Prevention of urinary tract infections.

Potential candidates for measures designed to prevent urinary tract infections include (1) women (or girls) with frequent exogenous reinfections; (2) pregnant women; (3) hospitalized patients with short-term indwelling catheters; (4) men with chronic bacterial prostatitis; (5) patients with neurogenic bladders being managed with either intermittent catheterization or chronic indwelling catheters; and (6) men undergoing urologic surgery. Patients in the first three categories represent the vast majority of men and women with preventable urinary tract infections. On the basis of our current unde…

Moxalactam therapy of serious infections.

Twenty-four patients were treated with moxalactam for 25 serious infections. Nineteen patients were septicemic and 18 presented severe underlying diseases considered to impair the normal response to bacterial pathogens. All of the pathogens had MICs of less than 12 mg/l except one Pseudomonas aeruginosa strain with an MIC of 32 mg/l. The dosage ranged from 3 to 12 g/day; the route of administration was either i.v. or i.m. The duration of treatment was six to 26 days. Six patients had urinary tract infections (three bacteremia), four had pulmonary abscesses (two bacteremia), five had septic thr…

Enterovirus infections in the neonate.

A chart review was made of 24 neonates less than one month of age with culture-proven enteroviral infection. The seasonal distribution was summer and fall. An antecedent illness was common in the mother or other family members. One patient had a mild gastroenteritis. Three categories of severe disease were noted: (a) meningitis accounted for 50 per cent of the illnesses; (b) myocarditis for 25 per cent of the illnesses; and (c) the remainder presented with a severe sepsis-like illness. High mortality rate was associated with low birth weight and low gestational age.

Culture diagnosis of Legionella infections.

Culture diagnosis of Legionella infections is feasible in most instances. Using selective culture techniques, sputum can be used to diagnose these infections specifically, and with a degree of sensitivity equal to or greater than currently used immunological means of diagnosis. Every laboratory performing diagnostic testing for this group of infections should perform cultures to achieve optimal yield.

Orofacial infections of odontogenic origin.

Purulent infections of the orofacial region are often of odontogenic origin. The clinical manifestations are largely dependent on the complex microbiota and the anatomical routes of spread. Serious complications may ensue, especially if hematogenous spread occurs. Direct invasion of different anatomical spaces may cause mediastinitis, airway obstruction and intracranial spread. Proper surgical treatment is mandatory to evacuate pus as soon as abscess formation is recognized. Anaerobic bacteria belonging to the normal oropharyngeal flora are usually isolated from these abscesses. The drug of ch…

[Cardiac complications in meningococcal infections].

The clinical courses of three children with meningococcal meningitis and cardiac complications are described. In two patients a mixed peri/myocardial form of cardiac involvement is probable. One child showed a pericarditis with relapses as found in the postpericardiotomy syndrome. The pathogenesis of the cardiac involvement in meningococcal infections is discussed based on the literature. The three cases observed are tentatively classified accordingly.

[Exfoliatin in neonatal staphylococcal infections].

An outbreak of staphylococcal skin infection in neonates was investigated clinically, bacteriologically and epidemiologically with the following findings: (1) In 8 out of 13 cases, exfoliatin-producing staphylococci were present in the bullae, which is unusual with bullous lesions occurring at other ages; (2) exfoliatin producing staphylococci were present in all children with bullous lesions, as well as in carriers; (3) 39% of the phage II group staphylococci studied produced exfoliatin; (4) purulent lesions due to phage II staphylococci which did not produce exfoliatin were observed. The con

Pernicious syndromes in Plasmodium infections.

Records of 560 male patients with Plasmodium vivax and P. falciparum infection, who were admitted to a military hospital in the northeastern sector of India during the period from June, 1978, to June, 1980, have been analysed to establish the prevalence of pernicious syndromes. One hundred and seventy-eight patients (31.8%) had P. vivax malaria, 309 (55.1%) had P. falciparum malaria, and the remaining 73 (13.1%) were found to have mixed infection (P. vivax and P. falciparum). Pernicious syndromes with single or multiple organ involvement, of which cerebral and haemopoietic complications were t
