OBJECTIVE: To explore stakeholder perspectives upon participant retention in clinical trials, and to generate strategies to support retention in a surgical, clinical trial.
STUDY DESIGN AND SETTING: The SWIFFT trial is a multicenter study comparing treatments for the fracture of the waist of the scaphoid bone in adults. Here we report upon a multistage, iterative consultative process with SWIFFT stakeholders, these include workshops with members of the public, with nurses involved in data collection, and with consultant clinicians. Structured discussions were digitally recorded and transcribe…

Background: Isolated scaphoid fractures (ISFs) are common, whereas transscaphoid fracture-dislocations (TSFDs) are not. Scaphoid fracture location and the extent of comminution are factors that affect treatment and outcome. The purpose of this study is to compare the radiographic characteristics of ISFs with TSFDs associated with greater arc injury. Methods: This study is a retrospective review of all ISFs and TSFDs that presented to our institution during a 5-year period. Fracture location (along the long axis of the scaphoid) was calculated by dividing the distance from the proximal pole to

To promote a quicker return to function, an increasing number of patients are treated with headless screws for acute displaced and even non-displaced scaphoid fractures. Therefore, it is imperative to understand and optimize the biomechanical characteristics of different implants to support the demands of early mobilization. The objective of this study was to evaluate the biomechanical fixation strength of 4 headless compression screws under distracting and bending forces. The Acutrak Standard, Acutrak Mini, Synthes 3.0, and Herbert-Whipple screws were tested using a polyurethane foam scaphoid

Fractures of the scaphoid are the most common surgically treated carpal fracture, and early diagnosis is critical to minimize complications including osteonecrosis. If the initial radiographs after the injury are inconclusive, early magnetic resonance imaging (MRI) provides an immediate diagnosis to allow for proper management. This has been shown to be cost effective both in direct measureable costs and likely in difficult-to-measure indirect costs related to lost productivity. In the cases in which no scaphoid fracture is present, MRI provides alternate diagnoses such as identification of ot

A suspected fracture of the scaphoid remains difficult to manage despite advances in knowledge and imaging methods. Immobilisation and restriction of activities in a young and active patient must be balanced against the risks of nonunion associated with an undiagnosed and undertreated fracture of the scaphoid. The assessment of diagnostic tests for a suspected fracture of the scaphoid must take into account two important factors. First, the prevalence of true fractures among suspected fractures is low, which greatly reduces the probability that a positive test will correspond with a true fract

A 39-year-old patient was presented with, to our knowledge, the first ever diagnosed case of extensor pollicis longus (EPL) tendon rupture as a result of scapholunate (S-L) dissociation. Arthroscopic findings showed that the dorsal aspect of the S-L ligament was ruptured and severe synovitis was revealed around the S-L joint. Direct observation revealed synovium at the rupture site of the EPL tendon. After careful resection of this synovium, an aperture in the S-L joint was observed. Thus, it was considered that the synovium in the S-L joint was connected to the rupture site of the EPL tendon,

Principles, and not established rules, form the basis for treating children with painful, posttraumatic arthritis involving the hand and wrist. The authors' preference is to exhaust nonoperative measures unless the involved joints are unstable or grossly incongruent. The authors would also recommend a greater degree of cautious observation in young children because of reports of successful outcomes associated with remodeling. Once surgery is necessary, the authors prefer motion-preserving procedures, bearing in mind that arthrodesis is well tolerated in the thumb interphalangeal joint and the

OBJECTIVE: Perilunate injuries are rare but devastating carpal injuries. The treatment of perilunate injuries remains challenging and contentious. This study aims to describe a novel mini-invasive surgical technique of arthroscopic-assisted reduction and robot-assisted fixation for the treatment of trans-scaphoid perilunate fracture dislocations (PLFDs).
METHODS: We retrospectively reviewed patients with PLFDs after surgical treatment from February 2021 to March 2021. The patients underwent arthroscopic-assisted precise reduction and robot-assisted headless screw fixation. Times of guide wire

Four-corner fusion is a reliable surgical option for symptomatic scapholunate advanced collapse and scaphoid nonunion advanced collapse. There are multiple current techniques with potential complications, including nonunion, hardware failure, impingement, and cartilage disruption. Despite these potential complications, 4-corner fusion can result in good functional outcomes and patient satisfaction. We present a cartilage-sparing technique using snap-off compression pins for 4-corner fusion. Potential benefits of this technique include strong internal fixation and stable compression with implan

BACKGROUND: Articular screw penetration is one of the most common hardware-related problems after scaphoid fracture fixation, occurring in up to two-thirds of patients, in particular into the scaphotrapezotrapezoidal (STT) joint. The aim of this study was to investigate whether this clinically important issue could be detected using standard anteroposterior (AP) and lateral, as well as additional nonstandard fluoroscopic views using direct open visualization with magnifying loupes as reference standard.
MATERIALS AND METHODS: Ten fresh cadaver wrists were used for this imaging study. A 2.2 mm

CASE: A 32-year-old man sustained a scaphoid waist fracture and was managed nonoperatively in a short-arm cast. Radiographic follow-up at 6 weeks showed marked loss of radiodensity of the whole scaphoid, although the fracture was clinically united. Patchy ossification at 3 months was followed by the complete restoration of radiodensity and trabecular pattern by 20 weeks. The patient had an uneventful and full functional recovery.
CONCLUSION: This, perhaps unreported, finding seems to be an indicator of preserved vascularity in scaphoid fractures. Further research/reporting of this radiologica
