Roc-based validation of palmar sensory latency in Carpal tunnel syndrome diagnosis.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy, and electrophysiological assessment remains the diagnostic gold standard. OBJECTIVE: This study aimed to determine the diagnostic performance of palmar sensory latency (PSL) in CTS using receiver operating characteristic (ROC) analysis and to examine its association with clinical severity. METHODS: A total of 136 upper limbs from 98 individuals evaluated for suspected CTS were retrospectively analyzed. Standard motor and sensory nerve conduction studies of the median nerve were performed, and PSL was obtained

Demyelinating Leprosy Neuropathy: An Unusual and Misleading Electrophysiological Pattern.

INTRODUCTION/AIMS: Peripheral neuropathy, especially mononeuropathy multiplex, is a frequent manifestation of leprosy. Electrodiagnostic studies (EDX) usually show predominant axonal involvement. In this study, we report patients with prominent demyelinating abnormalities consistent with the diagnosis of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). METHODS: We retrospectively reviewed the medical records of patients with leprosy who underwent EDX at Pitié-Salpêtrière University Hospital, Paris, France, between January 2014 and June 2022. We included patients exhibiting de

Neuroprotective effect of low-intensity ultrasound against diabetic neuropathy.

AIM: To investigate the possible neuroprotective effect of low-intensity ultrasound (US) application against diabetic neuropathy. METHODS: Adult male wistar rats were randomly divided into three groups as control, untreated diabetic and US-treated diabetic. Diabetes was induced by a single intraperitoneal injection of streptozotocin (50 mg/kg) to the rats and verified by the presence of hyperglycemia (> 250 mg/dl). US was applied to sciatic nerve at a power density of 1.5 W/cm2 and a frequency of 1 MHz for 3 min/day for 5 weeks over a 5 cm2 skin surface. Blood glucose, insulin and HbA1c levels

Influence of Preoperative Nerve Conduction Studies on the Outcome of Carpal Tunnel Release Surgery.

Nerve conduction study (NCS) is the most common investigative tool used in diagnosing and determining the severity of carpal tunnel syndrome (CTS). We aimed to evaluate the relationship between preoperative NCS and the outcome of patients who underwent carpal tunnel release (CTR). Medical records of patients who underwent CTR from January 2018 to December 2023 were reviewed retrospectively. Demographic, clinical characteristics and NCS reports were extracted. CTS severity was graded according to the Canterbury criteria. The outcome was defined as improved or not improved based on subjective sy

Effect of amiodarone on retrograde conduction and refractoriness of the His-Purkinje system in man.

The effects of long term treatment with oral amiodarone on retrograde conduction ( S2H2 interval) and refractoriness of the His-Purkinje system were studied in 11 patients using His bundle electrograms and the ventricular extrastimulus method. Ten patients had ventricular tachycardia and one supraventricular tachycardia. Electrophysiological studies were carried out before and after the patients had been taking their maintenance dose for a mean duration of 84 days. After amiodarone treatment the HV interval was prolonged in seven patients and unchanged in four. At comparable S1S2 intervals, th

Cardiac features of an unusual X-linked humeroperoneal neuromuscular disease.

To characterize an unusual, sex-linked recessive neuromuscular disease, we studied two families with 37 males who had involvement of distal leg and proximal arm muscle groups. Electromyography and muscle biopsy in five subjects showed features of both neuropathy and myopathy. Bradycardia and syncope in 15 involved subjects were associated with early death (before the age of 50 years). Electrocardiograms in 15 others showed a spectrum of atrial abnormalities that ranged from abnormal P waves to permanent atrial paralysis and from first-degree atrioventricular block to complete heart block. No p

Small-fibre neuropathy in coeliac disease: a cross-sectional study.

INTRODUCTION: Coeliac disease (CD) is associated with neurological manifestations, including peripheral neuropathy. While sensorimotor neuropathy has been described, objective evidence for small-fibre neuropathy (SFN) in CD remains limited. We aimed to assess large- and small-fibre involvement in patients with CD with and without neuropathic symptoms using contemporary diagnostic standards. METHODS: In this cross-sectional study, 36 adults with biopsy-proven CD were recruited nationwide and stratified by the Neuropathy Symptom Score into symptomatic (pwCD(NS+), NSS ≥ 3) and asymptomatic (pwCD

Electrodiagnostic Studies as a Diagnostic and Prognostic Tool in Acute Flaccid Myelitis.

INTRODUCTION/AIMS: Acute flaccid myelitis (AFM) is a poliomyelitis-like syndrome, characterized by severe flaccid weakness. Nerve conduction studies (NCS) and electromyography (EMG) may have a role in the diagnosis and prognostication of AFM, but have not been well studied. We aim to describe in detail electrodiagnostic studies in AFM and relate findings to outcome. METHODS: In this retrospective multi-center case series, we analyze electrodiagnostic data from patients with a probable or definite diagnosis of AFM. Patients were enrolled at 11 sites from four countries. A detailed description

Hearing aid performance relative to site of lesion.

This study investigated hearing aid acceptance and use as functions of the probable site of a lesion: conductive, cochlear, neural, and central as determined by an audiometric test battery. Results indicate significantly better acceptance of aids in conductive and cochlear disorders than in more central neural and central disorders.

Abnormalities of sensorimotor integration in focal dystonia: a transcranial magnetic stimulation study.

It has been postulated that sensorimotor integration is abnormal in dystonia. We investigated changes in motor cortical excitability induced by peripheral stimulation in patients with focal hand dystonia (12 patients with hand cramps) and with cervical dystonia (nine with spasmodic torticollis) compared with 16 age-matched normal controls. Motor evoked potentials (MEP) to focal (figure-of-eight coil) transcranial magnetic stimulation of the hand area were recorded from the right abductor pollicis brevis (APB), first dorsal interosseus (FDI), flexor carpi radialis and extensor carpi radialis mu

Auditory brainstem response (ABR) to tone pips in hearing-impaired children.

The correspondence between ABR thresholds to tone pips and behavioral thresholds at 500, 1000 and 2000 Hz was studied in 13 normal and 55 hearing-impaired children, including 42 sensori-neural losses and 13 conductive losses. The children's ages ranged from 1 year 3 months to 8 years. Tone pips were provided with 2-cycle rise-fall times and 2-cycle plateaus. The correlations between the two measures were highly significant at 3 frequencies both in sensori-neural losses and conductive losses. In the subjects with sensori-neural losses, the ABR thresholds tended to be lower than the behavioral t

Altered cortical inhibitory function in children with spastic diplegia: a TMS study.

Periventricular leukomalacia (PVL) is the most frequent cause of spastic diplegia. The movement disorder is attributed to damage to the corticospinal tract, but there is increasing evidence of additional cortical dysfunction associated with PVL. Aim of the present study was to evaluate the integrity of the corticospinal tract and cortical inhibitory function using transcranial magnetic stimulation. Fifteen children with bilateral PVL and spastic diplegia and twenty-two healthy children underwent single-pulse stimulations to the right tibial anterior muscle. We compared central motor conduction

Vecuronium directly inhibits hypoxic neurotransmission of the rat carotid body.

UNLABELLED: Previous studies have suggested that partial neuromuscular blockade by vecuronium may inhibit the chemoreceptor neural response to hypoxia. Because acetylcholine and its receptors are critically involved in the hypoxic neurotransmission of the carotid body, we examined whether vecuronium interferes with nicotinic processes in the carotid body and inhibits the chemoreceptor neural response to hypoxia. The carotid body was harvested from anesthetized adult Wister rats. Carotid sinus nerve activity (CSNA) was recorded in vitro, whereas the carotid body was perfused with Krebs solution

Interactions between cutaneous and muscle afferent projections to cerebral cortex in man.

In order to demonstrate interactions between cutaneous and muscle afferent volleys in the ascending somatosensory pathways, different nerves of the lower limb were stimulated together in a conditioning-test paradigm, the changes in the earliest component of the cerebral potential evoked by the test stimulus being taken to indicate such an interaction. It was first confirmed that the cerebral potential evoked by stimulation of the posterior tibial nerve at the ankle is derived from muscle afferents in the mixed nerve and has shorter latencies than the cerebral potential evoked by purely cutaneo

Immature developmental pattern of the monosynaptic reflex in isolated spinal cord of glial mutant taiep rats.

There is increasing evidence suggesting that glial cells play a crucial role in the formation and maturation of neural circuits. However, little is known about the effects of glial alterations on the establishment of functional circuitry in vivo during the development. The taiep rat, a long-lived neurological mutant characterized by early astrogliosis and demyelination affecting selectively the CNS, provides an interesting model to study the glia-neuron interaction in situ. In the present study, we evaluated the functional development of segmental neural circuits recording the monosynaptic ref

Electrophysiological evidence for projections of myelinated and non-myelinated primary vagal afferents to the rat insular cortex.

Two types of vagal evoked potentials were recorded in chloralose-anesthetized rats: a positive-negative one and a negative-positive one in granular and agranular insular cortical subareas, respectively, the former being more susceptible to barbiturate. Both potentials consisted of two subtypes which were distinct as to threshold and latency. Peripheral conduction velocities as well as thresholds of these two subtypes corresponded to myelinated and non-myelinated primary afferents, respectively. Thus, these two primary vagal afferents are probably both represented in two insular cortical subare

"Backfiring" in spinal cord monitoring. High thoracic spinal cord stimulation evokes sciatic response by antidromic sensory pathway conduction, not motor tract conduction.

Spinal cord stimulation has been advocated as an alternative to motor cortex stimulation for motor tract activation. To test this theory, evoked responses were recorded from lumbar spinal cord (L2; n = 14), spinal roots (L4-L7; n = 112), peripheral nerves (sciatics; n = 28), and hind limb muscles (n = 28) after epidural stimulation of the T1-T2 segment of the spinal cord in dogs (n = 12), cats (n = 2), and monkeys (n = 2). The spinal response evoked by spinal cord stimulation was resistant to a dorsal hemisectioning (depth, 7-8 mm) of the midthoracic spinal cord. A minimal attenuation of laten
