Axillary lymph node dissection versus radiotherapy in breast cancer with positive sentinel nodes after neoadjuvant therapy (ADARNAT trial).

INTRODUCTION: Breast cancer surgery currently focuses on de-escalating treatment without compromising patient survival. Axillary radiotherapy (ART) now replaces axillary lymph node dissection (ALND) in patients with limited sentinel lymph node (SLN) involvement during the primary surgery, and this has significantly reduced the incidence of lymphedema without worsening the prognosis. However, patients treated with neoadjuvant systemic treatment (NST) cannot benefit from this option despite the low incidence of residual disease in the armpit in most cases. Data regarding the use of radiotherapy …

Clinicopathologic features associated with having four or more metastatic axillary nodes in breast cancer patients with a positive sentinel lymph node.

BACKGROUND: The survival benefit of a completion axillary lymph node dissection (ALND) in patients after removal of a metastatic sentinel lymph node (SLN) is uncertain and is under study in ongoing clinical trials. The completion ALND remains necessary, however, for the identification of cases with at least four metastatic lymph nodes, in which extended-field locoregional and/or postmastectomy radiation will be recommended. Our goal was evaluate clinicopathologic features that might serve as surrogates for determining which patients with a positive SLN are likely or unlikely to belong to this …

Development of an optimized MRI protocol for a rapid preoperative identification of sentinel lymph nodes using superparamagnetic iron oxide - The Gothenburg fast acquisition sentinel lymph node tracking magnetic resonance imaging protocol (GO-FAST-MRI).

INTRODUCTION: Determining sentinel lymph node (SLN) status is important for treatment decisions in patients with melanoma. Superparamagnetic iron oxide nanoparticles (SPIO) combined with MRI have emerged as an alternative to Technetium99m lymphoscintigraphy for preoperative mapping of SLN, however, the MRI protocols so far are extensive with long in-camera time. This study aimed to evaluate an optimized MRI protocol for rapid identification of SLNs using SPIO as a tracer, without compromising diagnostic quality, the GOthenburg Fast Acquisition Sentinel lymph node Tracking MRI (GO-FAST-MRI). M…

Omission of Axillary Lymph Node Dissection in Breast Cancer Patients with 1-2 Positive Sentinel Lymph Nodes: A Multicenter Real-World Cohort Study in a Chinese Population.

The optimal management of patients with limited sentinel lymph node metastasis in breast cancer, particularly regarding whether to perform additional axillary surgery, continues to be an area of clinical uncertainty in routine practice. This multicenter retrospective cohort study aimed to evaluate adherence to ACOSOG Z0011 criteria and the oncological safety of omitting ALND in a Chinese population. We included 462 women with clinical stage T1-2N0 breast cancer who underwent breast-conserving surgery and were found to have 1-2 positive SLNs between January 2013 and December 2021. All patients …

Sentinel Lymph Node Biopsy vs No Axillary Surgery in Patients With Small Breast Cancer and Negative Results on Ultrasonography of Axillary Lymph Nodes: The SOUND Randomized Clinical Trial.

IMPORTANCE: Sentinel lymph node biopsy (SLNB) is the standard of care for axillary node staging of patients with early breast cancer (BC), but its necessity can be questioned since surgery for examination of axillary nodes is not performed with curative intent. OBJECTIVE: To determine whether the omission of axillary surgery is noninferior to SLNB in patients with small BC and a negative result on preoperative axillary lymph node ultrasonography. DESIGN, SETTING, AND PARTICIPANTS: The SOUND (Sentinel Node vs Observation After Axillary Ultra-Sound) trial was a prospective noninferiority phase…

Failure of sentinel lymph node mapping in breast cancer patients qualified for treatment sparing axillary lymph nodes-Clinical importance and management strategy-One-center analysis.

Sentinel lymph node biopsy (SLNB) is a standard in diagnostic and therapeutic management of patients with nonadvanced invasive breast cancer. The aim of this paper was to evaluate the clinical importance of the failure of sentinel lymph node (SLN) identification during SLNB performed to spare axillary lymph nodes. A total of 5396 patients with invasive breast cancer qualified for SLNB, treated in a period from Jan 2004 to June 2018. All cases of the failure of SLN identification and reasons underlying this situation were analyzed retrospectively. In 196 (3.6%) patients, SLN was not identified …

Evolution of the Use of Completion Axillary Lymph Node Dissection in Patients with T1/2N0M0 Breast Cancer and Tumour-Involved Sentinel Lymph Nodes Undergoing Mastectomy: A Cohort Study.

BACKGROUND: In breast cancer, completion axillary lymph node dissection (cALND) was previously recommended for patients with at least one tumour-affected sentinel lymph node (SLN). Several prospective trials predominantly in patients undergoing breast-conserving surgery showed no benefit and increased arm morbidity with this procedure. We report the influence of these trials on clinical practice of patients undergoing mastectomy. METHODS: We analysed prospectively collected data from patients with primary invasive breast cancer treated at German breast cancer units between January 2008 and De…

Outcome following sentinel lymph node biopsy-guided decisions in breast cancer patients with conversion from positive to negative axillary lymph nodes after neoadjuvant chemotherapy.

PURPOSE: Many breast cancer patients with positive axillary lymph nodes achieve complete node remission after neoadjuvant chemotherapy. The usefulness of sentinel lymph node biopsy in this situation is uncertain. This study evaluated the outcomes of sentinel biopsy-guided decisions in patients who had conversion of axillary nodes from clinically positive to negative following neoadjuvant chemotherapy. METHODS: We reviewed the records of 1247 patients from five hospitals in Korea who had breast cancer with clinically axillary lymph node-positive status and negative conversion after neoadjuvant…

VEILND (Video Endoscopic Inguinal Lymph Node Dissection) with Florescence Indocyanine Green (ICG): A Novel Technique to Identify the Sentinel Lymph Node in Men with ≥pT1G2 and cN0 Penile Cancer.

INTRODUCTION: In men with ≥pT1G2 cN0, penile cancer lymph node sampling is recommended with either (1) scintigraphically labelled Dynamic sentinel lymph node biopsy (DSLNB) or (2) modified inguinal lymph node dissection (MILND). Although DSLNB is a minimally invasive technique, the false negative rate can be about 10%, and a further operative procedure is required if positive. Open MILND is a diagnostic and therapeutic option but has a much higher morbidity. A potential compromise is the technique of LND-VEILND (video endoscopic inguinal LND) that can be combined with ICG florescence marking o…

Extramammary Paget's Disease With Bilateral Sentinel Lymph Node Biopsy: a Multicenter Study of Lymphatic Patterns and Outcomes.

BACKGROUND: Extramammary Paget's disease (EMPD) can metastasize to lymph nodes or distant sites. Sentinel lymph node biopsy (SLNB) is recommended for invasive EMPD without clinical nodal involvement. OBJECTIVES: Lesions near or across the midline may show bilateral lymphatic drainage, but the clinical significance of bilateral SLNB remains unclear. This study evaluated SLN metastatic distribution and its association with clinicopathological features, lymphatic drainage patterns, and survival in patients undergoing bilateral SLNB. METHODS: We retrospectively reviewed 147 patients with EMPD wh…

Development of Size-Tunable Superparamagnetic Iron Oxide/Fluorescent Conjugated Polymer Composite Nanoparticles for Sentinel Lymph Node Biopsy.

Sentinel lymph nodes (SLNs) are critical diagnostic targets for detecting breast cancer metastasis. Integrating preoperative magnetic detection and fluorescence-guided surgical resection is a promising strategy for accurate and minimally invasive SLN biopsy. Achieving this requires tracers with strong fluorescence, high superparamagnetism, and precise size tunability within the 10-100 nm range because particle size significantly affects SLN accumulation. Conjugated polymer nanoparticles (Pdots) are expected to be a promising platform for integrating fluorescent and magnetic detections due to t…

Is Methylene Blue a Reliable Substitute in Sentinel Lymph Node Biopsy? A Systematic Review Across Oncologic Sites.

Источник пометил публикацию как отозванную

BACKGROUND: Sentinel lymph node biopsy (SLNB) is a pivotal technique for evaluating regional lymphatic spread in breast, melanoma, and gynecologic malignancies. It minimizes surgical morbidity by avoiding complete lymphadenectomy in early-stage disease. The standard dual-tracer approach-combining a radiocolloid such as technetium-99m with a blue dye like patent blue or isosulfan blue-has high sensitivity but is not universally accessible. Radiocolloids require nuclear medicine facilities' licensing and pose logistical burdens, while patent blue may provoke allergic reactions, including anaphyl…
