Outer Retinal Changes Preceding Secondary Macular Hole Formation Years After Vitreomacular Traction Release Demonstrated on Spectral-Domain OCT.

This report describes changes documented on spectral-domain optical coherence tomography (SD-OCT) in the right eye of a 58-year-old woman who eventually developed a secondary full-thickness macular hole (FTMH). She had a history of vitreomacular traction (VMT) that had resolved spontaneously with a complete posterior vitreous detachment. Despite VMT resolution, a hyperreflective band from the internal to the external limiting membrane and evidence of photoreceptor disruption persisted on SD-OCT for 2 years, at which point an FTMH developed. Despite VMT release, the unexpected FTMH coupled with…

Flexible intramedullary nails with traction versus plaster cast for treating femoral shaft fractures in children: comparative retrospective study.

CONTEXT AND OBJECTIVE Femoral fractures are common in children between 2 and 12 years of age, and 75% of the lesions affect the femoral shaft. Traction followed by a plaster cast is universally accepted as conservative treatment. However, in some situations, a surgical approach is recommended. The objective here was to compare treatments for femoral shaft fractures using intramedullary nails (titanium elastic nails, TEN) versus traction and plaster casts in children. The hypothesis was that TEN might provide better treatment, with good clinical results in comparison with plaster casts. DESIGN …

Not so fast to skin graft: transabdominal wall traction closes most "domain loss" abdomens in the acute setting.

BACKGROUND: Damage-control laparotomy (DCL) has revolutionized the surgery of injury. However, this has led to the dilemma of the nonclosable abdomen. Subsequently, there exists a subgroup of patients who after resuscitation and diuresis, remain nonclosable. Before the adoption of our open abdomen protocol (OAP) and use of transabdominal wall traction (TAWT), these patients required skin grafting and a planned ventral hernia. We hypothesize that our OAP and TAWT device, which use full abdominal wall thickness sutures to dynamically distribute midline traction, achieve an improved method of fas…

One-year follow-up after open abdomen therapy with vacuum-assisted wound closure and mesh-mediated fascial traction.

BACKGROUND: Open abdomen (OA) therapy frequently results in a giant planned ventral hernia. Vacuum-assisted wound closure and mesh-mediated fascial traction (VAWCM) enables delayed primary fascial closure in most patients, even after prolonged OA treatment. Our aim was to study the incidence of hernia and abdominal wall discomfort 1 year after abdominal closure. METHODS: A prospective multicenter cohort study of 111 patients undergoing OA/VAWCM was performed during 2006-2009. Surviving patients underwent clinical examination, computed tomography (CT), and chart review at 1 year. Incisional an…

Flare-up of previously quiescent chronic osteomyelitis 20 years after childhood skeletal traction: a report of two cases.

Management of lower limb fractures in children involves many approaches, spanning from conservative treatment to open reduction and internal fixation. A number of intermediate treatments have also been shown to be effective, notably skeletal traction. However, each of these techniques has its own advantages and disadvantages in terms of complications. In this report, we describe two new cases of chronic osteomyelitis that manifested (many years) after childhood skeletal traction. After analysing these two cases, we proposed measures that can be implemented to avoid such complications, along wi…

Radiographic disk height increase after a trial of multimodal spine rehabilitation and vibration traction: a retrospective case series.

OBJECTIVE: Although spinal decompression therapy has been touted as an effective treatment of disk pathologies, there is little existing research that specifically uses disk parameters as an outcome measure after a course of spinal decompression therapy. Our study presents multidimensional outcomes after a structured protocol of multimodal chiropractic rehabilitation and uses a radiographic parameter of disk disease as an indication of the effects of a vibration traction decompression-type table. CLINICAL FEATURES: Patients selected for this retrospective cohort reported a medical history of …

STUDY DESIGN: Clinical case report of atlantoaxial rotatory fixation (AARF) in a girl presenting with torticollis and neck pain. OBJECTIVE: To report this rare case that was successfully treated with long-term traction and a brace. SUMMARY OF BACKGROUND DATA: AARF is a rare kind of subluxation that is a pathologic fixation of the atlas on the axis. It is most common in pediatric patients and is usually reduced easily with conservative treatments only in the acute stage. However, previously reported chronic AARFs have usually been treated with operative reductions. Although high success rates…

How to avoid and manage nerve injuries associated with aortic surgery: ischemic neuropathy, traction injuries, and sexual derangements.

Both open and endovascular surgery of the infrarenal aorta are attended by risks of neurologic complications. Injury to the periaortic autonomic plexi frequently results in ejaculatory and erectile dysfunction. Traction injuries to lumbosacral nerve roots can cause peripheral nerve injury, most commonly exhibited as a femoral nerve deficit. The least common but most feared neurologic complication that can occur with infrarenal aortic surgery is ischemic injury to the spinal cord, or conus medullaris. The risk of this complication is increased with emergent or complicated aortic reconstructions…

Scanning electron microscopy comparing exclamation mark hairs in alopecia areata with normal hair fibres, mechanically broken by traction.

A scanning electron microscope was used to compare the distal ends of exclamation mark hairs from alopecia areata patients with the fracture surface of normal hair fibres, mechanically broken by traction. The exclamation mark hairs shows minimal damage to the cuticular cell pattern of the hair shaft. Where the cuticle was absent, cortex and medulla showed low-density features fanning out into a fringe-like structure at the distal ends. The fracture surfaces of normal hair fibres, mechanically broken by traction showed a markedly abnormal cuticular scale pattern, whereas the features of cortica…

Impact of Operative Position on Outcomes of Proximal Femoral Nailing: A Comparison Between Supine on a Traction Table and Lateral Decubitus.

Introduction Unstable intertrochanteric fractures are common in older adults and are best stabilised with cephalomedullary nails. Surgeons favour either a supine position on a traction table or a lateral decubitus position on a standard table, yet evidence comparing their clinical value is limited and often conflicting. The traction table may simplify reduction and imaging but introduces set-up delays and post-related complications; lateral positioning avoids the post but relies on manual traction. This study aims to determine whether patient position meaningfully impacts intraoperative, radio…

Is there an added benefit of intraoperative traction to posterior spinal fusion for adolescent idiopathic scoliosis? A systematic review and meta-analysis.

BACKGROUND: Skeletal intraoperative traction (IOT) has been proposed as an adjunct to posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS) to facilitate deformity correction and reduce perioperative morbidity. However, its clinical benefit remains controversial, with conflicting evidence on radiographic outcomes and safety. This systematic review and meta-analysis aimed to determine whether skeletal IOT provides added benefit in patients undergoing one-stage PSF for AIS. METHODS: A comprehensive search of PubMed, Embase, Scopus, Cochrane Library, and Web of Science was perf…

Effect of traction force during surgery on physical integrity and histological changes in peripheral nerves: experimental study on rabbits.

Sensory and motor nerve damage is a common complication of maxillofacial surgery and trauma. Procedures such as orthognathic surgery, tumor resection, and salivary gland interventions can damage peripheral nerves when the surrounding soft tissue or the nerve itself is manipulated. The purpose of this study was to evaluate the histological changes in the sciatic and median nerves of albino rabbits following traction-induced nerve injury. Nine albino rabbits were included in the study and divided equally into three groups, with three rabbits per group. In each rabbit, four peripheral nerves were…

Axial Spinal Traction as a Potential Modulator of Cerebrospinal and Glymphatic Circulation in Neurodegenerative Diseases: A Technical Report and Biomechanical Hypothesis.

Impairment of glymphatic function contributes to the accumulation of metabolic and proteinaceous waste products implicated in neurodegenerative diseases such as Alzheimer's disease, frontotemporal dementia, Parkinson's disease, amyotrophic lateral sclerosis, Huntington's disease, and certain spinocerebellar ataxias. Pelvis-stabilized axial spinal traction (PSAST) is a biomechanical technique designed to produce brief, controlled cranio-caudal elongation of the vertebral column and spinal dural sac, potentially generating transient pressure gradients capable of influencing cerebrospinal fluid (…
