[Prolongated amnesia after "rohypnol" i.v. before local anesthesia and responsiveness during operation (author's transl)].

An up-to-eight-hour lasting anterograde amnesia is achieved by "Rohypnol" i.v. causing no excitation before local anesthesia is given (prolongation by analgetics, anesthetics, neuroleptics). The cardiovascular functions remain stable with spontaneous breathing and preservation of the swallow and coughing reflexes. During the operation the patient is responsive and cooperative. The patient answers all questions, moves his body into any wanted position and when ordered he performs Valsalva's manoeuver without any remembrane. If not spoken to and having no pain he falls asleep right away. Postope

Lung function and morphology after bilateral hilar stripping in dogs.

Acute changes of lung function and morphology were studied in 3 groups of dogs: after bilateral hilar stripping by ipsilateral approach (group 1), after right-side hilar stripping and left pneumonectomy by left-sided approach only (group 2) and in a control group. All dogs of group 1 showed a syndrome corresponding to functional hilar stripping with absent Hering-Breuer and cough reflex, slow respiration with increased tidal volume and discharge of fluid from the airways. Alveolar ventilation and oxygen tension, dead space/tidal volume ratio and pulmonary shunting did not change. Cardiac outpu

Adult-onset epilepsy with startle-induced seizure after febrile infection-related epilepsy syndrome: A case report.

Startle-induced seizure is a rare type of reflex seizure triggered by unexpected sensory stimuli that often occurs in children with early acquired cerebral lesions or brain malformations. We report a unique case of adult-onset epilepsy with startle-induced seizures. A 24-year-old woman had suffered high fever and focal to bilateral tonic-clonic seizures. A diagnosis of febrile infection-related epilepsy syndrome (FIRES) was made based on the febrile infection occurring 7 days to 24 h before the onset of status epilepticus, which met all criteria for cryptogenic new-onset refractory status epil

Case report: Spiller syndrome initially mimicking vestibular neuritis.

Spiller syndrome is a rare subtype of medial medullary infarction (MMI). Herein, we report on a patient with progressing stroke who presented with the initial features of acute peripheral vestibulopathy and MMI (Spiller syndrome), as confirmed by magnetic resonance imaging (MRI). A 42-year-old man experienced acute persistent vertigo with nausea, vomiting, and severe gait instability for 6 h before presenting to the emergency department. He exhibited spontaneous right-beating horizontal-torsional nystagmus that intensified on rightward gaze. The patient fell to the left side during the Romberg

Cutaneous information processing differs with load type during isometric finger abduction.

During submaximal isometric contraction, there are two different load types: maintenance of a constant limb angle while supporting an inertial load (position task) and maintenance of a constant force by pushing against a rigid restraint (force task). Previous studies demonstrated that performing the position task requires more proprioceptive information. The purpose of this study was to investigate whether there would be a difference in cutaneous information processing between the position and force tasks by assessing the gating effect, which is reduction of amplitude of somatosensory evoked p

Ganglioglioma of the Right Cerebrothalamus in a 7-Year-Old Quarter Horse Cross Gelding.

Intracranial neoplasia in horses is rare compared to other species. Detailed information such as neurological, electroencephalographic, and histopathological examination of horses with intracranial neoplasia associated with seizures is scarce in the literature. Furthermore, ganglioglioma has not been reported in the horse. A 7-year-old Quarter horse cross Paint gelding was examined due to recurrent seizure-like episodes of 1-year duration. The seizures had been increasing in frequency and length, occurring up to 20 times a day at the time of presentation. Neurological examination revealed inte

Lipomatosis of the parotid salivary gland: 2 case reports and review of the literature on fat-containing salivary gland lesions.

Two cases of lipomatosis of the parotid salivary gland in two dogs are reported. Both dogs were presented with a slow growing unilateral non-painful mass in the parotid region. The mass measured 12 and 15 cm on the longest axis for case 1 and 2 respectively. Initial incisional biopsies were suggestive of infiltrative lipoma or salivary gland adenocarcinoma. Case 2 underwent head radiographs that showed a soft tissue opacity on the area of the right parotid gland followed by a computed tomography that showed a multi-lobulated infiltrative mass occupying the right parotid gland. Both cases under

Spinal regions involved in baroreflex control of renal sympathetic nerve activity in the rat.

Spinal cord injury causes debilitating cardiovascular disturbances. The etiology of these disturbances remains obscure, partly because the locations of spinal cord pathways important for sympathetic control of cardiovascular function have not been thoroughly studied. To elucidate these pathways, we examined regions of the thoracic spinal cord important for reflex sympathetic control of arterial pressure (AP). In anesthetized rats, baroreceptor relationships between pharmacologically induced changes in AP and changes in left renal sympathetic nerve activity (RSNA) were generated in spinally int

Direct and reflex cardiac bradydysrhythmias from small vagal nerve stiumaltions.

Alterations in cardiac pacemaker location, its rate of discharge, and A-V conduction patterns were induced in anesthetized adult dogs by electrical stimulation of the thoracic vagi and their small cardiac branches before and after cervical vagotomy. Electrical activity from small, contiguous bipolar silver electrodes was amplified and recorded by an optical oscillograph. The electrodes were located over the SA node, the three internodal pathways, the left atrium, and ventricular epicardium. A hoffman-type plaque electrode was placed over the A-V node to record a His bundle electrogram simultan

Beyond the Ordinary: An Atypical Guillain-Barré Syndrome Case With Unique Characteristics.

We present a patient with pharyngeal-cervical-brachial Guillain-Barré syndrome (PCB-GBS) that progressed to a severe state followed by a quick recovery after treatment. This unique clinical course has not been documented previously and provides a potentially invaluable description of a novel GBS variant. A 42-year-old man arrived at the emergency department with a 24-hour history of dysphagia, weakness in his right arm, and bilateral shoulder weakness. Nerve conduction velocity testing revealed bilateral sensory and motor polyneuropathy, leading to the diagnosis of GBS with the PCB variant. Ti

[A case of chronic pure motor neuropathy associated with anti-GalNAc-GD1a-IgM antibody].

The patient was a 34-year-old male with chronic pure motor neuropathy with such acute onset as seen in Guillain-Barré syndrome. Neurological symptoms were gradually progressive for 4 weeks, and predominantly noted in the left side. Deep reflexes were normal and the distribution of muscle weakness was uneven. Plasma exchange reduced neurological symptoms. Four weeks later, right drop foot was relapsed. High dose intravenous immunoglobulin was effective. Serial electrophysiological studies indicated the asymmetric reduction of CMAP. Repeated assays of anti-GalNAc-GD1a IgM antibody were positive.

Syndrome of inappropriate secretion of antidiuretic hormone in a patient affected by metastatic melanoma.

The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is characterized by hyponatraemia due to water retention resulting from the persistent release of antidiuretic hormone (vasopressin). It may occur in a variety of malignant and non-malignant conditions, in particular in association with oat cell carcinoma, pulmonary and cerebral diseases. We report the case of a male patient affected by melanoma of the right temporal region with brain metastasis who developed acute headache, drowsiness, nausea, vomiting and pathological reflexes. Clinical and laboratory investigations led

Orthostatic hypotension in a case with multiple sclerosis.

A 37-year-old woman with a 5-year history of multiple sclerosis is reported. She began having recurrent syncope even in the sitting position; other neurological features included hiccup, faciooro-lingual flushing and clumsiness of the hands. She had alternating Horner's syndrome, mild hypoalgesia of the right face, exaggerated deep tendon reflexes of the upper extremities, decreased deep sensation and ataxia of the upper extremities, and incomplete transverse myelopathy with a T4 sensory level. Head-up tilt testing confirmed orthostatic hypotension with relative preservation of the heart rate

[Post-reperfusion syndrome in orthotopic liver transplantation].

Post-reperfusion syndrome is the most common hemodynamic pattern in liver transplantation, manifesting mainly through decreased heart rate, mean arterial pressure and systemic vascular resistances. Five factors have been implicated in its genesis: the heart, the circulatory system, the metabolism, reflexes and surgery. Ventricular function, both right and left, has been shown to be normal during reperfusion, in which case the visceral and, especially, liver dilation that occurs would be the main cause of arterial hypotension. Patients in poor physical condition before the transplant are more l

[A case of myokymia with abnormal F responses].

We reported a case of myokymia with abnormal F response. A 60-year-old male with chronic alcoholism was admitted to our hospital with complaint of muscle cramp in both legs just after drinking. Neurological examination revealed muscle pain and weakness of the legs, absence of bilateral Achilles tendon reflexes and prominent myokymia in his right quadriceps femoris and left calf muscles. The electrophysiological examination showed reduced conduction velocity, high amplitude, increased number of phases and long duration of F responses suggesting increased excitability of motoneuron pool. The epi

[Bronchopleural fistula after transbronchial passage of a feeding tube].

Inadvertent transbronchial insertion of a narrow-bore feeding tube using a metallic wire resulted, twice in less than 24 hours, in a right pneumothorax complicated by a bronchopleural fistula. This accident occurred in a conscious patient but with central neurologic chronic disorders recently aggravated by a postoperative Gayet-Wernicke syndrome. The incidence of transbronchial insertion is relatively important [2 to 4%]. Risk factors include a previous endotracheal intubation and an altered mental status. The insertion may be especially hazardous when the patient is uncooperative or unconscio

Anorectal dysfunction and delayed colonic transit in patients with progressive systemic sclerosis.

We studied 14 unselected patients with progressive systemic sclerosis (PSS), six with constipation, and eight with normal bowel habits. A control group, matched for age and sex, comprised six patients with idiopathic constipation and seven healthy subjects. Anorectal manometry was performed with perfused catheters and segmental colonic transit was measured by a radiopaque marker technique. The resting pressure of the anal canal was significantly reduced in PSS with constipation (P < 0.05). The rectoanal inhibitory reflex was detected in only one PSS patient with constipation, but was present i
