-nine articles consisting of 14,222 patients (14,134

Treatment of traumatic dislocation of the mandibular condyle into the cranial fossa: development of a probable treatment algorithm.

This study summarizes our experience of treating three rare cases of traumatic superior dislocation of the mandibular condyle into the cranial fossa and provides a potential treatment algorithm. Between the years 2002 and 2012, three patients with traumatic superior dislocation of the mandibular condyle into the cranial fossa were admitted to our department. After evaluating the interval from injury to treatment, the associated facial injuries including neurological complications, and the computed tomography imaging findings, an individualized treatment plan was developed for each patient. One…

Maxillofacial fractures sustained in bicycle accidents.

The present report is an analysis of 93 patients (52 male; 41 female) treated in 1981-1983 for maxillofacial fractures sustained in bicycle accidents. These accidents accounted for 7.1% of all facial bone fractures treated during the same period. Most accidents were single vehicle accidents (88%) and took place during the summer months. Of the patients, 65% had mandibular fractures, 35% had midface fractures and 5% had fractures in both the middle and lower thirds of the face. Condylar fractures were by far the most common of the mandibular fractures (67%)--a result which differs from studies

Efficacy of surgical repair for the functional restoration of injured facial nerve.

BACKGROUND: Early surgical repair to restore nerve integrity has become the most commonly practiced method for managing facial nerve injury. However, the evidence for the efficacy of surgical repair for restoring the function of facial nerves remains deficient. This study evaluated the outcomes of surgical repair for facial nerve lesions. METHODS: This retrospective observational study recruited 28 patients with the diagnosis of facial nerve injury who consecutively underwent surgical repairs from September 2012 to May 2019. All related clinical data were retrospectively analyzed according to

Techniques for dissection of the facial nerve in benign parotid surgery: a cross specialty survey of oral and maxillofacial and ear nose and throat surgeons in the UK.

We investigated techniques of dissection of the facial nerve currently being used in parotid surgery for benign disease in oral and maxillofacial (OMF) and ear, nose and throat (ENT) surgery. A postal questionnaire was sent to 300 OMF and 300 ENT consultants and 49% were returned(171(57%) OMF and 123 (41%) ENT. The antegrade technique was used routinely by 209 surgeons (87%), the retrograde technique by 9 (4%), and both techniques combined by 21 surgeons (9%). The antegrade technique was used by 135 surgeons (56%) for revision parotidectomy, by 193 (81%) for limited superficial parotidectomy,

Sialendoscopy as treatment of face aesthetic surgery complications: technical note.

OBJECTIVE: The demand for cosmetic surgery on the face and body has increased worldwide. Brazil is currently the world leader in open plastic surgery and second on facelift, buccal fat removal, injectables (fillers), and thread lifts. The incidence of injuries to salivary glands, ducts, and facial nerves related to cosmetic procedures is uncommon but devastating. Treatment includes dressings, botulinum toxin, anticholinergics, drainages and salivary gland resection. Sialendoscopy has been successfully described for the treatment of ductal diseases with gland preservation. We present five cases

Application of Secondary Intention Healing in Repairing Facial Defects: A Practical Review for General Surgeons.

Secondary intention healing (SIH) is an underappreciated yet valuable method for repairing facial skin and soft tissue defects, particularly for general surgeons practicing in trauma, rural, or community settings. This review reframes SIH from a general surgical perspective, focusing on clinical decision-making-including core principles, indications, contraindications, technical essentials, and referral criteria. We integrate evidence from recent years to highlight when SIH can safely yield acceptable functional and cosmetic outcomes, and when referral for advanced reconstruction is necessary.

Associated injuries are a common feature in pediatric facial fractures, which reinforce the importance of a complete initial assessment of a child with facial trauma.Particular kinds of fracture tend to occur at specific anatomic sites in children, with the severity and extent of the fracture varying according to the patient's age and the stage of skeletal development. The frequency of severe fractures increases with age, as does the frequency of surgically treated fractures. Maxillofacial surgical intervention is indicated only for the repair of severely displaced fractures that are likely to

Prospective analysis of mid-facial fractures in a single-center pediatric-adolescent cohort.

BACKGROUND: The complex architecture of the midface renders diagnosing and treating fractures challenging, especially for young patients who present the additional risk of suffering growth and development deficiencies, which is to be avoided at all costs. OBJECTIVES: This study sought to characterize pediatric mid-facial fractures considering the possible complications. METHODS: Between September 2008 and September 2018, data was collected on inpatients aged <18 years, treated for mid-facial fractures at the Halle University Hospital. Evaluated parameters were age, gender, cause and type of

Brain dead or not? CT angiogram yielding false-negative result on brain death confirmation.

We describe a case of severe traumatic brain injury with multiple facial and skull fractures where CT angiogram (CTA) failed to yield a defi … re refractory to medical management. Clinical confirmation on brain death was not feasible owing to facial injuries. A CTA, performed to determine brain perfusion, yielded a 'false-negative' result. Skull fractures have possibly led to venous prominence in the cortical and deep venous drainage system. This point needs to be borne in mind while considering CTA as …

A Comparative Study of Dermatoscopic Features of Acne-related Postinflammatory Hyperpigmentation in Facial and Nonfacial Areas in Asian Patients.

BACKGROUND: Postinflammatory hyperpigmentation (PIH) is a common problem, especially in patients with darker skin tones. It can occur on any area of the body following external injuries or intense inflammatory conditions. However, there is limited evidence regarding the differences in dermatoscopic patterns between facial acne-related PIH and nonfacial acne-related PIH. OBJECTIVE: We sought to determine the dermatoscopic features of acne-related PIH in facial and nonfacial areas in an Asian population. METHODS: Patients with acne-related PIH in both facial and nonfacial areas were enrolled.

Identifying patients at risk for intracranial and extracranial blunt carotid injuries.

BACKGROUND: Blunt carotid injuries are rare, often occult, and potentially devastating. Angiographic screening programs have detected this injury in up to 1% of blunt trauma patients. Implementing a liberal angiographic screening program at our hospital is impractical and we want to identify a high-risk group to target for screening. We hypothesize that intracranial and extracranial carotid injuries have different risks, presentations, and outcomes. METHODS: Patients with intracranial and extracranial carotid injuries were identified from the British Columbia trauma registry. Presentation and

Masseteric nerve transfer for short-term facial paralysis following skull base surgery.

BACKGROUND: Nerve transfers have been widely used to reanimate paralyzed facial muscles after irreversible proximal injuries to the facial nerve. The author has developed a technique involving masseteric nerve transfer combined with cross-facial nerve grafting for treating skull base surgery-induced facial paralysis. This paper aims to demonstrate that this procedure is effective and causes negligible donor site morbidity. METHODS: Seven patients who developed facial paralysis after the removal of skull base tumors were treated with masseteric nerve transfer combined with cross-facial nerve g
