[Clinical applications of the anterolateral skin flap and the fascial flap in the lower leg].

OBJECTIVE: To introduce the applied anatomy, operative method and clinical application of the anterolateral skin flap and fascial flap of the lower leg. METHODS: Anatomic dissection was performed on 14 adult cadavers' legs and one amputated lower limb. The origin, course and distribution of cutaneous branches of the superficial peroneal vessels were traced. Four types of anterolateral flaps were designed in the lower leg including the island skin flap, the island fascial flap, the rectangular flap, and the cross-leg flap. Since 1988, twenty-six cases of leg defects(21 patients), chronic osteo

Soft tissue reconstruction in the leg by orthopedic surgeons: Practices at an African trauma center.

INTRODUCTION: Soft-tissue coverage is a crucial step when treating open tibial fractures, which is often performed by orthopedic surgeons in a low resource setting. The objective of this study was to analyze the use of non-microsurgical flaps to treat such injuries in a West African trauma center. HYPOTHESIS: Pedicled flaps are reliable procedures that can be used by any orthopedic surgeon for soft-tissue coverage in the leg. METHODS: A retrospective study was conducted including patients treated between 2007 and 2011 for open tibial fractures or posttraumatic sequelae requiring flap coverag

Are oblique views needed for trauma radiography of the distal extremities?

OBJECTIVE: To our knowledge, few studies exist on the importance of the oblique view when radiography of the distal extremities is performed after acute trauma. Our prospective study aimed to determine whether the oblique view uniquely revealed abnormalities or clarified findings when it was obtained along with routine frontal and lateral radiographs. SUBJECTS AND METHODS: We prospectively interpreted 1461 consecutive radiographic examinations of the distal extremities in patients presenting with acute trauma to four family medicine clinics. The anatomic sites radiographed included the ankle,

Revascularization of the limbs using vein grafts after vascular injuries.

We report on 32 patients with vascular injury of a limb undergoing a total of 41 revascularization procedures with interposition vein grafts. A combined arterial and venous injury was present in nine cases, an isolated venous injury in four, and an isolated arterial injury in 19 cases. Eighteen per cent of patients with arterial injuries had normal distal pulses on initial examination. Preoperative arteriography was performed in 12 cases, and intraoperative arteriography in four. All venous injuries were diagnosed at operation. In most cases, the contralateral greater saphenous vein was used f

[Restorative microsurgery in severe injury and traumatic amputation of the distal portions of the extremities].

Application of microsurgical methods and free grafting of the compound complexes of tissues with the vessels and nerves in particular with microanastomoses permitted to achieve high functional results in traumatic amputation of extremities in contiguity, severe destroying traumas and their consequences. Experience of 1000 urgent microsurgical operations, including 638 replantations of digits and their blocks for various kinds of amputation on the palm level was summarized. Classification of amputation, taking into account the injury mechanism and character, promoting the choice of optimal tact

Lower extremity replantation.

Replantation of the traumatically amputated lower extremity is a technically feasible surgical undertaking. Successful outcome must be judged by functional achievements of the patient toward returning to the preinjury level. Appropriate patient selection significantly increases the potential for obtaining a satisfactory outcome. Patients in whom peripheral nerve injury precludes return of sensation in the extremity or in whom severe joint destruction will yield an immobile extremity will have marginal results. The risks of replantation, including blood transfusions, sepsis, and prolonged hospi

[Macroscopic and microscopic appearance of arterial stumps in limb segment amputation].

Until now, it has been assumed that in arterial lesions invagination of the intimal layer occurs. In a thigh and an upper-arm amputation this assumption was scrutinized. No hemorrhage or hypovolemic shock occurred in either case despite total amputation. The brachial and femoral arteries were closed and pulsed visibly within the soft tissue. Histology showed no invagination: the adventitial layer was drawn over the open lumen, which was filled with an extensive thrombus. Therefore, in these cases occlusion of the artery is presumed to occur by means of a fingertrap mechanism of the adventitia

Osseous Stress Injuries: Treatment Algorithms and Return to Play.

Osseous stress injuries are common in athletes. Specifically, lower extremity injuries are prevalent in running athletes and upper extremity injuries are prevalent in throwing athletes. Such injuries are suspected when there is focal bone tenderness and increased pain with the inciting activity. In elite athletes, osseous stress injuries are a relatively common culprit in lost play time. Thus rapid diagnosis and treatment is imperative to expedite return to play (RTP). The radiologist's role in these cases is not only for diagnosis, but also to grade the injury, which has implications in deter

Is Workload Associated With Hamstring and Calf Strains in Professional Baseball Players? An Analysis of Days of Rest, Innings Fielded, and Plate Appearances.

BACKGROUND: The influence of player workload on hamstring and calf injuries in professional baseball players remains unknown. HYPOTHESIS: Increased workload would significantly increase hamstring and calf strain injury risk. STUDY DESIGN: Case-control. LEVEL OF EVIDENCE: Level 3. METHODS: All professional baseball players who sustained a hamstring or calf injury between 2011 and 2017 were identified using the Major League Baseball (MLB) Health and Injury Tracking System (HITS). A separate player usage dataset was used to determine workload. We then compared these variables between player g

Navigating the complexity of calf injuries in athletes: a review of MRI findings.

The calf muscle group is a common area for injury within the professional athlete population. Anatomical and biomechanical differences between the different component muscles vary their individual predispositions to and patterns of injury. However, there is a common unifying factor: injuries involving tendinous components have greater clinical implications with regards to rehabilitation, potential intervention, length of time to return to play, and re-injury rates. As such, accurate understanding of the underlying anatomy and subsequent interpretation of the injury patterns carry significant c

Implementing Strength Training Strategies for Injury Prevention in Soccer: Scientific Rationale and Methodological Recommendations.

Due to the negative effects that injuries have on performance, club finances, and long-term player health (permanent disability after a severe injury), prevention strategies are an essential part of both sports medicine and performance. Purpose: To summarize the current evidence regarding strength training for injury prevention in soccer and to inform its evidence-based implementation in research and applied settings. Conclusions: The contemporary literature suggests that strength training, proposed as traditional resistance, eccentric, and flywheel training, may be a valid method to reduce in

Management of Sciatic Nerve Defects: Lessons Learned and Proposal for a New Strategy.

Management of sciatic nerve injuries can be difficult for surgeons without a special interest in nerve surgery as they would only treat a handful of such cases for many years. Sciatic nerve defects pose the greatest repair challenges, with nerve grafting producing mixed results because of the large size of the nerve in both diameter and length. This article first presents the peculiarities of sciatic nerve defects management, based on the authors experience and a literature review. Various issues are dealt with: When to operate depending on the injury mechanism? What are the results of nerve a

Worldwide Case Reports Using the iTClamp for External Hemorrhage Control.

BACKGROUND: Historically, hemorrhage control strategies consisted of manual pressure, pressure dressings, gauze with or without hemostatic ingredients for wound packing, or the use of tourniquets. The iTClamp is a relatively new alternative to stop external bleeding. METHODS: An anonymous survey was used to evaluate the outcomes of the iTClamp in worldwide cases of external bleeding. RESULTS: A total of 245 evaluable applications were reported. The iTClamp stopped the bleeding in 81% (n = 198) of the cases. Inadequate bleeding control was documented in 8% (n = 20) and in the remaining 11% (n
