Reconstruction and repair of atraumatic extensor tendon ruptures in rheumatoid wrists: Less extension lag after direct repair than interposition tendon grafting.

The purpose of this study was to investigate the outcomes of extensor tendon repair involving the original stump in atraumatic extensor tend … iginal extensor tendon stump yields satisfactory outcomes and appears to be a viable alternative to tendon transfers in patients with rheumatoid wrists with atraumatic extensor tendon ruptures. Direct repair reduces postoperative extension lag without a significant difference in flexion deficit when compared with interposition tendon grafting.

Vascularized free flap transfer in the upper limb.

Free tissue transfer is quite an important and attractive procedure for reconstruction of an injured limb, especially an upper limb. A simple skin flap can be transferred to close an open wound of the limb, but a sensory flap or a special flap such as a nail-skin, tendon-skin, or muscle-skin flap is a much better donor for reconstructing the function of the upper limb. The muscle or muscle-skin transfer procedure should be highly valued as a means of restoration of motor function in those patients in whom all other measures have totally failed in the past. Sensory flap transfer from the foot i

A new sign allowing diagnosis in the pathologies of the extensor tendons of the hand.

INTRODUCTION: The thorough knowledge of the anatomy of the extensor tendon system of the hand is crucial for clinical examination and detection of tendon injuries. The Juncturae between the EDCII and other extensor tendons presents different incidences, shapes ("r" or "y") and morphologies (Type I, II, III). The EIP does not receive connection. These characteristics may result in variable effects on extensor tendons during active finger movements. The purpose of this study is to investigate and describe a new sign helping diagnosis for injuries or pathologies of EIP and EDC index finger (EDC I

Recurrent dislocation of the elbow in a child: case report and review of the literature.

Recurrent dislocation of the elbow is extremely rare in children. Surgical correction must be aimed at the specific pathology present in each case. Transfer of the biceps tendon and a central slip of the triceps were successful in an 8-year-old boy who had sustained six dislocations in 2 years.

[Modification of the Furnas-Vilkki technic in the reconstruction of congenital or traumatic carpal hands].

A technique is described for reconstruction of a pincer, by a second toe transfer, in traumatic and congenital deformities, leaving only the wrist. Transfer on the anterior aspect of the radius allows to benefit from the wrist mobility to compensate for the limited range of motion of the second toe. Proximal situation of the toe gives the possibility of harvesting plenty of tendons to balance the toe. Results have been encouraging in two traumatic and 6 congenital cases of peromelic type of symbrachydactyly.

[Experimental study on the effects of tension-reduced early mobilization on extensor tendon healing].

Numerous clinical and experimental studies have been reported on early postoperative mobilization after flexor tendon repair. However, there have been only a few experimental studies reported on extensor tendon repair. In 1989, Ishiguro reported the usefulness of tension-reduced early mobilization in clinical cases of extensor tendon ruptures. The purpose of this experimental study was to examine the in vivo effect of tension-reduced early mobilization on extensor tendon healing in chicken. An experiment was performed on the extensor tendons of 218 chickens in an attempt to examine the effects

Assessing the Accuracy of Common Clinical Tests for Detecting Palmaris Longus: A Prospective Study in a Tertiary Center.

Background  Palmaris longus (PL) is a short-bellied muscle with a long tendon that merges with the palmar aponeurosis. It is supplied by the median nerve and acts as a tensor of the palmar aponeurosis and flexes the wrist. This tendon is commonly used for tendon transfers and as a donor for tendon graft. There are numerous clinical tests to detect the presence of PL like Schaeffer's test, Thompson's test, Mishra's test I, Mishra's test II, Pushpakumar's "two-finger sign" method, and AIIMS test. The principle of all these tests is to make the tendon prominent by eliciting its flexor action and …

Modified Boyes' procedure for the multiple finger extensor tendon ruptures in patients with rheumatoid arthritis: A report of two cases.

Multiple subcutaneous extensor tendon ruptures in more than the ulnar three fingers sometimes occur in patients with rheumatoid arthritis accompanied by wrist deformity. In these situations, the flexor digitorum superficialis tendon of the middle finger (the FDS3 tendon) and that of the ring finger (the FDS4 tendon) are used for the transferred tendon (modified Boyes' procedure). Here, we treated two patients with rheumatoid arthritis, whose extensor tendons of more than three fingers were ruptured, using the modified Boyes' procedure. Case 1 had ruptures in four fingers (index through little)

A simple technique for correcting footdrop: suspension tenodesis of the tibialis anterior tendon to the distal tibia.

Surgical correction of the footdrop deformity caused by various neuromuscular diseases is a time-honored technique, with most surgical procedures intended to correct the absence of active dorsiflexion and a concomitant cavovarus foot deformity. We describe suspension tenodesis of the tibialis anterior tendon to the distal tibia to correct the footdrop deformity. This technique is simpler to perform and more secure than transferring the distal attachment of a tendon to a new location in the foot itself or using tendon weaving techniques. In addition, compared with an ankle fusion, our technique

The importance of middle or long finger realignment in ulnar drift.

Based upon the hypothesis that ulnar drift is the result of a chain reaction starting with a medial shift of the proximal phalanx at the metacarpophalangeal joint and progessing to subluxation of the extensor tendon and finally subluxation of the metacarpophalangeal joint, the author recommends early operative realignment. The extensor tendon is partially divided and a portion passed through the base of the phalanx, the intrinsic tendons are transferred to divided, the junctura tendnium is served, the periosteum is stripped from the neck of the metacarpal, and areas of bony erosions are curett

Clinical outcome comparison of suture button suspensionplasty versus trapeziectomy with ligament reconstruction and tendon interposition in patients with carpometacarpal joint arthritis: a systematic review of direct comparison studies.

INTRODUCTION: Trapeziectomy nowadays is predominantly done with Ligament Reconstruction and Tendon Interposition (LRTI) or Suture Button Suspensionplasty (SBS). This article is intended to highlight comparison between SBS and trapeziectomy with LRTI through direct comparison evidence. MATERIAL AND METHOD: The LRTI technique involves the removal of the diseased trapezium bone and creating suspension through the use of flexor carpi radialis tendon, while SBS stabilize the metacarpal base by suspending the first metacarpal bone. The record screening process of this systematic and meta-analysis w

Massive rotator cuff tears: arthroscopy to arthroplasty.

The understanding of rotator cuff disease has increased exponentially since Codman drew attention to this pathology in the early 1900s. Although challenging, the surgical treatment of massive rotator cuff tears is rational, with treatment decisions based on physical examination, imaging, biologic, and patient factors. Arthroscopy can be used to treat ancillary pain generators, débride necrotic tissue, and possibly restore balance to the force couples about the shoulder. Tendon transfers may be effective in restoring functional strength to irreparable, ineffectual muscle units. Arthroplasty is …
