Traumatic Obturator Hip Dislocation: Injury Characteristics and Clinical Outcomes.

Purpose Traumatic hip dislocation into the obturator foramen is poorly understood due to the rarity of the injury. The purpose of this study was to analyze the mechanism of trauma, associated injuries, treatment, and complications in patients who sustained obturator hip dislocation and to evaluate long-term outcomes, including patient-reported outcome measures (PROMs). Methods Patient demographics, trauma mechanisms, concomitant injuries, and the treatment of all consecutive patients who sustained an obturator hip dislocation at three level-one trauma centers from 2009 to 2024 were analyzed. Femoral trochlear dysplasia after patellar dislocation in rabbits.

BACKGROUND: The aim of this study was to investigate the effects of patellar dislocation on the development of the trochlear groove in rabbits. METHODS: Eighty knees from forty one-month-old white rabbits were divided into three groups (sham surgery, experimental, control). Lateral patellar dislocation was established in the experimental group and the effect on the development of the trochlear groove was compared with that in the sham surgery and control groups. Computed tomography (CT) scans were conducted to measure the angle, width and depth of the trochlear groove one month and six months

Functional treatment of congenital dislocation of the hip.

A prerequisite for the development of congenital dislocation of the hip is ligamentous laxity of the hip-joint, which is demonstrable shortly after birth when the hip can be easily dislocated and reduced. This situation is described as dislocatable hip. Ligamentous laxity naturally tends to disappear, and 90% of all dislocatable hips in fact stabilize spontaneously. No dislocation develops unless an exogenous factor is involved. In infants born in breech presentation the hyperextension of the knees with the resulting increased tension in the hamstrings plays a role. During or shortly after bir

Traumatic obturator dislocation of the hip joint: About 2 cases and review of the literature.

INTRODUCTION: Traumatic dislocation of the hip is a serious lesion, and its obturator variety is rarely reported. It always reflects a high-energy trauma and represents a therapeutic emergency that requires a reduction in less than 6 h due to the risk of aseptic necrosis of the femoral head in the long term. CASE REPORT: We reported 2 cases of hip obturator dislocation in patients aged 21 and 45 years respectively, which occurred following a road accident, received for pain and functional impairment of the hip, and in a typical attitude with hip flexion, abduction, rotation and knee flexion w

[Dealing with early complications in unicondylar knee arthroplasty-what works, what does not?].

Complications in the early postoperative period following the implantation of a unicondylar knee arthroplasty (UKA) are both rare and rarely described in the literature. Often, only small case series or individual case reports are available. In this article, the most common complications of periprosthetic infection, periprosthetic tibial fracture, inlay dislocation and intra-articular cement residue in (loose bone cement body) are described and recommendations for conservative and surgical treatment are presented, including case examples. Ideally, surgical errors or an infection should be reco

Distal arthrogryposis type 5D with a novel ECEL1 gene mutation.

Distal arthrogryposis syndromes (DAs) show wide clinical variability and overlapping clinical findings with the other DAs classified by Bamshad et al. [1996]. Most of the DAs are inherited as autosomal dominant disorders. DA type 5D is a subtype of DA type 5 inherited as autosomal recessive disorder, clinically characterized by congenital distal joint contractures, knee extension contractures, congenital hip dislocation, club foot, ptosis and other eye findings, furrowed tongue, and scoliosis. Here, we report on a family with clinical features of DA type 5D with novel mutations in the ECEL1 ge

Hip dislocation in cerebral palsy: selection for treatment.

Forty-five patients with cerebral palsy hip-dislocation were reviewed and it was found that some did well without treatment. For patients with hip pain, the best results were obtained with extensive resection arthroplasty: soft-tissue surgery alone was unsuccessful. Open reduction with femoral osteotomy achieved concentric reduction in all 15 patients who had this type of surgery: however, most retained their abnormal postural reflex activity to such a degree that subsequent scoliosis, pelvic obliquity, hip extension contractures and knee flexion deformities compromised the success of the hip

The association between meniscal subluxation and cartilage degeneration.

PURPOSE: The menisci play a critical protective role for the knee joint through shock absorption and load distribution. We hypothesized that cartilage degeneration will be abruptly progressed if meniscal subluxation exceeds a critical point. METHODS: Of 56 cases that showed medial meniscal subluxation without cartilage degeneration of ipsilateral medial femoral condyle (MFC) on initial MRI, from January 2005 to June 2007, meniscal subluxation index (MSI), the ratio of meniscal overhang to meniscal width in mid-coronal image of initial MRI, was measured. After 2 years, 40 cases were evaluated

Avulsion fractures and chronic avulsion injuries of the knee: role of MR imaging.

Avulsion fractures and chronic avulsion injuries of the knee are common lesions in sports-related trauma, especially among adolescents. Magnetic resonance imaging may prove useful in detecting and characterizing such lesions, and has several advantages with regard to other imaging modalities. We review, illustrate, and discuss the MR imaging features of some of the more frequent avulsion fractures and chronic avulsion injuries of the knee, including avulsion fractures of the cruciate ligaments, avulsion fractures of lateral and medial stabilizers, avulsion fractures and chronic avulsion injuri

Patellofemoral anatomy and biomechanics: current concepts.

The patellofemoral joint, due to its particular bone anatomy and the numerous capsuloligamentous structures and muscles that act dynamically on the patella, is considered one of the most complex joints in the human body from the biomechanical point of view. The medial patellofemoral ligament (MPFL) has been demonstrated to contribute 60% of the force that opposes lateral displacement of the patella, and MPFL injury results in an approximately 50% reduction in the force needed to dislocate the patella laterally with the knee extended. For this reason, recent years have seen a growing interest i

Arthroscopically assisted proximal extensor mechanism realignment of the knee.

An arthroscopically assisted proximal extensor mechanism realignment procedure has been designed and performed on 27 knees in 24 patients. The study group has been followed a minimum of 18 months. The procedure consists of a lateral retinacular release and an arthroscopically controlled plication of the medial patellar retinaculum and oblique fibers of the vastus medialis. The indications for this procedure include recurrent patellar instability and acute initial patellar dislocation with a concomitant fracture of either the patella or lateral margin of the intertrochlear sulcus. Pediatric trauma: getting through the night.

As is the case in adults, the timing and type of emergent treatment of fractures in children can be controversial. Some emergent conditions, such as compartment syndrome, pelvic fractures with hemodynamic instability, and open fractures, are managed similarly in adults and children. However, other types of fractures are unique to children or are managed differently in children and adults. To prevent complications, it is important to understand the appropriate treatment and timing of treatment of supracondylar humeral fractures and other specific elbow fractures, hip fractures and dislocations,

Localized pigmented villonodular synovitis presenting as recurrent dislocation of the patella.

A 31-year-old woman presented with a 1-year history of recurrent dislocation of the patella. On examination, she was found to have a mass at the medial patellofemoral joint line. Arthroscopy showed this to be a vascular synovial tumor extending into the medial patellofemoral joint and causing lateral deviation of the patella. The mass was resected with resolution of her symptoms and histologic examination showed this to be pigmented villonodular synovitis. There are an increasing number of case reports of these lesions causing varying mechanical symptoms in the knee and they should be consider

[Arthrogryposis multiplex congenita with associated abnormalities. Case report of a fatal course in a premature triplet].

A male premature triplet with healthy siblings was born with characteristic symptoms of a Rossi syndrome: pterygiums at neck, axillae, elbows and in a moderate way at hips and knees with inhibition of extension of the affected joints, dislocation of the right hip, club-feet, cutis laxa et hyperelastica, facial dysmorphism, double pelvis renalis with ureter duplex on the right side, kryptorchidism, hypoplastic genital. The boy died after 38 days because of a haemorrhage of the lungs. An intrauterine immobilisation is discussed as cause of the syndrome.

Dynamic KINE-MRI in patellofemoral instability in adolescents.

PURPOSE: The impact of kinematic MRI (KINE-MRI) in the patellofemoral instability and anterior knee pain of the adolescents is rarely reported. Our special interest was to evaluate the patellofemoral joint biomechanics with KINE-MRI in adolescents with affected and unaffected knees in a case-control study. METHODS: KINE-MRI was performed in 29 adolescents (affected knee group, n = 29 and unaffected knee group, n = 26) aged 11-16 years with unilateral patellofemoral instability. For the control group, we enrolled ten healthy age- and sex-matched volunteers (healthy knee group, n = 19). The stu

Voluntary femoro-tibial subluxations: a benign differential diagnosis in the snapping knee of a child.

PURPOSE: Voluntary femoro-tibial subluxation is a rare entity predominantly found in pretoddlers. It presents as a dynamic phenomenon with uni- or bilateral audible snapping of the knee, often in a context of fatigue or irritation at the end of the day. The aim of the study was to observe the evolution and recovery in these patients. METHODS: Ten children were included. Other causes of dislocating joints and pathologies with snapping of the lateral meniscus were excluded from this study. Six-week immobilisation with a splint at 70° of knee flexion was primarily recommended to all patients. R

Surgical versus Nonsurgical Treatments of Acute Primary Patellar Dislocation with Special Emphasis on the MPFL Injury Patterns.

The objective of the present study was to compare the clinical results of the surgical versus nonsurgical treatments of acute primary patellar dislocations with a medial patellofemoral ligament (MPFL) injury in the nonoverlap region with the vastus medialis oblique. In this study, 62 patients with an acute patellar dislocation and a concurrent MPFL injury in the nonoverlap region were randomly divided into two groups based on their birth years (even vs. odd years) and received either surgical or nonsurgical treatment. Of the 62 eligible patients, 30 were in the nonsurgical treatment group (Gro

Current concepts review: Fractures of the patella.

Fractures of the patella account for about 1% of all skeletal injuries and can lead to profound impairment due to its crucial function in the extensor mechanism of the knee. Diagnosis is based on the injury mechanism, physical examination and radiological findings. While the clinical diagnosis is often distinct, there are numerous treatment options available. The type of treatment as well as the optimum timing of surgical intervention depends on the underlying fracture type, the associated soft tissue damage, patient factors (i.e. age, bone quality, activity level and compliance) and the stabi
