Validation of an Optimized Algorithm for Identifying Persons Living With Diagnosed HIV From New York State Medicaid Data, 2006-2014.

Algorithms are regularly used to identify persons living with diagnosed human immunodeficiency virus (HIV) (PLWDH) in Medicaid data. To our knowledge, there are no published reports of an HIV algorithm from Medicaid claims codes that have been compared with an HIV surveillance system to assess its sensitivity, specificity, positive predictive value, and negative predictive value in identifying PLWDH. Therefore, our aims in this study were to 1) develop an algorithm that could identify PLWDH in New York State Medicaid data from 2006-2014 and 2) validate this algorithm using the New York State H

Learning patient-specific spatial biomarker dynamics via operator learning for Alzheimer's disease progression.

Alzheimer's disease (AD) is a complex, multifactorial neurodegenerative disorder with substantial heterogeneity in progression and treatment response. Despite recent therapeutic advances, predictive models capable of accurately forecasting individualized future biomarker states remain limited. Here, we present a machine learning-based operator learning framework for personalized modeling of AD progression, integrating longitudinal multimodal imaging, biomarker, and clinical data. Unlike conventional models with prespecified dynamics, our approach directly learns patient-specific disease operat

A unified multi modal transformer framework for breast cancer recurrence prediction and survival analysis.

Breast cancer recurrence prediction is an important feature of post-treatment therapy, requiring accurate identification of both recurrence risk and time-to-event outcomes. In this paper, we offer a unified deep learning system that jointly performs survival analysis and multi-class recurrence classification, enabling full risk stratification for breast cancer patients. The proposed model includes a Transformer-based survival module to estimate time-until-recurrence, and an attention-guided classification module to differentiate between second primary cancer, low-risk, and high-risk recurrence

A multi-level machine learning pipeline for prediction and prioritization of hybrid insulin peptides in type 1 diabetes.

Hybrid insulin peptides (HIPs) are neoepitopes involved in type 1 diabetes (T1D), but their complete repertoire remains unknown. The vast combinatorial space makes experimental screening unfeasible and requires bioinformatics-based prioritization. We developed a multi-level machine learning pipeline for ranking HIP candidates. First, 36 physicochemical and junction-specific features were computed for a reference library of 240 HIPs with known enzyme-linked immunospot (ELISPOT) reactivity, and a baseline Ridge regression model was trained. Next, all possible HIP candidates with 7-9 amino acid r

Beyond Area Under the Receiver Operating Characteristic Curve: Evaluating Predictive Performance Metrics Under Class Imbalance in Real-World Clinical Data.

BACKGROUND: Predictive models increasingly support clinical decision-making, although imbalanced outcome distributions are common in health care datasets and can distort performance evaluation. The area under the receiver operating characteristic curve (AUROC) remains the most frequently reported metric, despite its limited ability to reflect clinically meaningful performance under class imbalance. OBJECTIVE: This study aimed to examine the influences of metric selection on the clinical interpretation of predictive models in imbalanced real-world health care data. METHODS: This was a retrosp

Transformer-based biomarker prediction from colorectal cancer histology: A large-scale multicentric study.

Deep learning (DL) can accelerate the prediction of prognostic biomarkers from routine pathology slides in colorectal cancer (CRC). However, current approaches rely on convolutional neural networks (CNNs) and have mostly been validated on small patient cohorts. Here, we develop a new transformer-based pipeline for end-to-end biomarker prediction from pathology slides by combining a pre-trained transformer encoder with a transformer network for patch aggregation. Our transformer-based approach substantially improves the performance, generalizability, data efficiency, and interpretability as com

Comparison of the diagnostic performance of machine learning algorithms for differentiating iron deficiency anemia and thalassemia.

Accurate discrimination between iron deficiency anemia (IDA) and thalassemia trait (TT) is clinically essential for the effective management of patients with hypochromic microcytic anemia. Although numerous discrimination indices based on red blood cell (RBC) parameters have been proposed, their diagnostic accuracy remains suboptimal and highly population-specific. This study aimed to develop and validate a machine learning model to enhance discriminative performance. We utilized a derivation cohort of 376 patients (IDA, n = 186; TT, n = 190) for model development and internal validation, and
