Long-term mental sequelae of political imprisonment in East Germany.

This paper presents an assessment of the long-term mental sequelae of prolonged stress due to political persecution and imprisonment in the German Democratic Republic. Fifty-five former political prisoners with enduring psychiatric disorders were examined retrospectively in an exploratory study. The patients' experiences before, during, and after imprisonment were investigated using a semistructured interview. Psychopathological symptoms were assessed on clinical and self-rating scales; diagnostic classification was conducted according to DSM-III-R. The patients had experienced serious trauma,

Beyond aspiration: An expenditure-based evaluation of progress towards the Abuja Declaration target and implications for domestic health financing in Africa.

In 2001, African Union (AU) member states committed to allocating 15% of their annual budget to health under the Abuja Declaration (AD). However, after more than two decades, health allocations across many countries still fall short of the target. This study uses an expenditure-based proxy for the AD to examine the likelihood and timing of attainment of the benchmark under historical fiscal trajectories. We compiled a 2002-2023 panel for 54 African countries using WHO and World Bank data. We distinguished observed, first and sustained attainment. Kaplan-Meier and Weibull models characterised ti

Cost-Effectiveness of Thyroid Nodule Molecular Testing in Manitoba.

OBJECTIVE: To determine the cost-effectiveness of molecular testing for indeterminate (Bethesda III/IV) thyroid nodules in a real-world, single-payer healthcare system. DESIGN: This retrospective population-based cohort study analyzed fine-needle aspiration biopsy (FNAB) and lobectomy data for thyroid nodules in Manitoba, Canada, during 2023. Microcosting was performed to capture surgical, surveillance, molecular testing, and complication costs in 2023 Canadian dollars. Six molecular tests (Afirma, ThyroSeqV3, ThyroidPrint, ThyroSPEC, ThyGeNEXT/ThyraMIR, and mir-THYpe) were evaluated using si

Scale-up of the DMPA-SC in Nigeria: Why policy matters.

BACKGROUND: Injectable contraceptives have contributed substantially to Nigeria's rise in modern family planning methods usage. They are one of the most commonly used and preferred means of contraception among women in the country. Enabling policies are required to assure contraceptive access, security, and use. This study aimed to investigate the policy environment and how it supports or limits Nigeria's introduction and scale-up of subcutaneous depot-medroxyprogesterone acetate (DMPA-SC). METHODS: The design of this mixed-methods study was cross-sectional. Desk reviews of policy papers, key

A Cross-Sectional Survey on Professionals to Assess Health Needs of Newly Arrived Migrants in Spain.

Heightened conflicts and lack of safety due to reasons related to economic, social, ethnic, religious, sexual orientation, political, or nationality matters have increased migratory movements during the last, few decades. Unfortunately, when migrants arrive in new territories, they can face many barriers. For example, in Spain, some migrants have difficulties in accessing health services. Accessing primary care following the Affordable Care Act: a qualitative study of low-income women's experiences in urban California.

BACKGROUND: The 2010 Affordable Care Act (ACA) led to Medicaid expansion, which expanded eligibility to low-income individuals below 138% of the federal poverty level in 41 states and Washington, DC. In California, over one-third of state residents are covered by Medicaid (Medi-Cal) insurance. Despite the 2014 Medicaid expansion in California, many individuals remain uninsured. Low-income women, in particular, face significant primary care access challenges due to socioeconomic status, education, and minority/disability status. This qualitative study aimed to explore the experiences of low-inc

Increase in Poison Center Reports Linked to Kratom-Containing Kava Products - National Poison Data System, United States, 2000-2025.

Kava (Piper methysticum), a central nervous system depressant derived from a plant in the pepper family native to the Pacific Islands, is traditionally consumed in religious, cultural, political, and social ceremonies. In the United States, kava emerged in the late 1990s and has experienced renewed growth and product diversification since the 2010s, with increasing availability of concentrated extracts and ready-to-drink beverages. These commercial products are commonly marketed as healthy alternatives to alcohol, sold near college campuses, and increasingly being combined with kratom, a psych

This study examined the prevalence of psychological distress among HCWs in Kabul, Afghanistan, in the wake of the political changes in August 2021. METHODS: A cross-sectional survey study was conducted from September to October 2021 in 16 public tertiary hospitals in Kabul. The Afghan Symptoms Checklist (ASCL) was used to assess psychological distress and its related symptoms. Multivariable analy

[Impact of the COVID-19 pandemic on outpatient physical therapy in Germany].

INTRODUCTION: Due to the COVID-19 pandemic, the German health system has faced major challenges since spring 2020. In addition to restrictions in the inpatient health care sector, the maintenance of outpatient care by health care providers has also been affected by the pandemic-related restrictions. Both a decline in treatment frequencies and temporary practice closures have been observed. The aim of the study was to survey the work and care situation in outpatient physiotherapy practices during the first wave of the pandemic. METHODS: An anonymous online survey including quantitative and qua

A Qualitative Insight into Barriers to Tuberculosis Case Detection in East Gojjam Zone, Ethiopia.

Tuberculosis (TB) remains to be the leading cause of morbidity and mortality in the developing world. Early TB case detection (TCD) and treatment of infectious cases is vital to reduce the TB burden. The objective of this study was to identify possible barriers to TCD in East Gojjam Zone, northwest Ethiopia. The study used a descriptive phenomenological research method. The study participants included 21 TB patients, six TB control officers, and 40 health workers (HWs) selected by a heterogeneous purposive sampling technique. In-depth interviews and focus group discussions were used to collect

Preparing for disaster: response matrices in the USA and UK.

Disasters, whether man-made or naturally occurring, require complex responses across multiple government agencies and private sector elements, including the media. These factors mandate that, for effective disaster management and because of the unpredictability of such events, response structures must be in place in advance, ready to be activated on short notice, with lines of responsibility clearly delineated and mechanisms for coordination of efforts already established. Disaster response experiences in the USA and the UK were reviewed at a conference convened by the New York Academy of Medi

Role of professional associations in the drive for policy change and uptake of new WHO recommendations for the prevention and treatment of postpartum hemorrhage.

The role of professional associations in the health ecosystem is crucial as they establish and uphold standards of professional practice, ethics, and continuing education, thereby safeguarding patient welfare and promoting accountability among practitioners. Their role is particularly important in combating postpartum hemorrhage (PPH), as outlined in the WHO PPH Roadmap's four strategic pillars: research, norms and standards, implementation, and advocacy. Professional societies, such as obstetrics/gynecology and midwifery associations and other collaborating cadres, such as anesthetists and ph

Prevention of postpartum haemorrhage: from evidence to implementation at scale.

Postpartum haemorrhage (PPH) is a leading cause of maternal death. Preventing PPH can spare women from experiencing the trauma and risks of PPH, reduce the strain on overstretched health systems, and probably produce better outcomes than a strategy solely focused on PPH treatment. Prevention of PPH is often interpreted as provision of uterotonic drugs to contract the uterus at the time of childbirth. Although uterotonics are a central strategy for PPH prevention, several other approaches can prevent PPH or ameliorate its severity. These approaches include addressing the unmet need for contrace
