Several reports have been published on the use of polymorphisms found in the human hemoglobin genes as a means for prenatal diagnosis of sickle cell anemia. The disadvantages of this approach reside in its limited application and the need for family analysis. Here we report that, by use of restriction endonuclease Dde I and diazobenzyloxymethyl-paper transfer procedures, a direct analysis can be made. Individuals with normal hemoglobin (AA) show two bands (175 and 201 base pairs) complementary to a 5'-specific beta-globin gene probe. Sickle cell trait individuals (AS) exhibit an additional ban…

Newborn screening for sickle cell disease (SCD) is a key public health intervention. Data from eastern Democratic Republic of the Congo (DRC) are limited. This study assessed the prevalence of SCD and the feasibility of screening in maternity units in South Kivu. Cord blood was analyzed by mass spectrometry. Among 1.300 newborns, 128 had sickle cell trait (9.8 %; 95 % CI: 8.2-11.5 %), and no homozygous cases. Screening is feasible at birth and findings support its integration into routine maternal and child health services.

Neurological complications of hemolytic anemias are rather uncommon. We are reporting two cases of hemolytic anemia presenting as chorea and recurrent ischemic stroke. The first one is a case of chorea in a patient with sickle cell trait. Reviewing the literature we could find only one case report of chorea in sickle cell disease disease. The second is a case of recurrent ischemic stroke in hereditary spherocytosis. We could trace two reports on a Medline search, though their association was less certain.

PURPOSE: In the last half century the molecular biology, pathophysiology and natural history of sickle cell disease have been well defined. … enal medullary carcinoma, a highly lethal tumor, develops almost exclusively in young patients with sickle cell trait.
CONCLUSIONS: Heightened awareness of the genitourinary complications of sickle cell disease may prevent end stage disease, including renal failure and impotence. New forms of therapy for sickle cell disease, such as hydroxyurea, may prevent these …

The purpose of this study was to evaluate red cell distribution width (RDW) in the differential diagnosis of patients with well defined iron deficiency (ID), anemia of chronic disease (ACD), and thalassemia trait. Mean RDW in 100 "normals" was 13.1%. Elevated RDW (greater than 15.0%) was observed in 90% of ID (N = 39), 32% of ACD(N = 79), 66% of beta-thalassemia trait (N = 100), 53% of alpha-thalassemia trait men (N = 127), 34% of alpha-thalassemia trait women (N = 116), 3% of sickle cell trait (N = 101), and 88% of sickling disorders (N = 32). The Bessman classification by mean corpuscular vo…

Remarkable changes are seen on gross and microscopic examination of placenta of patients with sickle cell disorders, hence the present study was undertaken to find out the pathological changes seen in the placenta of sickle cell disorder patients, as compared to control and to study the effect of maternal sickling on the fetus. It includes total 73 cases, of which 10 were of control group and 63 were from patients with sickle cell disorders, which included 47 sickle cell trait (AS) and 16 sickle cell disease (SS) patients. In group II, 9 (14.28%) patients with SS pattern developed complication…

Hematologic evaluation of a Nigerian obstetrical patient disclosed the presence of sickle-cell trait as well as evidence of a hemoglobin alp … nd the percentage of HbS in her erythrocytes was less than that usually present in individuals with sickle cell trait. These findings, together with a reduced alpha/beta globin synthesis ratio from her peripheral blood reticulocytes, indicated that the presence of alpha-thalassemia trait. The in-vitro synthesis of hemoglobin (Hb) chains was studied among 60 Hb S heterozygotes (AS) having different quantities of Hb S, including five with an associated alpha-chain heterozygosity (ASAG). Hematologic values and hemoglobin composition were studied in these cases and in 15 other ASAG heterozygotes. The percentages of Hb S (which fell between 27% and 42%) and the mean corpuscular volume values correlated directly with the alpha/non-alpha values, confirming previous suggestions (Huisman, Hemoglobin 1:349, 1977) that the concomitant occurrence of an alpha-thalassemia-2 heterozygosity (a

Forty-two out of a total of 45 children who presented with priapism in the University College Hospital Ibadan during an eight-year period had associated sickle cell disease (SCD). Conservative management was satisfactory in 35 out of 40 patients, but the outcome in all the five cases subjected to surgery was poor. Our findings suggest that associated SCD should be suspected in children presenting with priapism in areas of the world where SCD is endemic, and that conservative management is preferable to surgery.

The acetylator phenotype was determined for 183 Nigerians after administration of sulphamethazine at a dose level of 40 mg/kg body weight and analysis of urine after 6 h. Haemoglobin electrophoresis showed that 63 people (34.4%) carried the sickle cell trait. The % of slow acetylators amongst sicklers was 39.7 and amongst non-sicklers 40.8, a difference that was not significant.

Two cases of recurrent hematuria from sickle cell disease are described. The technique of "calyoscopy" is presented, whereby the bleeding is localized, enabling a partial renal resection to be done. During a 5-year followup neither patient has had significant hematuria.
