Delayed hard palate repair and speech deficiencies: a cautionary report.

In the management of patients with complete palatal clefts early repair of the soft palate (before 1 year of age) and delayed repair of the hard palate (after five or six years of age) has been advocated on the basis that good speech will develop following soft palate closure and that avoidance of trauma to the hard palate will obviate maxillary growth disturbance. In addition, it is said that many of the remaining hard palate fistulas will close spontaneously and that residual hard palate openings will be easy to close. Thirty-two cases treated in this way are reviewed, and a decade of experi

Hearing in Real-Life Environments (HERE): Structure and Reliability of a Questionnaire on Perceived Hearing for Older Adults.

OBJECTIVES: The ability to hear in a variety of social situations and environments is vital for social participation and a high quality of life. One way to assess hearing ability is by means of self-report questionnaire. For questionnaires to be useful, their measurement properties, based on careful validation, have to be known. Only recently has consensus been reached concerning how to perform such validation and been published as COSMIN (consensus-based standards for the selection of health status measurement instruments) guidelines. Here the authors use these guidelines to evaluate the meas

How Simple Can a Treatment for Early Stuttering Be? A Proposed Two-Factor Early Intervention.

PURPOSE: In this clinical focus article, we draw attention to the need for immediate intervention shortly after stuttering onset. More than half of stuttering onsets occur before 3 years of age. We argue that existing interventions for preschool children who stutter require varying levels of cognitive engagement from children; hence, they are not suitable for children of that age. For this reason, we argue that there is no clinical trials evidence for treatment efficacy with children younger than 3 years of age. CONCLUSIONS: There are many recommended parent strategies that do not require any

Utility of a palatal speech appliance combined with the five-point eight-line segment technique in total tongue reconstruction.

The aim of this study was to explore a new total tongue reconstruction strategy based on the five-point eight-line segment (FIPELS) technique and a palatal speech appliance, and to evaluate the functional and aesthetic outcomes. Twenty patients with tongue squamous cell carcinoma were included in this study. All patients underwent total tongue resection followed by tongue reconstruction with an anterolateral thigh flap. The patients were divided randomly into two groups according to the reconstruction strategy: FIPELS group (10 patients) and traditional flap design group (10 patients). All 10

Automated Speech Recognition in Adult Stroke Survivors: Comparing Human and Computer Transcriptions.

OBJECTIVE: Speech sound errors are common in people with a variety of communication disorders and can result in impaired message transmission to listeners. Valid and reliable metrics exist to quantify this problem, but they are rarely used in clinical settings due to the time-intensive nature of speech transcription by humans. Automated speech recognition (ASR) technologies have advanced substantially in recent years, enabling them to serve as realistic proxies for human listeners. This study aimed to determine how closely transcription scores from human listeners correspond to scores from an

The efficacy of oro-facial and articulation exercises in dysarthria following stroke.

Although there are many accounts of appropriate oro-facial and articulation exercises and treatment hierarchies for clients with dysarthria following stroke, there is little evidence relating to the efficacy of such therapy and little or no information indicating either the extent or the frequency with which these exercises should be practiced for optimum effect. This study investigates the efficacy of such traditional therapy for eight clients. A clinic-based therapy programme and an exercise routine for home practice was agreed by a group of speech and language therapists. The results indica

Discrimination of computer-synthesized speech.

The effects of learning on the discrimination of computer-synthesized speech was assessed by presenting 100 computer-produced monosyllabic words to 2 groups of 15 adult subjects. One group's errors were corrected while the other group's errors were uncorrected. A comparison of errors on the first 50 vs the second 50 presentations showed significant improvement for corrected group only. It was concluded that the discrimination of computer-synthesized monosyllables can be improved with correction of errors but is still about 14% poorer than the discrimination of human speech.

Quality Standard for Rehabilitation of Young Deaf Children Receiving Cochlear Implants.

Background and objectives: More and more children with severe-to-profound hearing loss are receiving cochlear implants (CIs) at an early age to improve their hearing and listening abilities, speech recognition, speech intelligibility, and other aspects of spoken language development. Despite this, the rehabilitation outcomes can be very heterogeneous in this population, not only because of issues related to surgery and fitting or the specific characteristics of the child with his/her additional disabilities but also because of huge differences in the quality of the support and rehabilitation o…

Accented Speech Perception in Noise After Mild Traumatic Brain Injury.

PURPOSE: Perceiving nonnative-accented speech is a cognitively demanding task that requires additional cognitive effort compared to perceiving native-accented speech. People who have experienced a mild traumatic brain injury (mTBI; also commonly referred to as concussion) report impairments in an overlapping set of cognitive capacities, leading to the prediction that the perception of nonnative-accented speech may be even more difficult than it would be for someone without a history of brain injury. Of interest is whether people who have suffered an mTBI find nonnative-accented speech less int

The interaction of vocal characteristics and audibility in the recognition of concurrent syllables.

In concurrent-speech recognition, performance is enhanced when either the glottal pulse rate (GPR) or the vocal tract length (VTL) of the target speaker differs from that of the distracter, but relatively little is known about the trading relationship between the two variables, or how they interact with other cues such as signal-to-noise ratio (SNR). This paper presents a study in which listeners were asked to identify a target syllable in the presence of a distracter syllable, with carefully matched temporal envelopes. The syllables varied in GPR and VTL over a large range, and they were pres

Outcomes measurement in voice disorders: application of an acoustic index of dysphonia severity.

PURPOSE: The purpose of this experiment was to assess the ability of an acoustic model composed of both time-based and spectral-based measures to track change following voice disorder treatment and to serve as a possible treatment outcomes measure. METHOD: A weighted, four-factor acoustic algorithm consisting of shimmer, pitch sigma, the ratio of low-to-high frequency spectral energy, and a measure of the cepstral peak was used to predict dysphonia severity in pre- and post-treatment vowel samples from 88 women with primary muscle tension dysphonia treated by manual circumlaryngeal therapy. Perceptual discrimination of speech sounds in developmental dyslexia.

Experiments previously reported in the literature suggest that people with dyslexia have a deficit in categorical perception. However, it is still unclear whether the deficit is specific to the perception of speech sounds or whether it more generally affects auditory function. In order to investigate the relationship between categorical perception and dyslexia, as well as the nature of this categorization deficit, speech specific or not, the discrimination responses of children who have dyslexia and those of average readers to sinewave analogues of speech sounds were compared. These analogues

Simpson-Golabi-Behmel syndrome types I and II.

Simpson-Golabi-Behmel syndrome (SGBS) is a rare overgrowth syndrome clinically characterized by multiple congenital abnormalities, pre/postnatal overgrowth, distinctive craniofacial features, macrocephaly, and organomegaly. Abnormalities of the skeletal system, heart, central nervous system, kidney, and gastrointestinal tract may also be observed. Intellectual disability, early motor milestones and speech delay are sometimes present; however, there are a considerable number of individuals with normal intelligence.

Consonant Acoustics in Parkinson's Disease and Multiple Sclerosis: Comparison of Clear and Loud Speaking Conditions.

PURPOSE: The impact of clear speech or an increased vocal intensity on consonant spectra was investigated for speakers with mild dysarthria secondary to multiple sclerosis or Parkinson's disease and healthy controls. METHOD: Sentences were read in habitual, clear, and loud conditions. Spectral moment coefficients were obtained for word-initial and word-medial /s/, /ʃ/, /t/, and /k/. Global production differences among conditions were confirmed with measures of vocal intensity and articulation rate. RESULTS: Static or slice-in-time first moments (M1) for loud differed most frequently from hab

Foreign accent syndrome in a patient with multiple sclerosis.

BACKGROUND: Foreign accent syndrome is a speech disorder which leads listeners to perceive the patient as having a foreign accent. It has been recognized previously after stroke, brain injury or unknown causes. CASE REPORT: A 52-year-old woman with clinically definite relapsing remitting multiple sclerosis (MS) presented with episodes of what was perceived as a Dutch accent along with other neurologic symptoms that would resolve simultaneously. She was assessed by a speech therapist both during an episode and after complete recovery. Speech and MRI changes (showing deep white matter lesions i

Speech production following partial glossectomy.

Changes in the dimensions and patterns of articulation used by three speakers to compensate for different amounts of tongue tissue excised during partial glossectomy were investigated. Place of articulation was shifted to parts of the vocal tract congruent with the speakers' surgically altered lingual morphology. Certain metrical properties of the articulatory gestures, such as width of the sibilant groove, were maintained. Intelligibility data indicated that perceptually acceptable substitute sounds could be produced by such transposed gestures.
