The frontal-nasal-orbital craniotomy has been utilized for craniofacial abnormalities and resection of tumors involving the anterior skull base. We describe modifications of this technique to approach extra-axial and intradural midline lesions of the anterior fossa with or without involvement of the skull base. A craniotomy was planned with an endoscope and image guidance. A modified frontal-nasal-orbital craniotomy encompassing the entire frontal sinus complex was performed in conjunction with osteotomies incorporating the bilateral superior orbital ridges and nasal septum. Removal of the pos

Adenoid cystic carcinoma (ACC) is an aggressive salivary gland neoplasm characterized by high rates of local recurrence, early metastasis, and a poor long-term prognosis. Approximately 20-50% of cases result in distant metastasis, commonly involving the lung, bone, liver, and brain. Cutaneous metastasis is a rare occurrence, with a small number of cases reported previously in the context of multiple metastatic sites of involvement. We present a unique case of ACC of the hard palate with isolated cutaneous metastasis. A 36-year-old woman underwent a subtotal maxillectomy and skull base resectio

The basic fibroblast growth factor (bFGF) is one of the most powerful growth factors to promote wound healing. In this paper we describe two cases of skull base tumor requiring extensive skull base reconstruction, in which the application of bFGF was effective in the management and prevention of CSF leaks. The role and safeness of bFGF for skull base reconstruction are discussed.

Non-melanomatous skin cancer (NMSC) is the most common malignancy worldwide. Perineural invasion (PNI) is an uncommon (<5%), high-risk feature observed with NMSC more commonly seen in those with squamous cell carcinoma. One-third of these patients will have clinical PNI, which refers to tumor cell invasion extensive enough to cause clinically detectable deficits of the involved nerve. Uncontrolled disease within the skull base can be debilitating and, if not eradicated, fatal. The primary management options including radiotherapy with or without surgery, with local control rates exceeding 50%.

PHACE syndrome consists of the constellation of manifestations including Posterior fossa anomalies of the brain (most commonly Dandy-Walker malformations), Hemangiomas of the face and scalp, Arterial abnormalities, Cardiac defects, and Eye anomalies. We present the case of a patient who presented with respiratory distress at birth secondary to a large nasal glioma. She was subsequently found to have a ventricular septal defect (VSD), a facial hemangioma, and a malformation of the eye and optic nerve head. The nasal glioma, which extended to the cribriform plate, has not been described in this

The technical approach for anterior skull base tumors has progressed considerably during the last decade. In the classical combined craniofacial resection (CFR) technique, anterior skull base lesions have been approached through bi-frontal craniotomy coupled with Weber-Ferguson and related modifications incisions. The purpose of this work is to present our experience with the subcranial approach for treatment of anterior skull base tumors. During the years 1994-2002, 64 patients underwent 72 procedures of anterior skull base tumor resection via the subcranial approach. The ages of the patients

BACKGROUND: Meningiomas of the petroclival region (MPR) are among the most challenging skull base tumors to manage surgically. Historically, their treatment carried high risks and significant mortality due to the complex anatomy and proximity to critical neurovascular structures. Advances in microsurgical techniques and neuroimaging have significantly improved surgical outcomes. The retrosigmoid approach is a well-established technique for accessing the posterior cranial fossa, further enhanced by specific working corridors and accessory techniques.
OBJECTIVE: To illustrate the versatility of

We present a case highlighting the critical role of early recognition of partial cavernous sinus syndrome (CSS), as diagnostic delay can lead to irreversible tumor progression. A 50-year-old woman presented with subtle binocular diplopia and facial tingling sensation for 1 month. Neurological examination revealed isolated right abducens nerve (VI) palsy and paresthesia at territories of right ophthalmic (V1) and maxillary (V2) nerves. Partial CSS was suspected. Brain magnetic resonance image (MRI) revealed a contrast-enhancing mass involving the pterygopalatine fossa, sphenoid, and maxillary s

Petroclival lesions represent a surgical challenge during the endonasal endoscopic approach, as they may involve maneuvers with severe comorbidity. To avoid the morbidity caused by these maneuvers, a contralateral transmaxillary approach (CTA) has been proposed to complement the endoscopic endonasal approach. The aim of our study is to review the safety and efficacy of this approach. We included three patients with lesions affecting the petroclival region, one cholesterol granuloma and two macroadenomas, who were surgically treated with a combined ipsilateral transpterygoid approach and a CTA.

AIMS: Meningiomas are the most frequent primary brain tumours. Recently, knowledge about the molecular drivers underlying aggressive meningiomas has been expanded. A hotspot mutation in the AKT1 gene (AKT1E17K ), which is found in meningiomas at the convexity and especially at the skull base, has been associated with earlier tumour recurrence.
METHODS: Here, we analysed the effects of the AKT1E17K mutation and treatment response to the Akt inhibitor AZD5363 in transgenic meningioma cell clones and mouse xenografts modelling convexity or skull base meningiomas.
RESULTS: We show that the AKTE1

BACKGROUND AND PURPOSE: Radiotherapy of skull-base meningiomas is challenging due to the close proximity of multiple sensitive organs at risk (OARs). This study systematically compared intensity modulated proton therapy (IMPT), non-coplanar volumetric modulated arc therapy (VMAT) and intensity modulated radiotherapy (IMRT) based on automated treatment planning. Differences in OARs sparing, with specific focus on the hippocampi, and low-dose delivery were quantified.
MATERIALS AND METHODS: Twenty patients, target diameter >3 cm, were included. Automated plan generation was used to calculate a

OBJECTIVE: Meningiomas frequently occur in elderly patients, and surgical treatment is hazardous owing to multiple comorbidities. We report a series of elderly patients who were treated with surgical removal of meningiomas and compare them with a group of younger patients with similar tumors.
METHODS: A retrospective study of elderly patients with symptomatic skull base meningiomas and a matched control group of younger patients was performed. Medical records, operative reports, radiologic examinations, and follow-up data were reviewed. Main outcome measures were morbidity, mortality, and Sim
