Strongyloides stercoralis infection in allogeneic stem cell transplant: a case report and review of the literature.

Strongyloides stercoralis infections may be documented in low-endemicity areas, particularly in immigrants from endemic areas. The case of a patient from Bangladesh, an immigrant to Italy who developed a S. stercoralis infection after allogeneic stem cell transplant, is described, and 7 further cases are reviewed. Because of the atypical clinical presentation, the low predictive role of the eosinophil count, and the low sensitivity of the microbiological tests, diagnosis of strongyloidiasis is a challenging problem. When a case of S. stercoralis infection is suspected, previous exposure may be…

Antigen recognition by IgG4 antibodies in human trichinellosis.

The antibody isotype response to Trichinella spiralis excretory/secretory (ES) products of muscle larva was examined using sera from patients with confirmed trichinellosis. Using Western blots we identify components of the ES antigen that are recognized by IgM and IgG antibodies. A 45 kDa component was strongly recognized by different antibody classes and subclasses. We observed a 45 kDa-specific IgG4 response that was detected exclusively using sera of patients with trichinellosis and not of patients with echinococcosis, filariasis, cysticercosis, ascariasis, strongyloidiasis or toxocariasis.…

[Opportunistic parasitic diseases in Africa. Clinical aspects and diagnosis].

Opportunistic parasitic infections are diagnosed more and more often in African patients with AIDS. Cryptosporidiosis is the most frequent with a prevalence between 7 and 31%. The prevalence of Isospora belli is between 5 and 19% and that of strongyloidiasis between 2 and 5%, depending on the authors. These parasitic infections are probably one of the frequent causes of chronic diarrhea in Africa. The prevalence of toxoplasmosis and pneumocystosis are not well known. (Between 5 and 17% for toxoplasmosis and 29% for pneumocystosis). The diagnostic technics used are those currently known. Toxopl…

Genetic Diversity of Strongyloides stercoralis with Attention to Clinical Features in Patients Originated from Three Endemic Provinces in the North and South of Iran.

BACKGROUND: Strongyloides stercoralis is one of the neglected tropical diseases. We aimed to verify the genetic diversity of S. stercoralis … or the first time included an isolate from a patient with hyper-infection syndrome and disseminated strongyloidiasis. CONCLUSION: The Cox1 gene showed genetic diversity for S. stercoralis isolates. Accordingly, no significant genetic difference was observed between the sequences from patients with hyper-infection and non-hyper-infection. The only isolate from a p …

Human T-cell Leukemia Virus Type 1 and Strongyloides stercoralis: Partners in Pathogenesis.

Infection with human T-cell leukemia/lymphoma virus type 1 (HTLV-1) has been associated with various clinical syndromes including co-infection with Strongyloides stercoralis, which is an intestinal parasitic nematode and the leading cause of strongyloidiasis in humans. Interestingly, HTLV-1 endemic areas coincide with regions citing high prevalence of S. stercoralis infection, making these communities optimal for elucidating the pathogenesis of co-infection and its clinical significance. HTLV-1 co-infection with S. stercoralis has been observed for decades in a number of published patient case…

Effective diagnostic tests and anthelmintic treatment for Strongyloides stercoralis make community control feasible.

Strongyloides stercoralis is endemic in tropical and subtropical countries, and is prevalent particularly in economically impoverished peopl … either infection is on its own. When people become infected with S. stercoralis, they develop acute strongyloidiasis which may be life threatening. At any time during the course of the disease, if the immune system is suppressed, most often by corticosteroid drugs, infected people may develop hyperinfective strongyloidiasis and they will die unless the underlying …

Strongyloides stercoralis: parasitological, immunological and pathological observations in immunosuppressed dogs.

Four specific pathogen-free, two to three-month-old, laboratory-raised beagles were infected with Strongyloides stercoralis and monitored by thrice-weekly faecal examinations. Larvae were not detected in the faeces after the 11th week. During the 14th week, daily oral prednisolone (2.2 mg/kg) was given. This led to weak recrudescence of the infection. During the 21st week, the daily dose of prednisolone was doubled and thereafter two of the four dogs developed severe hyperinfective strongyloidiasis with their adult worm populations markedly exceeding the number of larvae inoculated, as did one…

Fatal stongyloides hyperinfection in heart transplantation.

S stercoralis is an intestinal parasite that normally causes a chronic, low-grade, mostly asymptomatic infection. Hyperinfection syndrome and dissemination, with secondary sepsis, may occur in immunosuppressed patients, especially those on corticosteroid therapy. We present a case of fatal strongyloidiasis occurring in a patient 1 month after orthotopic heart transplantation (OHT). We assess the diagnostic challenges in these patients where pulmonary and gastrointestinal symptoms predominate as presenting clinical manifestations. We also investigate the role of screening and prophylactic anti-…

Endemic Thoracic Infections in Sub-Saharan Africa.

Although many of the thoracic infections endemic to Africa are also present around the world, this article focuses on entities that are emerging or disproportionately affect populations living in sub-Saharan Africa. Important emerging or reemerging viral and bacterial diseases that commonly affect the lung include dengue fever, plague, leptospirosis, and rickettsioses. Most parasitic infections endemic to Africa can also manifest within the thorax, including malaria, amebiasis, hydatid disease, schistosomiasis, paragonimiasis, ascariasis, strongyloidiasis and cysticercosis. Level of sanitation…

[Strongyloides hyperinfection in patient from Patagonia with HTLV-1 co-infection].

Strongylodiasis is an unattended condition caused by the parasite Strongyloides stercoralis. The Strongyloides hyperinfection syndrome can develop in immunosuppressed hosts, mainly in those with depression of cellular immunity. Co-infection with human T-cell lymphotropic virus (HTLV) is a risk factor for the development of severe forms of strongyloidiasis. We present the case of a 50-year-old man with Strongyloides hyperinfection and coinfection with HTLV. The diagnosis was delayed owing to its unusual epidemiology and an initial suspicion of inflammatory bowel disease. Identification of the p…

Strongyloides stercolaris infection mimicking a malignant tumour in a non-immunocompromised patient. Diagnosis by bronchoalveolar cytology.

Autoinfective strongyloidiasis is often fatal in immunosuppressed patients or in immunocomprised hosts. An interesting case of Strongyloides stercolaris hyperinfection was seen in an immunocompetent patient. This report describes a case of fatal strogyloidiasis in a 79 year old man, who had suffered gastrointestinal discomfort for years, and who presented because of respiratory illness. A chest radiograph showed an irregular mass close to the mediastinum and interstitial infiltrates, but blood eosinophilia was not observed. Cytological examination of the samples obtained from bronchial aspirat…

Thoracic manifestations of tropical parasitic infections: a pictorial review.

Parasitic infections are distributed worldwide and affect hundreds of millions of individuals-primarily those living in endemic areas or in regions with a high rate of immigration from endemic areas-causing significant morbidity and mortality. A broad spectrum of parasitic infections (eg, amebiasis, malaria, trypanosomiasis, ascariasis, strongyloidiasis, dirofilariasis, cystic echinococcosis, schistosomiasis, paragonimiasis) frequently affect the lungs, mediastinum, and thoracic wall, manifesting with abnormal imaging findings that often make diagnosis challenging. Although most of these infec…

Pelvic inflammatory disease complicated by massive helminthic hyperinfection.

Strongyloidiasis is a potentially life-threatening condition which, when appropriately diagnosed early in its course, is easily eradicated. When severe, it may mimic other disease processes including appendicitis or severe pelvic inflammatory disease. The diagnosis of parasitic infestation should be considered in any individual who has lived in an endemic area and presents with diarrhea regardless of attendant conditions and superimposed disease processes. A minimum workup consisting of stool examination for ova and parasites should be performed on all such individuals. Thiabendazol is the tre…

The accuracy of a recombinant antigen immunochromatographic test for the detection of Strongyloides stercoralis infection in migrants from sub-Saharan Africa.

BACKGROUND: Strongyloidiasis, a nematode infection which is mainly caused by Strongyloides stercoralis in humans, can lead to a fatal syndrome in immunocompromised individuals. Its diagnosis is challenging due to the absence of a diagnostic gold standard. The infection is highly prevalent in migrants from endemic countries in tropical and subtropical areas, and a rapid diagnostic test would be helpful for screening purposes. The aim of this study was to estimate the accuracy of a novel immunochromatographic test (ICT) for the diagnosis of S. stercoralis infection. METHODS: A single-centre dia…
