Glucocorticoid-remediable aldosteronism.

Glucocorticoid-remediable aldosteronism (GRA) is a monogenic form of human hypertension that predisposes to cerebral hemorrhage. As a result of a chimeric gene duplication, aldosterone is ectopically synthesized in the cortisol-secreting zona fasciculata of the adrenal gland under the control of adrenocorticotropin (ACTH). Hypertension frequently has its onset during childhood and is usually refractory to standard anti-hypertensives such as ACE inhibitors and beta-blockers. Hypokalemia can develop in those treated with a potassium-wasting diuretic, but random potassium levels are usually norma

[Occurrence of primary aldosteronism in a group of ambulatory hypertensive patients].

Findings pertaining to the diagnosis and treatment of primary aldosteronism are rapidly expanding. In the present work the authors focused attention on the clinical application of some progressive methods. They examined a group of 115 ambulatory patients with arterial hypertension, not suspected of secondary arterial hypertension, in the course of one year. As a screening method of primary aldosteronism they selected the aldosterone renin ratio (ARR). Using this method the authors diagnosed 125 cases of primary aldosteronism, i.e. a 13% prevalence in the examined group. Only in one instance th

Primary aldosteronism. Issues in diagnosis and management.

The clinical manifestations of primary aldosteronism are not distinctive, but additional studies are warranted in certain hypertensive patients, including patients with either spontaneous or diuretic-induced hypokalemia and those with refractory hypertension without an obvious secondary cause. The best test for identifying patients with primary aldosteronism is measuring the aldosterone excretion rate during salt loading. A rate exceeding 14 micrograms/24 hour provides the highest sensitivity and specificity. The presence of hypokalemia and suppressed plasma renin activity provides corroborati

Medical treatment of low-renin aldosteronism.

Among the subsets of LRA, IHA and GSA should always be treated medically. APA and APC should be treated surgically if possible, with unilateral adrenalectomy. First-line treatment of IHA and for preoperative preparation of APA and APC patients is the aldosterone antagonist spironolactone. If this agent cannot be used because of side effects, sodium transport inhibitors are appropriate alternatives. Patients with IHA and APA can also be treated with calcium channel blockers, but only nifedipine has been tested in these conditions. IHA can probably also be treated with the converting enzyme inhi

Secondary hypertension as a cause of treatment resistance.

In secondary hypertension, elevated blood pressure is caused by a known and/or potentially treatable underlying disease.Although the prevalence of secondary hypertension depends on the patient population and the thoroughness of applied diagnostic approaches, arterial hypertension is classified in 90 to 95% as primary in nature. In young patients, individuals without a family history of hypertension, late onset of hypertension or worsening of a previous well-controlled hypertension as well as in patients who have a difficult to treat hypertension, the prevalence of secondary hypertension is sig

The outcomes of transcatheter adrenal ablation in patients with primary aldosteronism: a systematic review and meta-analysis.

BACKGROUND: The use of transcatheter adrenal ablation as an alternative treatment for primary aldosteronism (PA) patients remains a subject of debate, with outcomes varying widely across existing studies. This meta-analysis aims to evaluate the results of adrenal ablation and estimate the effectiveness and safety of this therapeutic approach. METHODS: A comprehensive search was conducted across PubMed, Embase, and Cochrane Library databases for studies published up to October 2022. Outcomes analyzed included the combined clinical success rate, biochemical success rate, and complication rate,
