Stiff Person syndrome (SPS) is a rare autoimmune disorder of the central nervous system characterized by stiffness and spasms in the lumbar and proximal lower limb muscles. Nonmotor symptoms include phobias, anxiety, and depression. SPS exists on a spectrum ranging from a focal disease known as the stiff limb syndrome to progressive encephalomyelitis with rigidity and myoclonus. Collectively, these conditions may be referred to as stiff person spectrum disorders, as they share similar core clinical features and autoantibodies against several neuronal proteins, which are involved in modulating

Sundowning is an aggravation of the elderly's neuropsychiatric symptoms that happen when daylight disappears. Behavioral disorders occur during these hours and are a source of disturbance within the care teams. Caregivers may find themselves unable to do their jobs. The etiology remains unclear despite the pathophysiological hypotheses implying an alteration of the circadian rhythm by the reach of the suprachiasmatic nucleus and the decrease in the production rate of melatonin. There are some therapeutic solutions with pharmacological and non-pharmacological approaches, but the effectiveness h

BACKGROUND: Pheochromocytoma is a rare but severe disease of the adrenal glands. The aim of this study is to present and discuss recent developments in the diagnosis and treatment of pheochromocytoma.
MATERIAL AND METHODS: A narrative review article based on the most recent literature is presented.
RESULTS AND DISCUSSION: The proportion of pheochromocytomas as tumors of adrenal origin is about 5% of incidentally discovered adrenal tumors. The classical symptomatic triad of headaches, sweating, and palpitations occurs in only about 20% of patients, while almost all patients show at least 1 of

In this narrative, I step into the shoes of someone who has lived with gynaecomastia - male breasts. Using the imaginary character of Aarav, I reflect on the themes of the stigma associated with body image, the courage to face it, and the role human relationships can play in fostering self-acceptance.

Standard clinical protocols and the concept "one drug fits all" that are currently used to treat illness in many cases are not effective, and strikingly so in the treatment of cancer, where 75% of therapeutic schemes are ineffective. The concept of personalized medicine is that the treatment of the disease is designed on the basis of the individual needs of each patient and the factors that influence their response to different drugs. Individualization of patient care has the potential to generate novel effective therapies, limit the adverse drug effects, create optimal treatments for individu

Approximately 170 Finns drown annually. In drowning fluid gets into the victim's respiratory passages so that breathing of air is prevented. The victim may be sunken into the fluid completely or only partially. As a consequence of the sinking, the victim will usually first hold his/her breath, until a laryngeal spasm develops. At this stage, the victim usually swallows large amounts of water. This is followed by loss of consciousness, decreased heart rate and cardiac arrest resulting from hypoxia. The cornerstone of resuscitation of a drowned person is correction of hypoxia.

Anemia in older persons is commonly overlooked despite mounting evidence that low hemoglobin levels are a significant marker of physiologic decline. Using the World Health Organization definition of anemia (hemoglobin level less than 13 g per dL [130 g per L] in men and less than 12 g per dL [120 g per L] in women), more than 10 percent of persons older than 65 years are anemic. The prevalence increases with age, approaching 50 percent in chronically ill patients living in nursing homes. There is increasing evidence that even mild anemia is associated with increased morbidity and mortality. An

BACKGROUND: Development of neurofeedback was preceded by development of classical and quantitative EEG, construction of computer devices, programmes and individual medical protocols.
METHODS: The basic rule of this method is to achieve effective change of rhythm and power of brain bioelectric activity based on knowledge of normal activity and identification of abnormalities of present activity. Various types of disturbed activity occur in many syndromes and brain development disorders. Training process is characterized by repetitive local activation of chosen brain structures as well as by ge

Stiff person syndrome (SPS), stiff limb syndrome, jerking SPS and progressive encephalomyelitis with rigidity and myoclonus (PERM) are a family of rare, insidiously progressive diseases of the central nervous system. They all share the core clinical features of appendicular and axial rigidity caused by continuous involuntary motor unit activity, and superimposed stimulus-sensitive spasms. There is good evidence for a primary auto-immune aetiology. Anti-glutamic acid decarboxylase (anti-GAD) antibodies, specifically to the GAD65 isoform, are present in serum or cerebrospinal fluid of 60-80% of

BACKGROUND: Coccidioidomycosis is a fungal infection acquired via inhalation of airborne fungal arthrospores of Coccidioides species in regions of endemicity in the deserts of the southwestern United States and northern Mexico. In recent years, the incidence of coccidioidomycosis has increased in areas of endemicity, and previous studies have found the highest incidence of coccidioidal infection in Arizona among persons in older age groups.
METHODS: We conducted a retrospective review of data for all patients with coccidioidomycosis who were treated at our institution that compared clinical m

Tuberculosis is another of those diseases from which our attention has been diverted in recent decades, and which may now come back to plague us. I refer to the timely warning in Dr Frankel's report and Dr Reppun's editorial comment in the Hawaii Medical Journal of December 1991.
