Role of concealed conduction and allied phenomena in the genesis, maintenance, and termination of cardiac arrhythmias.

Concealed conduction is a common electrocardiographic phenomenon whereby a series of events may occur as a result of incomplete propagation of an impulse. The occurrence, maintenance, and resolution (termination) of several events such as functional block and cardiac arrhythmias are linked to concealed conduction. This phenomenon should be suspected on the surface electrocardiogram whenever an arrhythmic event occurs unexpectedly. Several electrophysiological concepts such as transseptal conduction and linking phenomenon are close allies of concealed conduction. His-bundle electrocardiography

Selected sex related differences in pathophysiology of cardiovascular system.

The annual incidence of cardiovascular diseases is age-dependently increasing both in men and women, however, the prevalence is higher in men until midlife. The higher incidence of cardiovascular disease in men than in women of similar age, and the menopause-associated increase in cardiovascular disease in women, has led to speculation that gender-related differences in sex hormones might have a key role in the development and evolution of cardiovascular disease. There are several suggested pathways in which gender and sex hormones can affect human cardiovascular system to produce original sex

The QRS Complex: Normal Activation of the Ventricles.

The ventricular conduction system starts below the His bundle, where it bifurcates into the right and left bundle branches that taper out to the subendocardial Purkinje network, which activates the ventricular myocardium. This system is responsible for the synchronized and almost simultaneous activation of both ventricles. On the surface electrocardiogram, the ventricular conduction system lies in the terminal portion of the PR interval, whereas the QRS complex composed of the electrical currents originating from ventricular depolarization. This article reviews the main electroanatomic feature

Diagnostic and Prognostic Implications of Surface Recordings from Patients with Atrioventricular Block.

The atrioventricular (AV) bridge is vulnerable to many circumstances that depress conduction. Abnormal impulse transmission may be caused by drugs, autonomic effects, or destructive processes. Type 1 (Wenckebach) AV block is owing to depressed AV nodal conduction and is recognized by a prolonging PR interval ending in a "dropped beat." Type II (Mobitz) AV block is owing to abnormal infranodal conduction, and is usually accompanied by bundle branch block. Second-degree AV block with 2:1 conduction can be a difficult problem. Third-degree (complete) AV block is a diagnosis too often rendered and

Anatomopathological changes of the cardiac conduction system in sudden cardiac death, particularly in infants: advances over the last 25 years.

Sudden cardiac death (SCD) is defined as the unexpected death without an obvious noncardiac cause that occurs within 1 h of witnessed symptom onset (established SCD) or within 24 h of unwitnessed symptom onset (probable SCD). In the United States, its incidence is 69/100,000 per year. Dysfunctions of the cardiac conduction and autonomic nervous systems are known to contribute to SCD pathogenesis, even if most clinicians and cardiovascular pathologists lack experience with detailed examination of the cardiac conduction system and fail to recognize lesions that are crucial to explain the SCD its

Risk of mortality in individuals with low QRS voltage and free of cardiovascular disease.

The prognostic significance of low QRS voltage (LQRSV) in the electrocardiogram (ECG) of individuals free of cardiovascular disease (CVD) is unclear. We evaluated the association between LQRSV and all-cause mortality in 6,440 participants (53% women, mean age 60 years) from the Third National Health and Nutrition Examination Survey, a representative sample of the US population. Participants with history of CVD or major ECG abnormalities were excluded. LQRSV was automatically defined from standard 12-lead ECG as QRS complex amplitudes of <0.5 mV in all frontal leads and/or <1.0 mV in all precor

Computer simulation of clinical electrophysiological study.

BACKGROUND: Computer simulation is an important method for the basic researches of cardiac electrophysiology to prove the mechanisms and hypotheses of cardiac arrhythmias by reproducing body surface electrocardiograms. In this research, we extend the researches of heart models to the computer simulation of clinical electrophysiological study (EPS) as a clinical application of heart models. METHOD: Through setting the standard EPS pacing protocols, including extrastimuli and incremental pacing in the heart models, we simulated the corresponding excitation propagations in the heart and the intr

[Unusual induction of the typical slow-fast AVNRT in a patient with multiple slow atrioventricular pathways].

In mammals, fertilization occurs via a comprehensive progression of events. Freshly ejaculated sperm have yet to acquire progressive motility or fertilization ability. They must first undergo a series of biochemical and physiological changes, collectively known as capacitation. Capacitation is a significant prerequisite to fertilization. During the process of capacitation, changes in membrane properties, intracellular ion concentration and the activities of enzymes, together with other protein modifications, induce multiple signaling events and pathways in defined media in vitro or in the fema

Histopathology of the conduction system in sudden infant death.

Because ventricular fibrillation or other arrhythmia have been suggested as a cause of unexplained sudden infant death, many authors focused their studies on the conduction system especially in the 1970s. These studies were not conclusive and many pathologists have abandoned the study of the conduction system. We have reviewed the conduction system of 87 sudden infant death cases between 15 days and 1 year old: 55 were SIDS cases; 17 had mild lesions insufficient to explain death and 15 cases were sudden explained deaths considered as controls. Fetal dispersion of the AV node and/or His bundle

[Pathophysiology of cardiac arrhythmias].

Cardiac arrhythmias are ambiguous symptoms. Intracardiac as well as extracardiac alterations may be responsible. There is little direct information from the ECG concerning their pathophysiological mechanism. Bradycardic as well as tachycardic arrhythmias can be the result of two different fundamental disturbances: alterations of impulse formation (automaticity) or alterations of impulse conduction. Either one of these, or both of them acting together, may be responsible for the arrhythmia. Tachycardias due to disturbances of impulse formation may be brought about by enhanced firing of regular

[Transesophageal study in the diagnostic evaluation of pre-excitation].

Electrophysiologic non-invasive transesophageal testing is compared to intracardiac study in the management of patients with Wolff-Parkinson-White (WPW) syndrome. Transesophageal study can be reliably used to identify the participation of the accessory pathway in reciprocating supraventricular tachycardia and to determine the anterograde conduction properties of the accessory pathway. Using the shortest pre-excited interval during induced atrial fibrillation, or programmed and continuous atrial transesophageal stimulation can markedly reduce the need of intracardiac evaluation. The greater saf

Nonpharmacologic treatment of the Wolff-Parkinson-White syndrome and other supraventricular tachycardias.

Nonpharmacologic treatment using surgery or intracardiac catheter ablation can be used for several forms of supraventricular tachycardia. Surgery is highly successful in preventing recurrent tachyarrhythmias in patients with the Wolff-Parkinson-White syndrome and can be recommended as first-line treatment in several situations. Catheter ablation of the atrioventricular conduction system combined with an implanted ventricular pacemaker can offer substantial relief to selected patients with drug-refractory atrial fibrillation. Technical advances in catheter ablation will undoubtedly lead to more

A nonreentrant arrhythmia due to a dual atrioventricular nodal pathway.

A 33-year-old woman presented with an atypical arrhythmia. The electrophysiologic study revealed dual atrioventricular nodal pathways. The difference in conductivity of these tracts enabled simultaneous atrioventricular conduction with a double ventricular response to a single atrial impulse. This disturbance is uncommon. Lack of awareness of the existence of dual nodal pathway can cause diagnostic difficulties and therapeutic errors.

Transcatheter electrical shock ablation of a concealed accessory pathway.

Transcatheter electrical shock ablation of a concealed posteroseptal accessory pathway was performed on a 57-year-old man with drug-refractory paroxysmal supraventricular tachycardia. A single 300-joules shock delivered at the ostium of the coronary sinus abolished conduction in the accessory pathway and resulted in long-term cure of the arrhythmia. No complications were observed during the procedure.

Atrial flutter.

Atrial flutter is a supraventricular tachydysrhythmia believed to arise from electrophysiologic disturbances in the atria. It tends to be an unstable rhythm and is usually associated with intrinsic cardiac or pulmonary disease or adverse extrinsic influences on the heart. It is due to either a reentry mechanism or an increased atrial automaticity. Atrial depolarization is regular at a rate of 260 to 340 beats per minute. With a normal atrioventricular (AV) node there is usually a physiologic second-degree block with resultant 2:1 conduction. Higher degrees of AV block can occur in patients wit
