Radiographic outcomes decline linearly with increased time to surgery in distal radius fractures: A cohort analysis.

INTRODUCTION: The optimal timing for distal radial fracture fixation remains controversial. Most previous studies have dichotomized timing into early or delayed categories, potentially obscuring the true effect of delay. This study investigated surgical timing as a continuous variable to determine its influence on radiographic alignment. METHODS: In a retrospective multicentre cohort study, we reviewed 691 surgically treated distal radial fractures across four Swedish hospitals. Radiographic parameters assessed included dorsal tilt (primary outcome), radial inclination, ulnar variance, intra-

When Is a Volar Plate Not Enough? Indications for Alternative Fixation Techniques in Distal Radius Fractures.

Distal radius fractures are among the most frequent injuries treated in orthopedic surgery. Volar locking plates have become the preferred fixation method, offering fixed-angle stability and lower risk of extensor tendon injury compared with dorsal plating. However, in certain complex fracture patterns, volar plating alone may not provide sufficient stability. In such cases, alternative fixation strategies-such as dorsal plating, fragment-specific fixation, or spanning constructs-may be necessary. Understanding the biomechanical challenges and appropriate surgical solutions is key to optimizin…

How Preoperative Closed Reduction and Time to Surgery Impact Postoperative Palmar Inclination in Distal Radius Fractures.

Background: The anatomical reconstruction of the wrist is the aim when treating distal radius fractures. Current literature on the importance of preoperative reduction in fractures that are treated operatively is limited. Methods: This study investigated the effect of the preoperative closed reduction of distal radius fractures on the day of trauma and the time to surgery on postoperative palmar inclination. A total of eighty patients (48 females and 32 males, mean age 55.6 years) were studied retrospectively. All patients were treated with an open reduction and internal fixation. The palmar i…

Time to surgical management of distal radius fractures: effects on health care utilization and functional outcomes.

BACKGROUND: Distal radius fractures are common injuries. Open reduction and internal fixation with volar locking plates is the most common approach for surgical fixation. This study investigated the association between time to surgery and health care utilization, income, and functional outcomes among patients undergoing open reduction and internal fixation for distal radius fracture. METHODS: We conducted a retrospective review of patients who underwent open reduction and internal fixation for isolated acute distal radius fracture between 2009 and 2019. Time to surgery was grouped as early (≤…

The Hematoma Block versus Hematoma Plus Wrist Nerve Block in the Reduction of Distal Radius Fractures.

BACKGROUND: Distal radius fractures are extremely common. Most of them can be treated conservatively, but there are still many controversies about this nonoperative treatment. The purpose of this study was to assess whether the pain experienced during distal radius fracture reduction using the hematoma block plus a nerve block at the wrist is less than the perceived pain using an isolated hematoma block and whether the success of the closed reduction remains at least the same. MATERIALS AND METHODS: A total of 192 patients were included in a comparative study to assess whether the pain experi…

Predictors of Extended Length of Stay Following Open Reduction and Internal Fixation for Distal Radius Fractures.

BACKGROUND: An extended length of stay (eLOS) following open reduction and internal fixation (ORIF) for distal radius fractures has been associated with increased patient morbidity and health care costs. The primary objective of this study was to identify risk factors, including patient characteristics, comorbidities, and postoperative complications, which are associated with an eLOS following ORIF for distal radius fractures. MATERIALS AND METHODS: All patients who underwent ORIF for distal radius fractures between 2015 and 2021 were queried from the American College of Surgeons National Sur…

Teardrop Alignment Changes After Volar Locking Plate Fixation of Distal Radius Fractures With Volar Ulnar Fragments.

BACKGROUND: We assessed factors associated with change in radiographic teardrop angle following volar locking plate (VLP) fixation of volarly displaced intra-articular distal radius fractures with volar ulnar fragments (VUF) within the ICUC database. The primary outcome was change in radiographic alignment on follow-up imaging, defined as a change in teardrop angle from intra-operative fluoroscopy greater than 5°. METHODS: Patients with distal radius fractures treated with a VLP within the ICUC database, an international collaborative and publicly available dataset, were identified. The prima…

Management Options and Clinical Outcomes After Flexor Tendon Ruptures Following Volar Plating of Distal Radius Fractures.

BACKGROUND: The transition to volar locking plates for the treatment of distal radius fractures has improved patients' range of motion, reduced extensor tendon issues, and hastened return to activity compared to older fixation methods or nonoperative treatment. One drawback to volar plating is the potential for flexor tendon ruptures due to aberrant plate or screw placement. This study aims to describe the reconstruction options and patient outcomes after flexor tendon rupture due to volar plating. METHODS: This is a single-institution, retrospective review of patients who presented with flex…

Technique, Indications and Limitations of a Minimally Invasive Approach to Treat Distal Radius Fractures and Malunions.

The minimally invasive flexor carpi radialis approach can be used for volar locking plate fixation of distal radius fractures. It offers the advantage of preserving ligamentotaxis, which facilitates the reduction and the small size of the scar improves the esthetic result of the procedure. It is indicated for extra-articular fractures of the distal radius. In the case of an intra-articular fracture, an arthroscopy may be associated. In the case of a proximal diaphyseal extension of the fracture, a second proximal approach can be added in order to use a longer plate. Relative contraindications …

The Safety of the Volar Intraarticular Extended Window (VIEW) Approach for Intra-articular Distal Radius Fractures.

BACKGROUND: A novel volar approach to intra-articular distal radius fractures has been introduced for treatment of intra-articular distal radius fractures, in which volar extrinsic ligaments are released to create a volar window into the radiocarpal joint (Volar Intraarticular Extended Window [VIEW] approach). Our purpose was to evaluate the safety of VIEW approach for treatment of intra-articular distal radius fractures. METHODS: A retrospective chart review was performed for 13 patients with intra-articular distal radius fractures treated operatively with the VIEW surgical technique using a…

Evaluation of the Distal Extent of Visualization Using Volar Approaches for Fixation of Distal Radius Fractures.

PURPOSE: Distal radius fractures may include difficult-to-treat fractures of the volar ulnar corner, colloquially termed as the "critical corner." These fragments are problematic because they are often missed, and their fixation is difficult. We aimed to compare the distal extent of the exposure of the volar surface of the distal radius between the classic Henry approach and the extended flexor carpi radialis (EFCR) approach. METHODS: Thirteen matched-pair specimens were randomized to receive either the Henry approach or the EFCR approach to the volar distal radius. A mini Hohmann retractor p…

Distal Radius Plates With Coracoid Suture Anchors for the Fixation of Displaced Lateral End Clavicle Fractures.

Introduction Management of displaced fractures of the lateral end clavicle has always challenged orthopaedic surgeons, due to a high non-union rate, and difficulty achieving stable fixation allowing early mobilisation. Different methods of fixation have been utilised to provide stability and improve healing and functional outcomes. In this series, we evaluate the results of internal fixation using dorsal distal radius locking plates and coracoid suture anchors. Patients and methods We retrospectively reviewed the clinical notes and radiographs of eight consecutive patients with fractures of th

Serum Vitamin D Levels in Patients With Distal Radius Fractures Presenting to a Tertiary Care Hospital.

Objective To compare the frequency of serum vitamin D levels in patients with and without distal radius fracture. Introduction Hypovitaminosis D is a common problem worldwide. Deficiency of vitamin D is not only associated with musculoskeletal problems, but also with chronic diseases like diabetes mellitus, cardiovascular disease and cancer. Vitamin D deficiency results in bone pain, aggravating osteoporosis and fragility fractures. Distal radius fractures are common upper limb fractures, mostly in the elderly population. Low serum levels of vitamin D have been reported to result in increased …

Arthroscopic debridement of the dorsal capsule in intraarticular distal radius fractures: does it provide superior outcomes?

INTRODUCTION: Distal radius fractures (DRFs) are very common. One of the most significant complications after intraarticular DRF is arthrofibrosis with loss of wrist motion and pain. Wrist arthroscopy has become increasingly popular in the treatment of DRF with the advantage of good visualization of the joint surface and soft tissue injuries. In intraarticular DRFs injuries of the dorsal capsule are a characteristic finding which potentially cause loss of wrist motion. In this study, we investigated if arthroscopic debridement of dorsal capsule injuries at time of surgical fixation provides su…

Forecasting effects of "fast-tracks" for surgery in the Swedish national guidelines for distal radius fractures.

BACKGROUND AND PURPOSE: National guidelines for treatment of distal radius fractures (DRFs) were presented in Sweden in 2021. In the guidelines, a fast-track is recommended for 4 subgroups of highly unstable DRFs. Regardless of the results of the closed reduction these are recommended for surgery within 1 week of injury. This study aims to evaluate the potential consequences of the newly presented national guidelines on incidence of surgical interventions. PATIENTS AND METHODS: In all, 1,609 patients (1,635 DRFs) with primary radiographs after a DRF between 2014 and 2017 at two Swedish hospit…
