Interaction between resting pulmonary ventilation function and cardiac autonomic function assessed by heart rate variability in young adults.

An association between ambient air pollution and reduced cardiac autonomic function assessed by heart rate variability (HRV) mainly in elderly persons has been suggested by a number of epidemiological studies, but the link between the HRV and pulmonary function in humans remains unknown although such air pollution should primarily affect pulmonary function. To clarify this link, pulmonary ventilation parameters such as oxygen uptake (V(O(2))) and carbon dioxide output (V(CO(2))), as well as the HRV with spectral analysis (high- and low-frequency components of HRV, i.e., CCV(HF) and CCV(LF), re

Modern treatment modalities for neonatal and pediatric respiratory failure.

BACKGROUND: Respiratory failure secondary to a variety of causes remains a significant cause of morbidity and mortality in the pediatric population. Newer therapies are appearing frequently in an attempt to decrease the number of deaths from this disease state. We briefly review the current literature on some of the newer modalities including: high-frequency ventilation, surfactant, liquid ventilation, and nitric oxide. We then present our experience from the past 11 years in the most invasive, yet successful, therapy for acute respiratory failure-extracorporeal membrane oxygenation (ECMO). M…

Symptomatic improvement in patients with excessive dynamic airway collapse (EDAC) following initiation of positive airway pressure therapy.

Excessive dynamic airway collapse (EDAC) is characterized by abnormal airway collapse during expiration, leading to symptoms such as breathlessness, cough, and reduced quality of life. Treatment options remain limited, and evidence-based guidelines are lacking. This prospective cohort study evaluated the impact of positive airway pressure (PAP) therapy on symptom burden in 23 patients with EDAC referred to a tertiary ventilation centre. Diagnosis was confirmed by computed tomography and/or bronchoscopy. Patients received nocturnal and ambulatory PAP, with symptom ratings and quality of life as

Treatment for hypotension in the first 24 postnatal hours and the risk of hearing loss among extremely low birth weight infants.

OBJECTIVE: To evaluate whether treated hypotension in the first 24 postnatal hours is associated with hearing loss in extremely low birth weight (ELBW) infants. STUDY DESIGN: In a cohort of 735 ELBW infants, we identified 25 with sensorineural hearing loss (SNHL) at 12-24 months adjusted age. For each case, we selected three controls with normal hearing. Logistic regression models were used to adjust for confounding variables. RESULTS: Sixty percent of cases and 25% of controls were treated for hypotension. After adjusting for confounding variables (gestational age, antenatal glucocorticoids

Current and future therapeutic options for persistent pulmonary hypertension in the newborn.

Persistent pulmonary hypertension of the newborn (PPHN) is a potentially life-threatening condition that is characterized by supra-systemic pulmonary vascular resistance causing right-to-left shunting through the ductus arteriosus and/or foramen ovale, leading to a vicious cycle of hypoxemia, acidosis and further pulmonary vasoconstriction. Advances in neonatology including surfactant instillation, high-frequency ventilation, extracorporeal membrane oxygenation and, most importantly, inhaled nitric oxide (INO), have revolutionized the management of PPHN. However, given that INO does not improv…

[Benzodiazepines in geriatrics--a case report on prolonged action].

Due to the age-dependent changes of pharmacodynamic and pharmcokinetic, the frequency of undesirable side-effects of benzodiazepines is higher in geriatric patients. An increased sensitivity to benzodiazepines and their metabolites can induce extremely prolonged duration of action leading to unconsciousness and respiratory insufficiency. Because of an assumed postoperative "psychosyndrome", a 72-year-old patient was treated with high doses of diazepam combined with single doses of tramal, clonidin and ranitidin. This treatment was followed by the development of respiratory insufficiency requir

Mortality prediction in infants with congenital diaphragmatic hernia: potential criteria for ECMO.

Over the last ten years the survival of infants born with congenital diaphragmatic hernia who reach the Intensive Care Unit of the Royal Children's Hospital, Melbourne has been constant at 56 +/- 6%. Experimental therapies such as extracorporeal membrane oxygenation, high-frequency oscillation and lung transplantation are now being considered as therapeutic options, and as such the ability to predict survival or death of these infants is increasingly important. The records of all infants with congenital diaphragmatic hernia admitted to the Intensive Care Unit between 1 January 1980 and 30 Apri

[Hemodynamic and respiratory effects of tramadol during nitrous oxide-oxygen-artificial respiration and in the postoperative period].

The haemodynamic effects of intravenous tramadol (two different doses: 0.75 mg/kg b.w. or 1.5 mg/kg b.w.) during artificial ventilation with oxygen and nitrous oxide were investigated in 20 female patients before the start of operation. Haemodynamic changes were characterized by a moderate increase in blood pressure concerning systemic as well as pulmonary circulation. These effects were due to a rise in peripheral and pulmonary vascular resistance. Application of tramadol immediately after the end of operation (n = 20:10 patients following neuroleptanalgesia and 10 patients after anesthesia w

Severe neonatal pulmonary artery hypertension rescued with vasopressin.

An inborn term neonate weighing 2600 g developed meconium aspiration syndrome at birth. Baby had respiratory failure requiring high-frequency oscillatory ventilation support at 15 hours of life. He additionally developed hypotension with left ventricular dysfunction noted on point-of-care echocardiography (POCE), which required dopamine and epinephrine infusions. At 28 hours of life, he was started on inhaled nitric oxide (iNO), followed by milrinone due to hypoxaemic respiratory failure and the POCE revealed severe pulmonary artery hypertension (PAH). As PAH was refractory to iNO and milrinon

[Acute respiratory distress due to Influenza A (H1N1) S-OIV and extracorporeal oxygenation: the benefit of a multidisciplinary care network].

Between August and September 2009, on Reunion Island, our Intensive Care Unit (ICU) treated several severe Influenza A (H1N1) S-OIV cases. We report the following case: a 23-year-old female patient with no prior medical history presented a severe respiratory distress that required high frequency oscillation ventilation and finally Extracorporeal Membrane Oxygenation (ECMO). She was hospitalized in the ICU for 41 days. Recovery was complete. It is important to note the stead of each technique in those types of respiratory distresses and describe the practical details of the ECMO's set up by a n

Delayed-onset hearing loss in respiratory distress syndrome: case reports.

The pre-, peri-, and postnatal histories for two infants who suffered respiratory distress syndrome (RDS) are presented. Each infant was diagnosed with RDS within 24 hours after birth, placed on high-frequency jet ventilation, and passed auditory brainstem response (ABR) screening prior to hospital discharge. Both infants were enrolled in a neonatal follow-up program with no report of hearing loss during the first year of life. At 2.5 years of age, each infant was found to have severe-to-profound sensorineural hearing loss. Neither infant suffered kidney failure, rubella, cytomegalovirus, or g

Group B streptococcal infection in Taiwan: maternal colonization and neonatal infection.

BACKGROUND: There is no national data on group B streptococcus (GBS) infection in Taiwan. We investigated incidence of maternal GBS colonization and neonatal GBS infection rate and clinical pictures of neonatal GBS infection to estimate the value of intrapartum chemoprophylactic strategy in Taiwan. METHODS: From January 2004 to June 2005, a prospective study to estimate maternal colonization rate by maternal rectovaginal culture at six hospitals was conducted. Neonatal GBS infection rate based on inborn infants was calculated retrospectively from January 2001 to June 2005; clinical pictures o

Continuous positive airway pressure (CPAP) for apnoea of prematurity.

BACKGROUND: Apnoea of prematurity (AoP) is defined as a pause in breathing for 20 seconds or longer, or for less than 20 seconds when accompanied by bradycardia and hypoxaemia, in a preterm infant. An association between the severity of apnoea and neurodevelopmental delay has been reported. Continuous positive airway pressure (CPAP) is a form of non-invasive ventilatory assistance that has been shown to be relatively safe and effective in preventing and treating respiratory distress among preterm infants. It is less clear whether CPAP treatment is safe and effective in the prevention and treat

Disseminated bronchiectasis in an adult with common variable immunodeficiency.

Primary immunodeficiencies (PID) are traditionally considered childhood diseases; however, adults account for 35% of all patients with PID. Antibody deficiencies, especially Common Variable Immunodeficiency (CVID), which have their peak incidence in adulthood, require a high suspicion index. Even though the estimated frequency of CVID is not high (1:25,000), high rates of under diagnosis and under reporting are very likely. The delay in diagnosis increases the morbidity and mortality; therefore, adult physicians should be able to suspect, identify and initiate management of individuals with PI

Secondary infections in a cohort of patients with COVID-19 admitted to an intensive care unit: impact of gram-negative bacterial resistance.

Some studies have shown that secondary infections during the COVID-19 pandemic may have contributed to the high mortality. Our objective was to identify the frequency, types and etiology of bacterial infections in patients with COVID-19 admitted to an intensive care unit (ICU) and to evaluate the results of ICU stay, duration of mechanical ventilation (MV) and in-hospital mortality. It was a single-center study with a retrospective cohort of patients admitted consecutively to the ICU for more than 48 h between March and May 2020. Comparisons of groups with and without ICU- acquired infection w
