Left ventricular thrombus formation after repair of anomalous left coronary artery from the pulmonary artery.

Although thrombus formation following myocardial infarction in adults is well known, intracardiac thrombosis in children is uncommon. We report the case of a large left ventricular thrombus in an infant with ischemic cardiomyopathy secondary to anomalous origin of the left coronary artery from the pulmonary artery. Given its mobility and protrusion across the aortic valve, the patient underwent urgent thrombus removal through a transaortic approach. There were no embolic or neurologic complications. This case highlights that thrombectomy may be performed safely and successfully in critically i

Rare case of Bland White-Garland syndrome with myocardial infarction in paediatrics.

A 3.5 months old female infant presented with acute history of loose motions, fever and irritability for 5 days. The infant had history of respiratory distress since birth that exaggerated while feeding along with sweating and increased milk intake. Patient had poor weight gain. On investigations she was found to have anomalous origin of the left coronary artery from the left pulmonary artery (ALCAPA) with dilated cardiomyopathy and anterior wall myocardial infarction which is a very rare diagnosis in paediatrics and very rare condition diagnosed in our settings.

[Fabry disease].

Fabry disease is an X-linked inherited condition with the absence or reduction of alpha-galactosidase A- activity in lysosomes leading to accumulation of globotriaosylceramide and related neutral glycosphingolipids. Manifestations of Fabry disease include progressive renal and cardiac insufficiency, neuropathic pain, gastrointestinal symptoms, cerebral infarction and skin and pulmonary symptoms. First symptoms of Fabry disease usually appear in childhood. The symptoms in females may be as severe as in males. Early diagnosis of Fabry disease is important because enzyme replacement therapy can s

Severe diabetic ketoacidosis leading to cardiac failure, pulmonary oedema and spinal cord oedema resulting in tetraplegia.

A 23-year-old man with poorly controlled insulin-dependent diabetes mellitus presented to casualty with community-acquired pneumonia and diabetic ketoacidosis. Shortly after admission he deteriorated and developed cardiac failure, pulmonary oedema and further decreased level of consciousness. He was sedated and ventilated for 3 weeks in the intensive care unit. On waking from sedation he was found to be tetraplegic. MRI scan showed gross oedema of the cervical spinal cord with area suspicious of infarction. We describe a rare cause of spinal cord injury and discuss the proposed hypotheses.

[Cardiac arrest in a five month old boy with ALCAPA].

A five month old boy was admitted with symptoms of asthmatic bronchitis. Careful anamnesis revealed failure to thrive latest month, cough since age of three weeks and a few episodes of severe crying. He had cardiomegaly and an electrocardiogram showed Q waves and ST segment elevation in several leads. His condition suddenly deteriorated and he died of cardiac arrest. Autopsy revealed infarction and fibrosis of the myocardium and a left coronary artery originating from the pulmonary artery. Symptoms that earlier could have raised suspicion of cardiac disease are emphasized.

Role of echocardiography/Doppler in cardiogenic shock: silent mitral regurgitation.

Two cases of cardiogenic shock and pulmonary edema due to acute, severe, silent mitral regurgitation are discussed. The mechanism for the mitral regurgitation was papillary muscle rupture in the setting of acute myocardial infarction. Echocardiography established the presence, severity, and cause of the mitral regurgitation and the associated hyperdynamic left ventricular function in the setting of cardiogenic shock. Transesophageal echocardiography is excellent for assessing the mitral valve in critically ill patients in whom transthoracic echocardiography may be inadequate or misleading. Thi

[Electrocardiographic characteristics of an anomalous origin of the left coronary artery from the pulmonary trunk].

Electrocardiographic characteristics of abnormal left coronary arterial branching (ALCAB) from the pulmonary trunk were examined in 36 patients, aged 2 months to 39 years. Signs of marked left-ventricular and left-atrial overstrain were typical for all ages. There were signs of past left ventricular myocardial infarction in 88.8% of patients, the cicatricial changes being anterolateral in 65.6%, which is typical for the defect. The follow-up of ECG pattern over 1 month to 8 years after the defect was first detected demonstrated that coronary circulation deteriorated in 64.3% of cases.

Postinfarction ventricular septal rupture: the importance of location of infarction and right ventricular function in determining survival.

Over a 5.5 year period, 1264 consecutive patients with acute myocardial infarction as confirmed by enzyme levels were prospectively identified. Of these, 25 (2%) suffered ventricular septal rupture (pulmonary/systemic flow range 1.5 to 6) 7 +/- 7 days after onset of myocardial infarction. Death occurred in 14 patients (56%) and was more common after inferior than anterior myocardial infarction (11 of 15 [73%] vs three of 10 [30%], p less than .05). Among 133 variables analyzed, survivors and nonsurvivors were similar with respect to all premorbid clinical characteristics, infarct size as asses

Beta-Blockers after Myocardial Infarction and Preserved Ejection Fraction.

BACKGROUND: Most trials that have shown a benefit of beta-blocker treatment after myocardial infarction included patients with large myocardial infarctions and were conducted in an era before modern biomarker-based diagnosis of myocardial infarction and treatment with percutaneous coronary intervention, antithrombotic agents, high-intensity statins, and renin-angiotensin-aldosterone system antagonists. METHODS: In a parallel-group, open-label trial performed at 45 centers in Sweden, Estonia, and New Zealand, we randomly assigned patients with an acute myocardial infarction who had undergone c

Successful Endovascular Treatment for Middle Cerebral Artery Occlusion Caused by the Thrombus Formation in the Pulmonary Vein Stump Following Left Upper Lung Lobectomy.

Thrombus formation in the pulmonary vein (PV) stump after lung resection can cause rare cases of cerebral infarction. These infarctions can result in embolism and ischemia in the relatively large intracranial vessels, severely impacting the quality of life (QOL) of these patients. We performed endovascular thrombectomy successfully for this rare complication after lung lobectomy. A 73-year-old woman with paroxysmal atrial fibrillation (AF) suffered from sudden left complete hemiplegia 19 days after undergoing a left upper lung lobectomy (LUL). Magnetic resonance imaging (MRI) showed middle cer

Takayasu arteritis presenting with massive cerebral ischemic infarction in a 35-year-old woman: a case report.

INTRODUCTION: Takayasu arteritis is a relatively rare type of large-vessel arteritis that primarily affects the aorta and its major branches, the coronary arteries, and the pulmonary arteries. Depending on the different groups of blood vessels involved in the disease process, the clinical presentation of Takayasu arteritis varies. Here we report a case of a woman presenting with a debilitating massive cerebral ischemic infarct that turned out to be a relatively rare first presentation of Takayasu arteritis. CASE PRESENTATION: A 35-year-old Chinese woman presented to the Emergency Department w

Long-term outcomes of perioperative myocardial infarction/injury after non-cardiac surgery.

AIMS: Perioperative myocardial infarction/injury (PMI) following non-cardiac surgery is a frequent cardiac complication. Better understanding of the underlying aetiologies and outcomes is urgently needed. METHODS AND RESULTS: Aetiologies of PMIs detected within an active surveillance and response programme were centrally adjudicated by two independent physicians based on all information obtained during clinically indicated PMI work-up including cardiac imaging among consecutive high-risk patients undergoing major non-cardiac surgery in a prospective multicentre study. PMI aetiologies were hie

Survival of cardiorespiratory arrest after coronary artery bypass grafting or aortic valve surgery.

BACKGROUND: Study objectives were to (1) report the clinical profile of and outcome for patients who experience a cardiorespiratory arrest after coronary artery bypass grafting or aortic valve replacement, and (2) identify factors associated with improved probability of survival. METHODS: We identified 108 consecutive patients who had cardiorespiratory arrest after coronary artery bypass grafting or aortic valve replacement between April 1999 and June 2008. We studied the characteristics of arrests and survivors, and performed a multivariate logistic analysis to determine features associated

Recurrent Stroke as a Presenting Feature of Takayasu Arteritis in an Adolescent: A Case Report and Literature Review.

Takayasu arteritis is a large vessel vasculitis, characterized by granulomatous inflammation of arterial vessels, that typically affects the aorta, its main branches and pulmonary arteries. Disease diagnosis is a challenge and requires awareness of the condition, as clinical signs can be not specific. We report a case of an adolescent with recurrent stroke diagnosed with Takayasu arteritis. A diagnosis of Takayasu arteritis was established due to angiographic findings in the magnetic resonance angiography in conjunction with systolic blood pressure discrepancy, arterial hypertension and increa

Does vaginal estrogen use increase the risk for adverse cardiovascular outcomes?

NO. In general, nonoral estrogen use for menopausal symptoms is associated with a lower cardiovascular (CV) risk profile than oral estrogen use (strength of recommendation [SOR], B; meta-analysis of cohort studies). Vaginal estrogen use is associated with lower risk for coronary heart disease (CHD) and similar risk for myocardial infarction (MI), stroke, and deep vein thrombosis/pulmonary embolism (DVT/ PE) compared with nonuse (SOR, B; cohort studies). Vaginal estrogen therapy also is associated with lower CV-related mortality for 3 to 5 years compared withnonuse (SOR, B; cohort study). No hi
