A tissue engineering approach to bone repair in large animal models and in clinical practice.

The repair of large segmental bone defects due to trauma, inflammation and tumor surgery remains a major clinical problem. Animal models wer … injectable cells/platelet-rich plasma composites have been used as grafting materials for maxillary sinus floor augmentation and/or onlay plasty. More recently, the reconstruction of a human mandible was also reported by means of a bone-muscle-flap in vivo prefabrication technique, where the patient served as his own bioreactor. Indeed continuous implementations test and …

Surgical correction of horizontal bone defect using the lateral maxillary wall: outcomes of a retrospective study.

PURPOSE: To report on the outcomes of a new surgical technique for the treatment of severe horizontal bone resorption that impedes the placement of dental implants. MATERIALS AND METHODS: Eleven patients in need of bone augmentation to reconstruct a narrow alveolar ridge were recruited for the present study. Surgical correction of the alveolar width was performed by harvesting an onlay bone graft from the lateral wall of the maxillary sinus and filling the intervening space with plasma rich in growth factors alone or mixed with autologous bone particles. A fibrin membrane was then placed to c

Human histologic evaluation of the use of the dental putty for bone formation in the maxillary sinus: case series.

A proof-of-principle study was conducted to assess the safety and efficacy of dental putty as an alternative sinus augmentation biomaterial. Six healthy patients requiring a total of 10 sinus augmentations received sinus augmentations. All patients volunteered and signed an informed consent based on the Helsinki declaration of 1975, as revised in 2000. The sinus augmentation was performed under local anesthesia with a mucoperiosteal flap elevated to expose the buccal wall of the maxillary sinus. The space was then filled with the dental putty in several increments, and the window was covered w

Effects of Maxillary Sinus Augmentation on Endosseous Dental Implant Survival: A Systematic Review and Meta-Analysis.

Maxillary sinus floor augmentation (MSFA) is a procedure for rehabilitating the atrophic posterior maxilla, but the optimal surgical technique and its long-term impact on implant survival remain debated. This systematic review and meta-analysis were conducted to synthesize the available evidence on the long-term survival of endosseous dental implants placed in augmented maxillary sinuses. Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a search of major electronic databases was performed to identify randomized controlled trials (RCTs) and cohor…

Posterior atrophic jaws rehabilitated with prostheses supported by 5 x 5 mm implants with a novel nanostructured calcium-incorporated titanium surface or by longer implants in augmented bone. Preliminary results from a randomised controlled trial.

PURPOSE: To evaluate whether 5 × 5 mm dental implants with a novel nanostructured calcium-incorporated titanium surface could be an alternative to at least 5 × 10 mm-long implants placed in bone augmented with bone substitutes in posterior atrophic jaws. MATERIALS AND METHODS: Forty patients with atrophic posterior (premolar and molar areas) mandibles having 5 to 7 mm of bone height above the mandibular canal and 40 patients with atrophic maxillae having 4 to 6 mm below the maxillary sinus, were randomised according to a parallel group design to receive one to three 5 × 5 mm implants or one t

Reconstruction of alveolar defects using titanium-reinforced porous polyethylene as a containment device for recombinant human bone morphogenetic protein 2.

PURPOSE: The use of recombinant human bone morphogenetic protein 2 (rhBMP-2) in a collagen carrier is a proven alternative to autogenous bone grafting for certain cases. One of the principle limitations of rhBMP-2 at present, however, is the lack of a reliable rigid containment system to prevent soft tissue collapse of the collagen carrier sponge. The present case series describes the use of titanium-reinforced porous polyethylene as a containment device for rhBMP-2-grafted alveolar defects. PATIENTS AND METHODS: A case review was performed of 8 patients who had undergone rhBMP-2 bone graftin

Membranes over the lateral window in sinus augmentation procedures: a two-arm and split-mouth randomized clinical trials.

OBJECTIVE: This study evaluates whether or not, among other factors, membrane-coverage of antrostomy defects improves implant survival in sinus augmentation procedures. MATERIALS AND METHODS: We performed a two-arm and split-mouth randomized controlled clinical trial on 104 and 5 patients respectively. In the two-arm study, antrostomy defects were membrane-covered in 66 procedures and uncovered in 69, before placing a total of 265 implants that were followed up for 1 year. In the split-mouth study, following bilateral sinus augmentation, antrostomy defects were membrane-covered on one side an

Experience with bone morphogenetic protein-2 and interpositional grafting of edentulous maxillae: a comparison of Le Fort I downfracture to full-arch (horseshoe) segmental osteotomy done in conjunction with sinus floor grafting.

This paper is a retrospective report of the treatment of six patients with severely resorbed maxillae. Patients were treated, based on the amount of maxillary retrognathia, with either a Le Fort I downfracture or a "horseshoe" interpositional sandwich osteotomy, along with sinus elevation. Recombinant human bone morphogenetic protein-2 in an absorbable collagen sponge carrier was used for grafting in all patients, either alone or in combination with other grafting materials. Implants were placed and the patients were restored with fixed prostheses. Both grafting techniques are described, and t

Autotransplantation with simultaneous sinus floor elevation.

INTRODUCTION: Autotransplantation of the third molar is a viable option for replacing missing molars and has some benefits. This case report showed successful autotransplantation with simultaneous sinus floor elevation and implant installation. METHODS: The patient suffered a loss of maxillary molars and had a pneumatized maxillary sinus under the area in need of restorative treatment. Instead of implant installation in the molar area, we performed autotransplantation of the mandibular third molar with simultaneous sinus floor elevation with the aid of computer-aided rapid prototyping model.

Local sinus lift for single-tooth implant. I: clinical and radiographic follow-up.

BACKGROUND: For single-tooth implant replacement in the posterior maxilla, it is often necessary to do an augmentation of the alveolar process because of post-extraction resorption and include part of the maxillary sinus. PURPOSE: The purpose of this study is to present a technique for a local sinus lift with autogenous bone in a one-stage procedure. Additionally, volume changes of the grafted area were evaluated radiographically up to 2 years. MATERIALS AND METHODS: Twenty patients, 12 female and 8 males, were included in the study. Single-tooth replacement was made in the second premolar a
