Combined periodontal and restorative approach to the treatment of gingival recessions with noncarious cervical lesions: a case treated with acellular dermal matrix allograft and compomer restorations.

Treatment of gingival recessions has become one of the most challenging procedures in periodontal plastic surgery. Various surgical options with predictable outcomes are available, but in cases with cervical lesions or restorations, optimal functional and esthetic results may require the combination of periodontal and restorative procedures. In this case report, one patient treated with acellular dermal matrix allograft and a coronally positioned flap in combination with compomer cervical restorations is presented. Clinical parameters were recorded immediately prior to surgery and after 12 mon

Blepharoplasty in the Artificial Intelligence Era.

Blepharoplasty and related periorbital procedures sit at the interface of functional reconstruction, esthetic surgery, ophthalmology, facial plastic surgery, and oral and maxillofacial surgery. Because eyelid and periorbital changes can be captured in photographs, videos, and 3-dimensional imaging, artificial intelligence has growing relevance for objective measurement, screening, planning, outcome assessment, and patient education. Current evidence is promising but heterogeneous. Image-based computer vision and deep learning appear closest to clinical use, whereas large language and generativ

Reduction mammaplasty and resource allocation--are patients being treated fairly? An examination of the current New Zealand situation, and looking towards the future.

AIM: To review the access to publically-funded reduction mammaplasty for New Zealand (NZ) women. Additionally, to evaluate quality of life gains from reduction mammaplasty and other surgical treatments of chronic conditions. Ultimately to determine whether access to surgical treatment for this condition is equitable. METHOD: Four tertiary referral centres for Plastic Surgery in NZ completed a survey to characterise patient access. A literature search was done to investigate the global situation and obtain quality of life information following breast reduction and other operations for chronic

Self-Administered Dietary Supplements, Herbal Products, and Ergogenic Aids Before Plastic Surgery: A Narrative Review and the PLASTIC-S Perioperative Management Framework.

The autonomous consumption of dietary supplements, high-dose vitamins, herbal products, ergogenic aids, and "natural" weight-loss preparations has increased substantially among candidates for plastic surgery. In Brazil, this pattern is amplified by a large market of compounded (manipulado) formulas that may contain undeclared active ingredients, by gym-oriented prohormones and selective androgen receptor modulators, and by the expanding access to cannabidiol products since the regulatory changes introduced by the Brazilian Health Regulatory Agency (ANVISA) in 2019 and 2022. These substances ar

High Prevalence of Body Dysmorphic Disorder and Moderate to Severe Appearance-Related Obsessive-Compulsive Symptoms Among Rhinoplasty Candidates.

BACKGROUND: Rhinoplasty is one of the most sought-after procedures in plastic surgery by individuals with body dysmorphic disorder (BDD). The identification of BDD symptoms is a challenge for plastic surgeons. The purpose of this study was to use a specific instrument for detection of BDD symptoms as a screening tool in rhinoplasty candidates and estimate the prevalence and severity of BDD symptoms in this population. METHODS: Eighty patients of both sexes seeking rhinoplasty were consecutively recruited at a plastic surgery outpatient clinic of a university hospital from February 2014 to Mar

The wider expansion of pediatric robotic urology: exploring the patterns of adoption in complex bladder reconstruction.

Robotic surgery is increasingly used in pediatric urology, particularly for moderate-complexity procedures such as pyeloplasty and ureteral reimplantation. Whether this expansion has extended to high-complexity lower urinary tract (LUT) reconstruction remains unclear. To characterize global trends in robot-assisted appendicovesicostomy (APV), augmentation cystoplasty, and bladder neck reconstruction (BNR) between March 2016 and March 2025, and to evaluate changes in proportional robotic adoption. Diagnosis and management of epispadias.

Epispadias is a rare diagnosis and most commonly described as a part of the bladder exstrophy complex. Epispadias is characterized by failure of the urethral plate to tubularize on the dorsum with the defect ranging from a glandular to a penopubic location. In addition, male patients demonstrate a dorsal chordee whereas female patients exhibit a bifid clitoris. The goal of surgical correction is the placement of the meatus in its anatomical position and the creation of functional genitalia with good cosmetic outcomes. Two different major reconstruction principals, the modified Cantwell-Ransley

Critical-sized bone defects: essential concepts for the higher orthopaedic examination.

PURPOSE: To provide a practical review of the assessment and management of critical-sized bone defects (CSDs) for candidates preparing for higher orthopaedic examinations. METHODS: This narrative review considers definitions and classification, clinical assessment, acute management, reconstructive techniques and salvage options. RESULTS: A critical-sized bone defect (CSD) is unlikely to heal without intervention because of its size and location, although no numerical threshold defines criticality. Assessment should consider skeletal stability, infection, vascularity, soft tissues, host facto

Staged buccal mucosa graft urethroplasty in the treatment of male urethral war injuries.

Modern armed conflicts are characterized by the widespread use of high-energy weapons, resulting in an increased incidence of severe combined injuries, including those involving the genitourinary system. The involvement of the genitourinary system, especially the lower part of the genitourinary system in males, is common and clinically significant due to the complexity of its treatment and subsequent impact on patients` quality of life. These injuries may be classified according to different degrees of tissue destruction leading to different levels of treatment sophistication. Most of these un

Optimizing the Laparoscopic Vecchietti Procedure: Tips and Tricks.

BACKGROUND: The Vecchietti procedure is a staged procedure that, through use of an acrylic olive, allows for expedited traction and dilation to create a neovagina in patients with congenital vaginal agenesis. Although the steps are fairly standardized for the laparoscopic Vecchietti procedure, we have appreciated specific nuances in technique that have optimized our surgical approach and efficiency. CASE: We present a case of congenital vaginal agenesis in a genetically female patient who completed neovaginal creation using the laparoscopic Vecchietti technique. During this case, we review ea

Urethral Stricture Disease and Urethroplasty in Older Men.

Urethral stricture disease is scarring of the anterior urethra leading to urinary obstruction. The incidence of urethral stricture disease increases with age. Older men have an increased risk of coexisting medical conditions which may exacerbate urinary symptoms in the face of a urethral stricture. Initial management of urethral stricture disease uses minimally invasive methods. Urethral reconstruction should be reserved for patients with previous treatment failures and complex stricture disease. Urethral reconstruction can be safe and effective in older men, but fragility and comornidities sh

The Glanular-Frenular Collar (GFC) technique: Dual approach to hypospadias reconstruction.

INTRODUCTION: The Glanular-Frenular Collar (GFC) technique has evolved since it was first described in 2017. The technique involves reconstruction of the septum glandis, which has been overlooked in the history of hypospadias, and formation of the frenulum, creating the ventral wall of the glanular urethra without dissecting the glans, leaving room for the formation of the navicular fossa. MATERIAL AND METHODS: We present a case of distal hypospadias corrected with the GFC technique and demonstrate the important steps of the surgical technique. RESULTS: The aesthetic result corresponded to t

Allograft reconstruction of the medial patellofemoral complex in skeletally immature patients: a technical note.

Patellofemoral instability (PFI) is a common disorder in children and adolescents. Surgical stabilization of the patella poses challenges in skeletally immature patients due to anatomical peculiarities at this stage, leading to the absence of an established standard reconstruction technique. Recently, there has been a notable interest in the medial patellofemoral complex (MPFC), encompassing the medial patellofemoral ligament (MPFL) and the medial quadriceps tendon femoral ligament (MQTFL), owing to their role in restricting lateral patellar displacement. This paper aims to describe the surgic

Megameatus intact prepuce: a systematic review of surgical techniques and long-term outcomes.

Megameatus intact prepuce (MIP) presents with diverse phenotypes that complicate the management of this rare but complex hypospadias variant. Current data on optimal treatment methods and patient outcomes are sparse, unintegrated, and therefore challenging to implement clinically. A comprehensive systematic review of the existing literature on MIP was conducted according to PRISMA guidelines. Electronic databases including PubMed, Embase, and Scopus were searched for relevant articles published up to [2024]. Key aims were to assess the safety and efficacy of different surgical interventions, a
