Prenatal Earthquake Exposure and Midlife Uric Acid Levels Among Chinese Adults.

OBJECTIVE: To test whether prenatal exposure to earthquake (as a surrogate for acute prenatal stress) could have unfavorable effects on uric … and other potential confounders. We also used multiple logistic regression to estimate the risk of hyperuricemia (>416 μmole/liter in men or >357 μmole/liter in women) in 2010 by calculating the odds ratios (ORs) and 95% confidence intervals (95% CIs) after adjustment for the previously mentioned covariates. RESULTS: Participants with prenatal exposure to the …

Multiple flesh coloured nodules with unilateral segmental distribution.

BACKGROUND: Anemia in infancy is a frequent clinical challenge, often attributed to nutritional deficiencies. However, persistent or unexplained anemia warrants further investigation. In this case report, we describe an infant in whom anemia was first identified at six months of age; however, the underlying etiology became apparent only after the development of elevated serum creatinine and hyperuricemia. CASE PRESENTATION: We present a 1.7-year-old boy with persistent normocytic anemia, hyperuricemia, and elevated creatinine, initially evaluated for hematologic causes without a clear diagnos…

A Case of Allopurinol-Induced Drug Reaction with Eosinophilia and Systemic Symptoms in a Patient With Polycystic Kidney Disease and Chronic Kidney Disease.

INTRODUCTION: Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a severe drug hypersensitivity reaction. CASE PRESENTATION: A 59-year-old male with a history of stage IV chronic kidney disease, polycystic kidney disease, hypertension, and hyperuricemia on allopurinol presented to the emergency department directly from an outpatient nephrology appointment with concern for severe DRESS syndrome with acute-on-chronic kidney failure, liver failure, and pancreatic involvement. DISCUSSION: The existing literature on the course of DRESS syndrome in patients with preexisting …

Controversies and practical management of patients with gout and chronic kidney disease.

Uric acid is a toxin retained with advancing kidney disease. Clinical manifestations of hyperuricemia include gout and systemic inflammation that are associated with increased risk of cardiovascular mortality. As many as one-third of all patients with chronic kidney disease have a history of gout, yet <25% of these patients are effectively treated to target serum urate levels of ≤6 mg/dl. A major reason for ineffective management of gout and hyperuricemia is the complexity in managing these patients, with some medications contraindicated and others requiring special dosing, potential drug inte…

Anaerobic purinolytic enzymes enable dietary purine clearance by engineered gut bacteria.

Uric acid, the end product of purine degradation, causes hyperuricemia and gout, afflicting hundreds of millions of people. The debilitating effects of gout are exacerbated by dietary purine intake, and thus a potential therapeutic strategy is to enhance purine degradation in the gut microbiome. Aerobic purine degradation involves oxidative dearomatization of uric acid catalyzed by the O2-dependent uricase. The enzymes involved in purine degradation in strictly anaerobic bacteria remain unknown. Here we report the identification and characterization of these enzymes, which include four hydrola…

Febuxostat and renal outcomes: post-hoc analysis of a randomized trial.

Effect of urate-lowering on renal outcomes in patients at high-risk for cardiovascular disease with hyperuricemia without gout is not known. We conducted a post hoc analysis of a randomized trial (Febuxostat for Cerebral and CaRdiorenovascular Events PrEvEntion StuDy [FREED]). The FREED trial enrolled 1070 asymptomatic, hyperuricemic elderly patients with at least one risk factor for cardiovascular disease, divided into febuxostat (n = 537) and non-febuxostat (n = 533) groups. We compared the effect of these treatments on renal outcomes including 40% decline in estimated glomerular filtration …

Why focus on uric acid?

In gouty patients, urate lowering therapies (ULTs) are recommended to bring serum uric acid (SUA) levels below 6.0 mg/dL, with the aim of dissolving urate depositions, thereby reducing disease impact. However, patients with hyperuricemia often present with other conditions associated with cardiovascular (CV) risk, such as high blood pressure, obesity, insulin resistance, fatty liver, and chronic kidney disease. In the last decade, several well grounded pieces of evidence showed that the elevation of uric acid often occurs prior to the development of hypertension or metabolic syndrome, thus sug…

Is the apolipoprotein E4 allele always hazardous? Serum uric acid level as a conflict.

Hyperuricemia, dyslipidemia, and apolipoprotein E (apoE) polymorphism are risk factors for cardiovascular diseases (CVDs). This study sought to determine the association of apoE gene polymorphism with hyperuricemia and dyslipidemia in young healthy people. Association of serum uric acid (SUA) and serum lipids with apoE was studied by analysis of variance in 198 healthy southern Iranian candidates. Subjects with an E2/E3 genotype had a lower cholesterol level in comparison with E3/E3 individuals. In addition, male E3/E4 subjects had a lower SUA level in comparison with other men. Lower choleste…

Gout and related morbid conditions: pharmacological and SPA therapy.

Gouty arthritis is estimated to be the most frequent manifestation of inflammatory arthritis in men aged over 40. Hyperuricemia occurs because of both exogenous and genetic factors, which are particularly influential in some populations such as Taiwan aborigines. Current understanding of the disease etiopathogenesis, its clinical manifestations and the stages of its progression are presented here. The criteria for a correct diagnosis of the disease are also reported, pointing out how to distinguish gout from clinical events of different origin but with a very similar symptomatology. A distinct…

Febuxostat for prevention of gout attacks.

(1) Febuxostat is a selective inhibitor of xanthine oxidase. Its use in the management of hyperuricemia and gout is being studied. (2) In a 52-week, phase III randomized clinical trial, febuxostat was superior to allopurinol for lowering uric acid levels. Its efficacy in preventing gout attacks was similar to that of allopurinol. Despite a similar rate of adverse effects, individuals on febuxostat were more likely to stop treatment than those on allopurinol. (3) The most commonly observed adverse effects with febuxostat include liver function test abnormalities, diarrhea, headache, nausea, vom…

Allopurinol-induced DRESS syndrome.

Hyperuricemia is present in approximately 5% of the population, the vast majority of whom are asymptomatic and at no clinical risk. Complications, including renal calculi, uric acid nephropathy and gout, occur in a small proportion of patients. Allopurinol, an analog of hypoxanthine, has been widely used in clinical practice for over 30 years for the treatment of hyperuricemia and gout. Two percent of patients taking this medication develop a mild exanthema. A syndrome characterized by exfoliative dermatitis, hepatitis, interstitial nephritis and eosinophilia has been described previously. Ter…

Neurocognitive functioning in Lesch-Nyhan disease and partial hypoxanthine-guanine phosphoribosyltransferase deficiency.

Lesch-Nyhan disease (LND) is a rare, X-linked genetic disorder that involves the nearly complete absence of an enzyme (hypoxanthine-guanine phosphoribosyltransferase, or HPRT) that is essential for purine salvage. In addition to hyperuricemia, all patients with classic LND suffer from movement disorder and compulsive self-injury, and most have mental retardation. Patients with partial HPRT deficiency (variants) always have hyperuricemia and often have neurologic abnormalities, but do not self-injure and usually are described as having normal intelligence. Here we compare 15 patients with LND t…

The kidney: an unwilling accomplice in syndrome X.

The ability of insulin to stimulate glucose disposal by muscle varies widely within the population at large. Individuals with muscle insulin … lthough the kidney is not considered to be an insulin sensitive tissue, two features of syndrome X, hyperuricemia and hypertension, are likely to be dependent on the retention of normal insulin action on the kidney. 102 patients with EPH-gestosis in different stages of severity and 29 patients with normal pregnancy were examined regularly in a longitudin … rs have not been useful at this early stage in the detection of likely development of EPH-gestosis. Hyperuricemia is primarily due to the isolated reduction of uric acid clearance by normal glomerular filtration rate.
