Effects of age and isolated systolic hypertension on cardiovascular reflexes.

OBJECTIVES: Given the reported relationship between systolic hypertension and orthostatic hypotension in the elderly, to test the hypothesis that systolic hypertension causes impairment of the cardiovascular reflex function additional to the effects of age alone. DESIGN: Responses were compared in normotensive healthy young (n = 12) and elderly (n = 15) participants and elderly participants with disproportionate supine systolic hypertension (n = 11) using a baroreceptor-mediated stress (head-up tilt) and two non-baroreceptor-mediated stimuli (cold pressor test and isometric exercise). METHOD…

The excitability of blink reflexes in patients with neurologic and psychiatric disorders.

The blink reflex and its recovery curve were studied in 12 patients with neurologic and psychiatric disorders, i.e., juvenile Parkinson's disease (case 1), Gilles de la Tourette's syndrome (case 2), anorexia nervosa (case 3), mild developmental delay (cases 4 and 5), a meningomyelocele with winking spasms (case 6), Parkinson's disease (cases 7-10) and OPCA (cases 11 and 12). The cases were divided into 4 groups on the basis of the results, (1) hyperexcitability of facial motoneurons only (case 6), (2) hyperexcitability of facial motoneurons and brainstem interneurons (case 1 and 2), (3) hypere…

Are H and stretch reflexes in hemiparesis reproducible and correlated with spasticity?

The measurement of spasticity has always presented a problem to clinicians and researchers alike. As yet, there is no literature addressing the reproducibility of the existing barrage of clinical evaluations of spasticity and reflex measurement. Also not clear is whether or not a systematic relationship might exist between these multiple indices of spasticity. After delineating the differences in spasticity scores and reflex functions between spastic and normal states, the aims of this study were to examine the reliability of these measurements in hemiparetic subjects, and the correlation betw…

Electrophysiological study of superficial abdominal reflexes in normal men.

We studied the superficial abdominal reflexes of 83 normal men, using as stimuli a train of electrical pulses or a needle scratch. Electrical stimulation delivered to the midline of the abdominal wall evoked, almost symmetrically on both sides, two reflex discharges: an early response having an oligophasic wave form, and a late response of polyphasic wave form. The threshold of the early response significantly exceeded that of the late response. With repetitive stimulation, the late response generally revealed habituation. Electrical stimulation of the unilateral abdominal wall evoked two resp…

The photic sneeze reflex as a risk factor to combat pilots.

The tendency to sneeze upon exposure to bright light is autosomal dominant and affects 18-35% of the population. This uncontrolled sneezing … ters in a clinical setting showed no effect on sneezing in a susceptible subject. The photic sneeze reflex does not appear to be mediated by specific wavelengths of light, but rather by the change in light intensity. This could trigger an unexpected sneezing episode during critical periods of flight. This is an unrecognized and previously unreported dang …

Stapedius reflexes to electrical stimulation in the rabbit for the assessment of hearing.

The possibility of using electrically induced stapedius reflexes as a means of objective hearing evaluation was investigated in the rabbit as an animal model. The contralateral stapedius reflex to acoustic and electrical stimulation was measured in anesthetized rabbits. For electrical stimulation, the middle ear was opened surgically and stimulation was applied by a monopolar electrode placed at or into the round window. Contraction of the middle ear muscle was monitored by digital sampling and averaging of the impedance changes in the contralateral ear. Acoustically evoked reflexes were recor…

Simulating reflex induced changes in the acoustic impedance of the ear.

A simple procedure for measuring changes in the acoustic impedance of the ear is described. The technique has several applications, including simulation using a standard coupler of changes in real ear impedance produced by the acoustic reflex. The technique has also been used for calibrating the response time of an otoadmittance meter used in measuring the temporal characteristic of reflex activity.

The prognostic value of the stapedius reflex in peripheral facial palsy.

Stapedius reflex can provide useful prognostic information in FPs when the middle ear is normal. Its preservation may have a favorable significance in partial FP and Bell's palsy. Its loss, even though less reliable, can also have some value as an indicator for a poor prognosis in patients with total and herpes zoster FPs. Most importantly, our findings have shown that the SR alone is not sufficient to be of prognostic value, but is useful if combined with other clinical parameters and electrical responses.

Electrocutaneous reflexes and multimodality evoked potentials in multiple sclerosis.

Electrical stimulation of the digital nerves of the index finger produces changes in the EMG signal during steady voluntary contraction of the first dorsal interosseous muscle. This electrocutaneous reflex (ECR) was studied in 90 patients classified into different categories according to diagnostic criteria for multiple sclerosis. In addition, pattern reversal visual evoked responses (VER), brainstem auditory evoked responses (BAER) as well as spinal and scalp recorded somatosensory evoked potentials (SSEP) were investigated by stimulating both the index finger and the posterior tibial nerve. …

[Changes in the RIII response of the flexion reflex with painful maneuvers].

In the search for reliable indirect indices of pain sensation, interest has recently focused on the nociceptive flexion reflex and late components of the brain evoked potentials. In ten volunteers with sciatica, the nociceptive flexion reflex (RIII) and the late component (N150-P220) of the evoked potentials were recorded, with the subjects at rest and during pain produced by the Lasegue manoeuvre. In recordings with the subjects at rest, both responses were stable. During the Lasegue manoeuvre, the RIII response was markedly suppressed, with a mean reduction in area of 40% (P less than 0.001)…

[Use of nociceptive flexion reflex in the evaluation of morphine analgesia].

A close correlation between the threshold of the nociceptive flexion reflex and pain threshold makes this reflex useful for the examination of pain and analgesia level. Earlier investigations showed that in sciatica the threshold of the nociceptive reflex was decreased in the extremity with pain, and that the degree of this decrease was proportional++ to pain intensity and intensity of radicular sings. In the presently reported study the nociceptive flexion reflex was investigated in 20 patients after epidural injection of 5 mg of morphine. After this blockade the degree of analgesia and the v…

Early-onset cerebellar ataxia with retained tendon reflexes.

The authors report a clinical review of 16 childhood cases with early-onset cerebellar ataxia with retained tendon reflexes. The preservation of tendon reflexes distinguishes this disorder from Friedreich's ataxia. The mean age of onset of symptoms was 7.1 years. The main presenting symptom was abnormal gait (100%). Ataxia of gait and limbs and normal or increased tendon reflexes were found in all cases. This disorder is associated with dysarthria, pyramidal signs in the limbs, and in some instances, sensory loss. Other important differences from Friedreich's ataxia are absence of optic atroph…

Stapedius reflex in the monitoring of NaF treatment of subclinical otosclerosis.

Stapedius reflex testing was utilized to monitor the progression of subclinical otosclerosis and evaluate the efficacy of NaF treatment in stabilizing the disease process. The investigation was performed on 93 relatives of patients suffering with surgically confirmed otosclerosis. Subjects were divided into two groups. The first group was treated with NaF in doses ranging from 6 to 16 mg in relation to age. The treatment lasted two years. The second group served as control subjects. Before entering the research, all subjects presented reflex abnormalities represented by a partial or complete o…

Influence of sulphur dioxide breathing on defensive reflexes of the airways.

The influence of sulphur dioxide (SO2) on cough and expiratory reflexes was studied in 22 anaesthetized (pentobarbital, 30 mg/kg i.v.) rabbits. The cough reflex (CR) was elicited by tracheobronchial mucous membrane stimulation, using a soft venous catheter. In order to induce the expiratory reflex (ER) irritation of the larynx was performed by a silon fibre loop. The strength of both reflexes was assessed from the interpleural pressure fluctuation before SO2 breathing, immediately after, and 30-120 minutes after SO2 breathing had been stopped. The animals breathed SO2 in 200-300 ppm concentrat…

Differential modulation of the baroreceptor reflex by brain and plasma vasopressin.

Plasma vasopressin sensitizes the baroreceptor reflex, whereas vasopressin given into the cerebral ventricle overrides the baroreceptor reflex by means of sympathetic stimulation. To test the hypothesis that arginine vasopressin stimulates two different receptor subtypes (V1 and V2) in the central nervous system, we measured the baroreceptor reflex (change in pulse interval vs change in blood pressure) after administering methoxamine (10-300 micrograms/kg i.v.) in conscious rats. Animals were pretreated either with a V1 vasopressin receptor antagonist administered intravenously or intracerebro…

Spinal opiate modulation of cardiovascular reflexes in the exercising dog.

The possible role of spinal opiate receptors on hind limb muscle afferents, participating in arterial blood pressure and heart rate responses to ischemic exercise, were investigated following lumbosacral intrathecal injection of morphine and naloxone in chronically instrumented dogs. Morphine markedly attenuated these responses. Naloxone had no effect alone but blocked the morphine responses. Thus, spinal opiate receptors may modulate ascending afferent information mediating cardiovascular reflexes from ischemic muscle.

Relationship between primitive reflexes and severity in Parkinson's disease.

The reliability of primitive reflexes as monitors of dementia, depression, and severity of Parkinson's disease were studied. The results indicated that the proportion of certain reflexes increased with severity of disease and cognitive impairment, but no relationship was seen between any of the reflexes and degree of depression.

Upper airway reflexes in newborns with respiratory distress syndrome.

In 16 newborns with respiratory distress syndrome (RDS) not artificially ventilated, oesophageal pressure was recorded with an electromanometer and the reactions to mechanical stimulation of the upper airways by means of a nylon fibre were investigated. The results were compared with the values obtained in 16 newborns without cardiorespiratory disturbances. The reactions were evoked by mechanical stimulation of the airways. Mechanical stimulation of the nasal mucosa produced expulsive reactions only in 45% of cases in newborns with RDS as compared with 95% in healthy newborns (p less than 0.00

Vasoactive intestinal polypeptide in the hyper-reflexic neuropathic bladder.

The distribution of nerves containing vasoactive intestinal polypeptide (VIP) in the hyper-reflexic neuropathic bladder was investigated using immunocytochemistry and radioimmunoassay. Biopsy specimens were taken from the bladders of 21 patients with urodynamically proven detrusor hyper-reflexia and from 20 control patients with no urodynamic abnormality. The results showed a reduction in the number of VIP immunoreactive nerves and a reduction in VIP concentration in the hyper-reflexic bladder when compared with control samples.
