BACKGROUND/OBJECTIVES: Fetal growth restriction (FGR), formerly known as intrauterine growth retardation (IUGR), is defined as a fetus' failure to reach its genetically predetermined growth potential. FGR has also been implicated in the development of autism spectrum disorders (ASD) and attention-deficit/hyperactivity disorder (ADHD), though strong supporting literature has yet to be published. This study aims to review the existing associations between FGR and autism spectrum disorder or attention-deficit hyperactivity disorder as studied in relevant literature, as well as the mechanisms that

Substance use during pregnancy and the short- and long-term impacts of different substances on maternal, fetal, and longer-term health outcomes of individuals prenatally exposed have been the subject of much investigation. Alcohol has been recognized as harmful during pregnancy and has been clearly recognized as a neurobehavioral teratogen, and the pattern of effects has been termed fetal alcohol spectrum disorder. More recently, the effects of prenatal cannabis exposure have been vigorously explored as a priority research area following decriminalization/legalization of cannabis in Canada and

Ethanol causes fetal alcohol spectrum disorders (FASDs) partly by inhibiting cell adhesion mediated by the L1 neural cell adhesion molecule. Ethanol interacts with an alcohol binding pocket in the L1 extracellular domain (ECD), and dephosphorylation of S1248 in the L1 cytoplasmic domain (CD) renders L1 adhesion insensitive to inhibition by ethanol (L1 insensitive). The mechanism underlying this inside-out signaling is unknown. Here we show that phosphorylation of the human L1-CD at S1152, Y1176, S1181, and S1248 renders L1 sensitive to ethanol by promoting L1 coupling with ankyrin-G and the sp…

Fetal alcohol spectrum disorder (FASD) is a preventable cause of developmental disabilities linked to prenatal alcohol exposure (PAE). Congenital heart defects (CHDs) are frequently observed in FASD, with a notable association between PAE and dextro-type transposition of the great arteries (d-TGA). A potential pathogenetic mechanism of d-TGA in FASD, involving retinoic acid (RA) deficiency due to the interference of ethanol with RA biosynthesis, is proposed. Further investigation is required to understand the timing and impact of alcohol exposure on congenital anomalies, particularly in the co

Many neurodevelopmental disorders affect early brain development in ways that are still poorly understood; yet, these disorders can place an enormous toll on patients, families, and society as a whole and affect all aspects of daily living for patients and their families. We describe a pragmatic, evidence-based framework for engaging in empiric ethics inquiry for a large consortium of researchers in neurodevelopmental disorders and provide relevant case studies of pragmatic neuroethics. The 3 neurodevelopmental disorders that are at the focus of our research, cerebral palsy (CP), autism spectr

Alcohol consumption during pregnancy remains a common occurrence and is associated with a multitude of adverse birth and long-term outcomes. Binge drinking in particular is shown to be particularly harmful to the developing fetus. Effects include full fetal alcohol syndrome, with characteristic facial dysmorphology, growth restriction, and developmental to delays. Exposed children may also have partial fetal alcohol syndrome, alcohol-related birth defects, and alcohol-related neurodevelopmental disorders. These effects are preventable, and efforts must begin with accurate identification of wom

Very rarely therapeutic drug monitoring is evaluated for its ability to provide cost savings. A new article describes the cost-effectiveness of meconium analysis for fatty acid ethyl esters in the management of fetal alcohol spectrum disorder. This article is a summary and a critical appraisal of the article by Hopkins et al.

Prenatal alcohol exposure (PAE) is recognized as a leading preventable cause of birth defects, giving rise to a continuum of cognitive, behavioral, and physical impairments collectively referred to as Fetal Alcohol Spectrum Disorders (FASD). While PAE affects multiple developing organ systems, the fetal brain is particularly vulnerable, exhibiting enduring structural and functional abnormalities in response to alcohol exposure. Recent research highlights mitochondrial dysfunction as an important mechanism in the pathogenesis of alcohol-related neurodevelopmental deficits. Mitochondria are high…

BACKGROUND: Fragile X syndrome (FXS) and fetal alcohol syndrome disorders are both common causes of intellectual disability in children. When both conditions are present in the same individual, the resultant phenotype may make identification of clinical issues and management challenging.
CASE PRESENTATION: In this case report, we present a case of triplets who had significant in utero alcohol exposure; 2 of whom also have FXS and the other not having the fragile X mutation. The siblings with FXS have subtle differences in the physical phenotype compared with the other one, who has prominent f

Neurodevelopmental disorders result from impaired development or maturation of the central nervous system. Both genetic and environmental factors can contribute to the pathogenesis of these disorders; however, the exact causes are frequently complex and unclear. Individuals with neurodevelopmental disorders may have deficits with diverse manifestations, including challenges with sensory function, motor function, learning, memory, executive function, emotion, anxiety, and social ability. Although these functions are mediated by multiple brain regions, many of them are dependent on the hippocamp

The purpose of the present investigation was a comparative study of the effect of prenatal exposure to alcohol on the histological characteristics of neurons in the frontal cortex of the rats of different ages. The study was conducted on 175 outbred albino rats — the offspring of 25 females given a 15% solution of ethanol as a source of drinking throughout pregnancy. The cortex was examined at Days 2–90 after birth using histological, histochemical and morphometric methods. An increase (Days 2, 5), followed by the reduction (Days 10 and 90) of the thickness of the cortex and the size of neuron

Epigenetic mechanisms of paternal inheritance are an emerging area of interest in our efforts to understand fetal alcohol spectrum disorders. In rodent models examining maternal alcohol exposures, different maternal genetic backgrounds protect or sensitize offspring to alcohol-induced teratogenesis. However, whether maternal background can mitigate sperm-inherited alterations in developmental programming and modify the penetrance of growth defects induced by preconception paternal alcohol exposures remains unaddressed. In our previous studies examining pure C57Bl/6J crosses, the offspring of a…

The knowledge of the dangers of alcohol consumption during pregnancy isn't indeed a new issue, but the recent evidences of ethyl-glucuronide and ethyl-sulfate in meconium as novel biomarkers of prenatal ethanol exposure open new perspectives for the early diagnosis of the alcohol-related birth defects. This is crucial for a better developmental outcome of the affected patients and for preventing additional cases in at risk families. The fetal alcohol syndrome is not a single entity but represents the most severe form of a spectrum of disorders, including distinctive craniofacial alterations, s

BACKGROUND AND AIMS: Quantifying the health burden of alcohol has largely focused upon harm to drinkers, which is an underestimate. There is a growing literature on alcohol's harm to others (HTO), but it lacks the systematic transfer of HTO into a comparative risk assessment framework. This study calculated disability-adjusted life years (DALYs) for fetal alcohol spectrum disorder (FASD), interpersonal violence and traffic injury due to another's drinking.
DESIGN: This study is a disease burden analysis, using modelling of DALYs for New Zealand in 2018.
SETTING AND PARTICIPANTS: The study to
