INTRODUCTION: Multimorbidity or multiple long-term conditions (MLTCs) are defined as the coexistence of two or more chronic conditions in an individual. With increased longevity and the rising burden of chronic non-communicable diseases (NCDs), multimorbidity is becoming the norm. Although more prevalent in older populations and people with low socio-economic status, multimorbidity is rapidly rising in the younger age groups. Accurate data on its incidence and health and economic impacts, ie, disability-adjusted life years (DALY) lost and quality-adjusted life year (QALY) are not available for

BACKGROUND: Some individuals living with obesity may be relatively metabolically healthy, whilst others suffer from multiple conditions that may be linked to adverse metabolic effects or other factors. The extent to which the adverse metabolic component of obesity contributes to disease compared to the non-metabolic components is often uncertain. We aimed to use Mendelian randomisation (MR) and specific genetic variants to separately test the causal roles of higher adiposity with and without its adverse metabolic effects on diseases.
METHODS: We selected 37 chronic diseases associated with ob

Psoriasis is a chronic, immune-mediated inflammatory skin condition with variable clinical courses and therapeutic responses. We present the case of a 55-year-old female with chronic plaque psoriasis refractory to multiple biologic agents, including conventional systemic immunosuppressants, Janus kinase inhibitors, and interleukin inhibitors. Despite sequential biologic regimens, the patient demonstrated only partial and transient improvement. Given the persistence of severe disease, combination therapy with secukinumab, an IL-17A inhibitor, and apremilast, a phosphodiesterase-4 inhibitor, was

Inflammatory bowel disease (IBD) is a chronic condition, characterized by recurrent inflammation of the gastrointestinal tract. While the primary focus of IBD management consists in controlling gastrointestinal symptoms, IBD also presents an increased risk of thromboembolic complications. Tumor necrosis factor alpha (TNF-α) inhibitors, namely infliximab, have become increasingly common in the management of multiple chronic inflammatory diseases such as IBD. However, recent studies have shown an association between treatment with infliximab and an increased thrombotic risk. We report an unusual

Systemic Autoimmune Rheumatic Diseases, including Rheumatoid Arthritis, Systemic Lupus Erythematosus and Sjogren's syndrome, are characterised by a loss of immune tolerance and chronic inflammation. There is marked heterogeneity in clinical and molecular phenotypes in each condition, and the aetiology of these is unclear. NF-κB is an inducible transcription factor that is critical in the physiological inflammatory response, and which has been implicated in chronic inflammation. Genome-wide association studies have linked risk alleles related to the NF-κB pathway to the pathogenesis of multiple

Splenic infarct is a rare condition often related to haematological, cardiovascular, autoimmune or infectious diseases diagnosed with increasing frequency due to the large use of abdominal imaging techniques. Cryoglobulins are serum proteins that reversibly precipitate at low temperatures, small vessels vasculitis represent their most common clinical manifestation and are often associated with chronic liver disorders or with lymphoproliferative diseases. Here the authors report the cases of two patients, both affected by chronic liver disease associated with cryoglobulinemia, admitted to our u

BACKGROUND AND OBJECTIVE: Avatrombopag is a thrombopoietin receptor agonist that has demonstrated clinical efficacy in rapidly increasing platelet counts and achieving bleeding control in children with chronic immune thrombocytopenia. Early prediction of treatment response is critical for reducing unnecessary drug exposure and guiding personalized treatment strategies. However, accurate prediction remains challenging due to the inherent characteristics of clinical tabular data, including missing values caused by inconsistent data recording, small sample size, and high-dimensional clinical feat

INTRODUCTION: Inflammatory bowel disease (IBD) is associated with Takayasu arteritis (TAK). Chronic recurrent multifocal osteomyelitis (CRMO) is a nonbacterial osteomyelitis that occurs at multiple sites, some of which are associated with IBD. We herein report a pediatric patient diagnosed with CRMO and TAK 13 years after the onset of very early-onset ulcerative colitis (VEO-UC). Concurrent presentations of UC, TAK, and CRMO are extremely rare.
CASE PRESENTATION: A 15-year-old female diagnosed with VEO-UC at 22 months developed CRMO and TAK 13 years later despite maintained remission on infli

Chronic health conditions (CHCs) are common and associated with functional limitations. Acceptance and commitment therapy (ACT) shows promise in improving functioning, quality of life, and distress across several CHCs. The purpose of this study was to conduct a systematic review of technology-supported ACT for CHCs and perform a meta-analysis on functioning and ACT process outcomes. Multiple databases were systematically searched for randomized controlled trials. A total of 20 unique studies with 2,430 randomized participants were included. CHCs addressed in these studies were chronic pain (k

Recurrent respiratory tract infections are one of the most frequent reasons for pediatric visits and hospitalization. Causes of this pathology are multiple ranging from congenital to acquired and local to general. Immune deficiencies are considered as underlying conditions predisposing to this pathology. Our work is about to determine when and how to explore the immune system when facing recurrent respiratory infections. This was based on the records of 53 children hospitalized at the pediatrics unit of Hassan II University Hospital, Fez Morocco. Thirty boys and 23 girls with age ranging from

Chronic pulmonary aspergillosis is a lung disorder characterized by the presence of single or multiple cavities with or without an aspergilloma or nodules on chest imaging, with mycological evidence of and/or demonstration of immunological response to Aspergillus spp. The affected patient should manifest relevant symptoms for at least 3 months. Chronic cavitary pulmonary aspergillosis is the most common subset of chronic pulmonary aspergillosis, which is often reported in patients previously treated for pulmonary tuberculosis, having residual cavities in their lungs. We present a case of a 55-

The classical definition of Chronic Obstructive Pulmonary Disease (COPD) as a lung condition characterized by irreversible airway obstruction is outdated. The systemic involvement in patients with COPD, as well as the interactions between COPD and its comorbidities, justify the description of chronic systemic inflammatory syndrome. The pathogenesis of COPD is closely linked with aging, as well as with cardiovascular, endocrine, musculoskeletal, renal, and gastrointestinal pathologies, decreasing the quality of life of patients with COPD and, furthermore, complicating the management of the dise

Sarcoidosis is a chronic inflammatory condition of uncertain origins, affecting multiple organs and characterized by the formation of granulomas. Cardiac involvement, known as cardiac sarcoidosis (CS), occurs in 5%-10% of cases and can lead to heart failure, arrhythmias, and sudden death. Distinguishing CS from other heart conditions poses a significant challenge. However, improved diagnostic techniques such as cardiac magnetic resonance (CMR) and positron emission tomography combined with computed tomography (CT) have enhanced recognition rates, replacing invasive procedures like endomyocardi

Systemic lupus erythematosus (SLE) is a chronic multisystem inflammatory disease with a wide spectrum of clinical manifestations. Its association with autoimmune hepatitis is well documented; however, there is limited evidence linking it to hepatic overlap syndromes involving primary sclerosing cholangitis. Although cytopenias are a common hematologic manifestation of SLE, pancytopenia is less frequent and is associated with increased morbidity. Lupus enteritis is an uncommon gastrointestinal manifestation, typically presenting with nonspecific symptoms, which necessitates a high index of clin

Fatigue is the most common and debilitating symptom in multiple sclerosis (MS) and is believed to be distinctly different from fatigue seen in other chronic conditions. It can affect a patient's mood, sleep and have a detrimental effect on their quality of life. In the recent years much literature has emerged in an attempt to elucidate the potential causes and treatment of this common symptom. This review article aims to examine the most recent theories on the pathophysiology of fatigue in MS as well as its association with sleep and depression. We describe the pharmacological and non-pharmaco

The purpose of this report is to describe the epidemiological and pathological features of humeral osteomyelitis based on four pediatric cases observed during the subacute and chronic stage over a 12-month period. All locations were encountered. The offending bacterium was always Staphylococcus aureus. All four patients presented involvement of both the shoulder and elbow. Spreading of infection was largely due to the adverse effect of traditional massage therapy performed on these young patients. The poor living conditions of these children accounted for the existence of multiple foci of infe

Some results of multiple epidemiological, clinical, electroneurophysiological, and psychological studies that reveal different types of neurotoxicoses are presented in this work. Under conditions of present-day industry, when a worker is exposed to concentrations close to but below the maximum allowable concentrations, or MAC values, of toxic substances, acute neurotoxicoses rarely develop, occurring perhaps only in catastrophies. Acute intoxications may be accompanied by mass hysteria, which impedes etiological diagnosis and prognosis. Vegetative polyneuropathy is the most common type of chro

In a Zurich autopsy study comprising 7947 adults (over 20-year-olds), cor pulmonale was diagnosed in 8.9% (709 cases). In more than half of the cases the cor pulmonale was caused by lung diseases associated with chronic bronchitis and emphysema or with fibrosis of the lung. In 7 cases recurrent thromboembolism was the sole cause of cor pulmonale, whereas in 103 cases additional lung diseases were involved. 7 cases could be attributed to primary pulmonary hypertension. Cor pulmonale is, as a rule, the result of multiple lung conditions. The lesions of pulmonary vessels in cor pulmonale are prod

OBJECTIVES: Adverse childhood experiences (ACEs) in youth have been found to be frequently associated with several chronic illnesses, including chronic pain. However, this relationship remains poorly understood. Previous research has examined the association between ACEs and chronic pain, but these have primarily been examined in the context of adults with limited assessment of ACEs and individual pain conditions. The primary objectives of this study were to examine the incidence and psychological/pain-related correlates of ACEs in a sample of youth with chronic pain. Exploratory analyses were

CD8+ macrophages appear in the central nervous system (CNS) under various pathological conditions such as trauma and ischemia. Furthermore, macrophages expressing CD8 were found in CNS lesions of chronic, but not acute, experimental autoimmune encephalomyelitis (EAE). To further characterize cells with this phenotype, we examined CD8+ macrophages/monocytes in the CNS and peripheral organs during the course of acute and chronic EAE that had been induced by immunization of rats with myelin basic protein and myelin oligodendrocyte glycoprotein, respectively. Counting CD8+ macrophages in CNS lesio
