Post-op lumbar subdural hygroma: a case report.

Lumbar spinal decompression surgery is commonly performed to relieve radicular symptoms to good effect. The formation of post-operative spinal subdural hygroma, or spinal subdural extra-arachnoid hygromas (SSEH) following incidental intra-operative durotomies, have been described in previous literatures. We report a case of a 63 years old lady who had initially underwent an uneventful posterior lumbar decompression for lower limb radiculopathy, without intraoperative durotomy, with development of concurrent SSEH and spinal subarachnoid hygroma (SSSH) during the early post-operative period with

Accuracy of dentists in the clinical diagnosis of oral lesions.

OBJECTIVE: Dentists play an important role in the early diagnosis and treatment of oral lesions. However, treatment based solely on a clinic … s (16%), focal inflammatory fibrous hyperplasia (10%), fibroma (8%), periapical granuloma (7%), and radicular cyst (6%). Cancerous lesions were misdiagnosed 5.6% of the time. CONCLUSIONS: The high rates of clinical misdiagnosis by dental practitioners indicate that all excised lesions should to be submitted for histologic diagnosis.

Monocyte chemotactic protein-3: possible involvement in apical periodontitis chemotaxis.

AIM: To study the expression of monocyte chemotactic protein-3 (MCP-3, also known as chemokine CCL-7) in tissue from apical lesions (AL) and to associate MCP-3 expression with symptomatic or asymptomatic apical periodontitis. METHODOLOGY: To determine the expression of MCP-3 in AL, biopsies obtained during tooth extraction procedures were fixed, subjected to routine processing and diagnosed as apical granuloma (AG) (n = 7) or radicular cyst (RC) (n = 5). As controls, apical periodontal ligament (PDL) specimens from healthy premolars extracted for orthodontics reasons were included (n = 7). Al…

Prognosis of large cyst-like periapical lesions following nonsurgical root canal treatment: a clinical review.

AIM: To evaluate clinically and radiographically the long-term clinical outcome of nonsurgical root canal treatment using calcium hydroxide in teeth with large cyst-like periapical lesions. SUMMARY: A total of 42 mature anterior teeth with large periapical lesions ranging in size from 7 to 18 mm in diameter were included in this study. Fluid samples from the lesions contained cholesterol crystals, which were identified by light microscopy. After adequate draining, the teeth were treated endodontically using calcium hydroxide as the intracanal medicament. All cases were followed up for a perio

Apicectomy of posterior teeth--a clinical study.

Studies evaluating periapical surgery of posterior teeth are relatively few. A retrospective study of 32 apicectomies on posterior teeth using both clinical and radiographic means yielded a success rate of 62 per cent. There were five cases (16 per cent) of failure, the majority of which presented as delayed infection between three and six months following surgery. Post-surgical review of at least six months is recommended. Anatomical considerations for posterior tooth apicectomy and the necessity for retrograde fillings are discussed.

Spontaneous bone healing of the large bone defects in the mandible.

Spontaneous healing of large bone defects in the mandibles of 33 patients was studied. Standard postoperative clinical and radiographic examinations were performed immediately after surgery and after 2, 6 and 12 months. They were analysed using a novel relative bone densitometry method and indexes of relative bone healing were established. Spontaneous bone regeneration occurred in all patients clinically, and the computer analysis of radiographs showed that the mean final bone density in the bone defects was 88% of the bone density of the surrounding healthy bone. In the cases of smaller defec

Noninvasive treatment choice for an aged down syndrome patient presenting a residual periapical cyst.

AIM: This is the first report to illustrate the marsupialization as an effective treatment for a Down Syndrome (DS) patient presenting a residual periapical cyst. BACKGROUND: These cysts occur within the alveolar ridge, usually at the local site of a previously extracted tooth that did not received proper curettage; usually the surgical excision of a cyst and also the vigorous curettage of a socket is very simple, if not for the fact that mentally disabled patients require rapid and non-stressful procedures. CASE DESCRIPTION: The 54-year-old DS patient represented herein received a minimally

Features of odontogenesis and expression of cytokeratins and tenascin-C in three cases of extraosseous and intraosseous calcifying odontogenic cyst.

To characterize further the nature of calcifying odontogenic cyst (COC), we studied histologically and immunohistochemically an extraosseous and two intraosseous lesions. The extraosseous COC was in continuity with the stratified squamous epithelium of the alveolar mucosa. Immunostaining with monoclonal antibodies showed reactivity of both low- and high-molecular-weight cytokeratins, the degree of coexpression decreasing with the increasing morphological diversity of the cyst/tumour epithelium. Staining for the matrix glycoprotein tenascin-C was seen not only in the connective tissue, where it

Odontogenic Sinusitis: The Selective Role of a Modified Caldwell-Luc Approach in Two CT/CBCT-Documented Cases.

Tooth-bearing structures are commonly involved in the spread of various odontogenic lesions. Radicular and dentigerous cysts are among the most common odontogenic cysts, exhibiting significant variations in size and morphology within the jawbones. On the other hand, the influence of dental caries, periodontal diseases, and periapical lesions extends beyond tooth-bearing bone and also affects adjacent anatomical structures, especially the maxillary sinuses. In addition to untreated odontogenic lesions, treatment-related complications in the posterior maxilla may also disturb the sinus environme

Utility of Chitra-HASi Granules in Cystic Defects of the Maxillofacial Region: A Pilot Study.

BACKGROUND: Cystic defects that are critical sized or larger require bone replacement strategies. However, due to inherent disadvantages of the various types of grafts, none of the available materials are best suited for these defects. Among the alloplastic materials, hydroxyapatite (HA)-based grafts are the most popular, due to their osteoconductive nature and resemblance to mineral bone. The aim of the study was to assess the utility of the novel material "Chitra-HASi" as a bone substitute in the maxillofacial region. MATERIALS AND METHODS: In a single-arm, prospective study, patients with

Correlation of radiographic size and the presence of radiopaque lamina with histological findings in 70 periapical lesions.

PURPOSE: This study was conducted to relate the histological diagnosis of chronic inflammatory periapical lesions with the radiographic images of these lesions. MATERIALS AND METHODS: A total of 70 biopsy specimens obtained during periapical surgery were stained with hematoxylin and eosin and examined under a microscope. Histological analysis established the diagnosis as granuloma, cyst, or scar tissue. The location of the lesion was recorded, as were the radiographic size (maximum and minimum diameter in mm and area in mm(2)) and the presence of radiopaque lamina around the lesion, using an

Actinomycosis is not Frequent in the Periapex But is a Persistent Lesion.

Periapical actinomycosis caused by a gram-positive anaerobic pathogen characterizes a typical extra-radicular infection. This study determined the frequency and correlated the content of bacteria colonies with the of periapical actinomycosis size. The study comprised a total of 218 periapical lesions (PL) (cysts, granulomas or abscess). The specimens embedded in paraffin were sliced into 4-µm sections and stained with hematoxylin-eosin, Gram, Periodic Acid-Schiff (PAS) and Grocott's stain. The presence of bacterial colonies composed of filamentous structures labeled with the histochemical stai

Histological and immunohistochemical analysis of human periapical lesions: a study of TGF-β1 and CD68 markers.

BACKGROUND: Various inflammatory and anti-inflammatory mediators, along with diverse cell types, are implicated in the development and progression of periapical lesions. This work aimed to assess the immuno-expression of transforming growth factor-beta 1 (TGF-β1) and CD68 (a macrophage marker), elucidating their roles and potential correlations. Additionally, histological analysis was conducted to evaluate the intensity of inflammatory infiltrates in chronic periapical lesion samples. METHODS: Tissue samples from fifty individuals with chronic periapical lesions [25 radicular cysts (RCs) and …

Binary Decision Trees for Preoperative Periapical Cyst Screening Using Cone-beam Computed Tomography.

INTRODUCTION: Cone-beam computed tomographic (CBCT) analysis allows for 3-dimensional assessment of periradicular lesions and may facilitate preoperative periapical cyst screening. The purpose of this study was to develop and assess the predictive validity of a cyst screening method based on CBCT volumetric analysis alone or combined with designated radiologic criteria. METHODS: Three independent examiners evaluated 118 presurgical CBCT scans from cases that underwent apicoectomies and had an accompanying gold standard histopathological diagnosis of either a cyst or granuloma. Lesion volume,
