Intratendinous ganglion in the extensor digitorum communis.

We encountered a patient with intratendinous ganglion which developed in the extensor digitorum communis tendon. Although its developmental mechanism is unclear, synovitis is considered the cause. For treatment, it may be necessary to prepare for tendon transfer and tendon grafting in consideration of the risk of tendon injury.

Spontaneous bilateral quadriceps tendon rupture: a case report.

Quadriceps tendon rupture is an uncommon injury and mostly occurs among middle aged individuals that are involved in sports such as running or jumping. Spontaneous bilateral quadriceps tendon rupture is a rarer injury, however, can be debilitating. Patients with such injuries usually present with history of minor trauma, swelling and inability to actively extend the knee. Its occurrence secondary to minor trauma is mostly associated with chronic diseases and long-term use of certain medications. Occurrence of simultaneous bilateral quadriceps tendon rupture in the absence of trauma in a health

MRI overestimates the full-thickness tear of distal triceps tendon rupture.

PURPOSE: Injury to the distal triceps brachii tendon is rare. Imaging radiographs are used to confirm the findings of physical examination, classify the extent of injury, and guide treatment. Magnetic resonance imaging (MRI) is considered the gold standard of diagnostic imaging. However, no previous study has reported on the accuracy of differentiation between partial- and full-thickness triceps tendon tears. Our study's aim was to define the accuracy of MRI in differentiating partial- from full-thickness tear of the distal triceps tendon. We hypothesized that MRI has low accuracy in different

Simultaneous bilateral quadriceps tendon rupture in renal patients.

AIMS: Simultaneous bilateral quadriceps tendon rupture occurs in patients with renal disease. It has been hypothesized that the length of renal disease and the length of dialysis may correlate with the development of tendon ruptures. This study aims to characterize the factors involved in patients with renal disease and simultaneous bilateral quadriceps tendon rupture. METHODS: All reported cases of simultaneous bilateral quadriceps tendon rupture were identified using MEDLINE. Each case was reviewed for information regarding this injury. RESULTS: Of the 65 cases reviewed, 24 cases involved

Subluxation of the extensor carpi ulnaris on magnetic resonance imaging on neutral wrist position: correlation with tenosynovitis of the extensor carpi ulnaris and translation of the distal radioulnar joint.

OBJECTIVE: To identify the subluxation degrees of extensor carpi ulnaris (ECU) tendon on neutral wrist MRI in patients with ulnar-sided wrist pain and to evaluate the relationships of ECU subluxation with accompanying imaging findings such as ECU tenosynovitis, distal radioulnar joint (DRUJ) stability, and triangular fibrocartilage complex (TFCC) pathology. MATERIALS AND METHODS: A total of 297 MRIs of 292 patients having ulnar side pain were reviewed retrospectively. Degrees of ECU subluxation was classified into four grades: grade 0, no subluxation; grade 1, tendon displacement < 50% of the

Ipsilateral hamstring tendon graft reconstruction for chronic patellar tendon ruptures: surgical technique.

BACKGROUND: Patellar tendon chronic ruptures are more debated to manage that acute injuries as the patella tends to retract proximally (after 2 weeks), and surrounding soft tissues may make the repair more complicate. PURPOSE: We propose a novel technique in which the patellar tendon is reconstructed using ipsilateral hamstring tendons. This surgical procedure implies to drill a single tunnel through the patella and another through the tibia to reduce the risk of bony breakage. In addition, the tendon is secured to the bone, at the tunnel exit points, by periostium sutures without requiring a

Evidence of imbalanced adaptation between muscle and tendon in adolescent athletes.

Adolescence may be regarded as a critical phase of tissue plasticity in young growing athletes, as the adaptation process of muscle-tendon unit is affected by both environmental mechanical stimuli and maturation. The present study investigated potential imbalances of knee extensor muscle strength and patellar tendon properties in adolescent compared with middle-aged athletes featuring long-term musculotendinous adaptations. Nineteen adolescent elite volleyball athletes [(A), 15.9 ± 0.6 years] and 18 middle-aged competitively active former elite volleyball athletes [(MA), 46.9 ± 0.6 years] part

Patellar tendon versus doubled semitendinosus and gracilis tendons for anterior cruciate ligament reconstruction.

The results of intraarticular anterior cruciate ligament reconstruction with either the patellar tendon or the semitendinosus and gracilis tendons (four strands) were prospectively compared in a consecutive series of 60 patients with chronic injuries. A single surgeon performed arthroscopically assisted reconstructions in an alternating sequence. Preoperative and operative data revealed no significant differences between the two groups. After 28 months of followup there were no significant differences in the incidence of symptoms, and recurrent giving way was present in only one knee with semi

Biceps tendinitis and subluxation.

Since the 17th century, the long head of the biceps tendon as a source of shoulder pain and its functional significance has been a source of debate. Although the term tendinitis is commonly used, overuse tendon injuries infrequently demonstrate inflammatory cells; instead, degenerative changes resulting from the failure of self-repair usually are found. Bicipital tendinitis or bicipital tenosynovitis is most often secondary to impingement beneath the coracoacromical arch. Primary bicipital tendinitis and tendinitis secondary to instability are possible, however. Through a careful history, phys…

Surgical techniques/case videos-complex aortic case: top tips (TAAAR).

INTRODUCTION: Anterior cruciate ligament (ACL) injuries are among the most common orthopedic injuries. While Hamstring Tendon (HT) autograft reconstructions have been the most frequent graft type over the last decade, quadriceps tendon (QT) autografts have become increasingly popular in the past 2 years. The procedure of vancomycin graft soaking has become the state-of-the-art in many institutions as it avoids septic knee arthritis after anterior cruciate ligament reconstruction (ACLR). In the present study, we analyzed all patients presenting with an ACL rupture who subsequently underwent pri

First Extensor Compartment Tendon Rupture in the Setting of a Radial Styloid and Both-Bone Forearm Fracture: A Case Report.

CASE: We report on the rare occurrence of first extensor compartment tendon injury at the myotendinous junction after fractures of the radial styloid and both-bone forearm in a healthy 59-year-old man. Here, a novel technique for repair of this unique injury and an additional proposed mechanism of tendon rupture are discussed. CONCLUSION: We advocate for the suspicion of tendon rupture in the first dorsal compartment with fractures of the radial styloid and both-bone forearm. Tubularization and transfer of the abductor pollicis longus and extensor pollicis brevis tendons to the brachioradiali

Editorial Commentary: Is Endoscopy Really Helpful During Repair of Acute Rupture of the Achilles Tendon?

Endoscopically assisted Achilles tendon repair is an attractive minimally invasive approach to reduce the risk of sural nerve injury. However, I do not believe that endoscopy is necessary for all types of minimally invasive Achilles tendon repair. Endoscopy is only helpful to make suture passage through the proximal lateral portal safe. It cannot help to reduce the risk of sural nerve injury if the tendon is repaired with percutaneous locking sutures. Abandonment of the proximal lateral portal and development of "all-inside" endoscopic repair of the Achilles tendon with locking sutures should

Endoscopic Approach to Proximal Hamstring Avulsion Repair.

BACKGROUND: Endoscopic repair of a proximal hamstring avulsion promotes precise anatomical repair and lowers the risk of neurovascular injury. DESCRIPTION: Indications for proximal endoscopic repair of the proximal part of the hamstrings include acute tears of 2 tendons with >2 cm of retraction in young active patients, acute complete tears of 3 tendons with >2 cm of retraction, or failed conservative treatment of tears of ≥2 tendons with ≤2 cm of retraction. Repair of a proximal hamstring avulsion is performed using 2 portals. The medial portal is developed percutaneously under fluoroscopic

Tropical Achilles Tendinopathy: Sea Urchin Spine Injury.

Sea urchin spine injuries have been reported in the hand and foot, but there are no published cases in the Achilles tendon. We report an unusual case of Achilles tendinopathy secondary to sea urchin spine injury. The patient had Achilles tendon pain that increased over time and was worsened with weightbearing activity. His left ankle plantarflexion was limited by pain. He had received medical care 3 months earlier to remove sea urchin spines after stepping on a long-spined sea urchin. Bedside ultrasound and imaging studies revealed that there were foreign bodies related to sea urchin spines on

MR imaging assessment of the pectoralis major myotendinous unit: an MR imaging-anatomic correlative study with surgical correlation.

OBJECTIVE: MR imaging is the optimal imaging technique to study the normal and abnormal conditions of the pectoralis major muscle and tendon unit. The purpose of this study was to use MR imaging to provide an anatomic survey of the normal pectoralis major tendon and its insertion and to compare these findings with surgically proven cases of rupture. CONCLUSION: MR imaging shows the normal pectoralis major myotendinous unit has low signal intensity on both T1- and T2-weighted images. Reliable anatomic landmarks for visualization and examination of injuries to the muscle and myotendinous unit i

[The rupture of the tibial anterior tendon in a world class veteran fencer].

The rupture of the tendon of the M. tibialis ant. is a rare sport injury and has not been yet described in fencing. The lunge, the most common offensive movement in fencing, displays a high stress on the spanned tendons and ligaments over the ankle joint and led to the rupture of the pre-damaged tendon of our patient. Pain over the inner side of the foot should be assessed as knells of this tendon injury. The exact patient's history and the precise clinical investigation are adequate for diagnosing the rupture. Hereby, an absence of the tendon shape over the ankle joint and a distinct active d

Personalized tendon loading reduces muscle-tendon imbalances in male adolescent elite athletes.

An imbalanced adaptation of muscle strength and tendon stiffness in response to training may increase tendon strain (i.e., the mechanical demand on the tendon) and consequently tendon injury risk. This study investigated if personalized tendon loading inducing tendon strain within the effective range for adaptation (4.5%-6.5%) can reduce musculotendinous imbalances in male adolescent handball athletes (15-16 years). At four measurement time points during a competitive season, we assessed knee extensor muscle strength and patellar tendon mechanical properties using dynamometry and ultrasonograp

Collagen V deficiency during murine tendon healing results in distinct healing outcomes based on knockdown severity.

Tendon function is dependent on proper organization and maintenance of the collagen I tissue matrix. Collagen V is a critical regulator of collagen I fibrils, and while prior studies have shown a negative impact of collagen V deficiency on tendon healing outcomes, these studies are confounded by collagen V deficiency through tendon development. The specific role of collagen V in regulating healing tendon properties is therefore unknown. By using inducible Col5a1 knockdown models and analyzing gene expression, fibril and histological tendon morphology, and tendon mechanical properties, this stu

Extensor pollicis longus tendon rupture from Dorsal Nail Plate distal radius fixation with concomitant myostatic atrophy.

Distal radius fractures are one of the most common upper extremity injuries seen by hand surgeons each year. Many of these fractures require reduction and surgical fixation because of displacement, comminution or inherent fracture instability. New hardware is designed and introduced each year to help surgeons manage these injuries. We report a case of a major complication from the use of the Dorsal Nail Plate hardware. The patient presented to our clinic with an extensor pollicis longus attrition rupture, which required tendon transfer for treatment. Intraoperatively, we also found ongoing ext

Clinics in diagnostic imaging. 148. Bursal-sided partial-thickness supraspinatus tendon tear.

Rotator cuff injury comprises a continual spectrum of lesions ranging from tendinopathy, which may progress to partial- or full-thickness tear. This progression may be influenced by the interplay of extrinsic and intrinsic factors. We describe the case of a 29-year-man who presented with right shoulder pain for one year. His initial magnetic resonance (MR) arthrogram showed supraspinatus tendinopathy. Subsequent MR arthrogram, obtained after a road traffic accident, showed a bursal-sided partial-thickness tear of the supraspinatus tendon. In view of his worsening clinical symptoms, surgery was
