Intra-arterial thrombolysis in a patient presenting with an ischemic stroke due to spontaneous internal carotid artery dissection.

We describe a case of a 38-year-old male who presented with acute onset of right-sided hemiplegia and aphasia, who was transferred for emergent percutaneous intervention. Angiography revealed a dissection with total occlusion of the left internal carotid artery (ICA) with propagation of thrombus in the distribution of the middle cerebral artery (MCA). Therapy was directed at the MCA and not the ICA. Intra-arterial thrombolysis was performed on the M1 and M2 branches of the left middle cerebral artery, resulting in almost complete resolution of symptoms during the angiography procedure. Heparin

Hepatic arterial infusion chemotherapy (HAIC) combined with sequential or concurrent systemic targeted immunotherapy for advanced hepatocellular carcinoma: a single-center retrospective study.

Triple therapy of hepatic arterial infusion chemotherapy (HAIC) combined with immune checkpoint inhibitors (ICIs) and tyrosine kinase inhibitors (TKIs) achieves satisfactory clinical efficacy in advanced hepatocellular carcinoma (HCC). However, the optimal therapeutic strategy to improve prognosis in this patient population remains controversial. The objective of this retrospective study was to evaluate the efficacy and safety of HAIC-based therapy, either sequentially (SE) or concurrently (Con) combined with targeted immunotherapy, in patients with Barcelona Clinic Liver Cancer (BCLC) stage C

Hepatic arterial infusion chemotherapy combined with apatinib plus camrelizumab for advanced hepatocellular carcinoma with type Vp4 portal vein tumor thrombosis: a multicenter propensity score-matching analysis.

BACKGROUND: Portal vein main trunk invasion is a serious and difficult complication of hepatocellular carcinoma (HCC), with extremely poor prognosis and limited treatment options. The traditional standard sorafenib has a limited efficacy. The combination of hepatic arterial infusion chemotherapy (HAIC) with camrelizumab and apatinib has shown satisfactory efficacy in previously advanced HCC. Therefore, this approach has potential advantageous survival benefits for HCC with invasion of the portal vein main trunk. METHODS: A retrospective review was conducted on the clinical data of advanced HC

Hepatic arterial infusion chemotherapy plus tyrosine kinase inhibitors with or without PD-1 inhibitors for advanced hepatocellular carcinoma with VP4 portal vein tumor thrombosis: a retrospective cohort study.

BACKGROUND: Hepatocellular carcinoma (HCC) with VP4 portal vein tumor thrombosis (PVTT) has a poor prognosis. Although hepatic arterial infusion chemotherapy (HAIC) combined with tyrosine kinase inhibitors (TKIs) and programmed cell death protein 1 (PD-1) inhibitors shows activity in advanced HCC with PVTT, whether adding PD-1 inhibitors to HAIC plus TKIs improves outcomes specifically in VP4 PVTT remains unclear. METHODS: This single-center retrospective study enrolled consecutive treatment-naïve patients with advanced HCC and VP4 PVTT who received HAIC plus TKIs (dual therapy) or HAIC plus

The association between sarcopenia and the prognosis of patients with high tumor burden hepatocellular carcinoma receiving hepatic artery infusion chemotherapy combined with lenvatinib and programmed death receptor-1 inhibitor: a multicenter, propensity score matching comparative study.

BACKGROUND AND OBJECTIVE: Patients with hepatocellular carcinoma (HCC) and a high tumor burden, such as those presenting with a tumor diameter of ≥ 10 cm or/and major portal vein tumor thrombosis (PVTT), generally face a poor prognosis. Sarcopenia is common in this population. Triple therapy combining hepatic arterial infusion chemotherapy (HAIC), lenvatinib, and programmed death receptor-1 (PD-1) inhibitors has shown encouraging outcomes in these patients. This study investigated the association between sarcopenia and the prognosis in patients with high tumor burden HCC patients receiving thi

Efficacy and safety of lenvatinib plus durvalumab combined with hepatic arterial infusion chemotherapy for unresectable intrahepatic cholangiocarcinoma.

BACKGROUND: The prognosis for unresectable intrahepatic cholangiocarcinoma (ICC) is poor and the efficacy of traditional chemotherapy remains unsatisfactory. Hepatic arterial infusion chemotherapy (HAIC) with oxaliplatin, leucovorin, and 5-fluorouracil (FOLFOX) is effective in patients with unresectable ICC. In this study, we determined the preliminary clinical efficacy and safety of lenvatinib plus durvalumab combined with FOLFOX-HAIC in patients with untreated, unresectable ICC. MATERIALS AND METHODS: Between July 2021 and July 2023, patients with unresectable ICC who initially received len

A pilot study of hepatic arterial infusion of chemotherapy for patients with advanced hepatocellular carcinoma who have failed anti-angiogenic therapy.

BACKGROUND & AIMS: For patients with advanced hepatocellular carcinoma (HCC) who have failed first-line anti-angiogenic therapy, there is no salvage treatment. Hepatic arterial infusion of chemotherapy (HAIC) has been reported to achieve substantial treatment responses in HCC patients. We aimed to explore the feasibility of using HAIC as second-line therapy for advanced HCC. METHODS: We retrospectively reviewed all consecutive patients who received HAIC for advanced HCC after failure of first-line anti-angiogenic therapy at a single institute. Patients received HAIC with 60 mg/m(2) cisplatin

Transarterial Infusion of Pembrolizumab Plus TACE for Liver Metastases of Melanoma.

BACKGROUND: Melanoma hepatic metastases demonstrate limited responsiveness to conventional systemic immune checkpoint blockade therapy, primarily attributed to the hepatic microenvironment's immunosuppressive characteristics. To our knowledge, this is the first reported case describing selective hepatic arterial infusion of pembrolizumab immediately followed by transarterial chemoembolization (TACE) for liver-dominant melanoma metastases after progression on systemic therapy. CASE PRESENTATION: This case report describes an alternative therapeutic strategy employed for a 61-year-old female pa

Locoregional Treatment Options for Locally Advanced Intrahepatic Cholangiocarcinoma.

Intrahepatic cholangiocarcinoma (ICC) is an aggressive primary liver malignancy. Although surgical resection remains the standard of care, most patients present with either metastatic or locally advanced, unresectable disease. Although effective systemic therapy is paramount in these situations, locoregional tumor control frequently delays liver-related complications and mortality. To address this challenge, a variety of liver-directed therapies have emerged, including transarterial chemoembolization, transarterial radioembolization with yttrium-90, hepatic arterial infusion chemotherapy, exte

The influence of nebivolol on the activity of BRL 37344 - the β3-adrenergic receptor agonist, in the animal model of detrusor overactivity.

AIMS: The cardiotoxic effects of antimuscarinics constitute a significant restriction in their application in elderly people. Overactive bladder syndrome pharmacotherapy using compounds with cardioprotective properties would seem an ideal solution. The main goal of the study was to assess the impacts of nebivolol (NEB) on the activity of BRL 37344 - β3-adrenergic receptor (β3AR) agonist, in the animal model of detrusor overactivity. As both these substances can impact on the cardiovascular system, their effect on the parameters of this system and diuresis was also examined. METHODS: Retinyl a
