Hepatocyte transplantation.

Repopulation of the liver with transplanted cells holds significant promise for developing novel therapies. The liver is a most suitable target for treating a variety of genetic, metabolic and acquired diseases. Liver disease, such as chronic viral hepatitis, constitutes an enormous burden worldwide. Advancing liver cell therapy requires insights into mechanisms of cell engraftment and proliferation, as well as unique requirements of specific diseases for correction by cell transplantation. This review highlights recent developments in the area of hepatocyte transplantation. Aspects concerning

[Heart transplantation].

Since February 1999, organ transplantation from brain-dead donors has started in Japan. During the more than 3 years since, 77 patients have received organ transplantations from 19 donors (heart in 13, lung in 10, liver in 17, pancreas-kidney in 6, pancreas alone in 1, small intestine in 1, and kidney in 29). As of April 2002, 116 patients were registered with the Japan Organ Transplant Network as heart transplant (HTx) candidates, and 13 and 8 underwent HTx in Japan and abroad, respectively, while 32 died while on the waiting list. Ten of 13 HTx recipients were implanted with a left ventricul

Fat transplantation.

Fat transplantation has been used by physicians for over 100 years; however, this technique has been refined considerably since the development of liposuction in the late 1970s. Fat transplantation can be used for augmentation of subcutaneous defects throughout the body. It continues to be the safest and most versatile form of soft tissue augmentation.

Pediatric transplantation.

Advances in organ preservation, surgical technique, and postoperative care have permitted the rapid development of liver transplantation in children. Consequently, the applicability of this procedure has gone beyond the treatment of life-threatening complications of chronic liver disease and now includes disabling morbidities and quality-of-life issues. The use of hepatic segments for transplantation with reduced or split cadaveric grafts and living-related donors has decreased the mortality of children awaiting liver transplantation. We are presently armed with a new potent immunosuppressive

Pancreas transplantation.

Although intensified insulin therapy regimens enable normalization of blood glucose levels and related metabolic parameters, these regimens are associated with an increased incidence of hypoglycemic episodes. Pancreas transplantation has achieved the goal of providing insulin independence with stable and continuous normoglycemia. But because of the associated morbidity and mortality and the need for life-long immunosuppression after transplant, it is difficult to justify pancreas transplantation in diabetic patients at a pre-uremic stage. Pancreas transplantation is therefore performed in conj

Transplant risks.

1. Changes in serum creatinine is a potentially useful predictor of chronic rejection. Patients with 2 10% increases in creatinine values in 3 consecutive years between 1-5 years had 4 times the risk of chronic rejection graft loss than patients with stable creatinine. 2. Formation of HLA antibody may correlate with graft rejection since losing a kidney increased the risk of broad sensitization 5-fold and losing multiple kidneys increased the risk ten-fold. 3. Sensitization increased the risk of acute and chronic rejection while pregnancies decreased the risk of acute and chronic rejection sug

Intestinal transplantation.

Intestinal transplantation is often the only alternative form of treatment for patients dependent on total parenteral nutrition for survival. Although a limited number of intestinal transplantations have been performed, results with FK 506 immunosuppression are comparable to those for other organ transplants. The impact of successful intestinal transplantation on gastroenterology will likely be similar to the impact of kidney and liver transplantation on nephrology and hepatology.

[Lung transplantation].

As alternative methods of lung transplantation are available, in terminal respiratory or combined cardio-respiratory failure, the surgical procedure can be chosen to suit the needs of the individual recipient in accordance with the philosophy of the transplant centre. At one Scandinavian centre the preferred procedure has been en bloc double lung transplantation with bronchial artery revascularisation, whereas at another centre the preference is for single lung transplantation if considered suitable in view of the pathology resulting in respiratory failure. Short-term results compare favourabl

Liver transplantation.

From more than 900 articles in the medical literature, published between August 1, 1991 and July 31, 1992, 46 were selected for review. During the year of review, major international symposiums were held on a new drug, FK506, and intestinal transplantation. In addition, at least one new attempt at a unifying hypothesis regarding graft adaptation was proposed. Other studies included living related donor transplantation, results with transplantation for alcoholics, and the controversy regarding the role of liver transplantation for the treatment of Budd-Chiari syndrome. Concerns regarding the re

Heart transplantation.

From September 1988 until March 1990, 22 orthotopic heart transplantations (HTX) were performed in 20 patients (18 male and 2 female). Median age was 56.5 years (23-66). The indication for HTX was an end-stage ischemic disease in 7 pts. a dilated cardiomyopathy in 13 pts, and a retransplantation in 2 pts. The mean waiting time was 58 days. Immunosuppressive therapy included OKT3, prednisone and azathioprine. Cyclosporine was introduced at day 10. Donor hearts were obtained from our institution in 5 cases, from other hospitals in Belgium in 9 cases, and from other European countries in 8 cases.

Transplantation of the lung.

The introduction of cyclosporine as a highly effective immunosuppressive agent and the development of new techniques for heart-lung and lung transplantation have led to a new treatment for a wide range of fatal cardiopulmonary diseases. Indications for surgery are now becoming clear, together with major contra-indications. Suppurative lung disease, such as cystic fibrosis, can be effectively treated by heart-lung transplant (HLT). A whole new field of pulmonary medicine is emerging to provide the physiological monitoring and diagnostic techniques for major complications such as opportunistic l

Cardiac transplantation.

With the advent of cyclosporine and triple-drug therapy, cardiac transplantation has developed from a curiosity into an effective therapy. Optimal results require proper recipient selection, careful donor identification and preparation, and skillful surgical and postoperative care. Recent trends to expand the donor and recipient pools will result in an amplification of the problems encountered to date. Causes of significant perioperative morbidity and mortality continue to be graft failure, infection, and rejection. Careful attention to elevated recipient pulmonary vascular resistance and dono

[Pancreas transplant].

Pancreas transplantation offers the possibility of preventing the development and progression of diabetic lesions by adequate control of hydrocarbon metabolism. Moreover, the diabetic patient is freed from dietary and physical restrictions, as well as from insulin treatment. However, this is achieved at the expense of an immunosuppression not always free of risk. For this reason, up to now, the transplantation has been reserved to those patients at a risk higher than that of immunosuppression: uremic patients who also require a kidney transplantation, patients suffering of preproliferative ret

Cardiac transplantation.

Cardiac transplantation is now an accepted therapeutic option for patients with end-stage myocardial failure. Provided donor and recipient are appropriately selected and adequately matched, expected survival rates at one and five years are 85% and 65%, respectively. Two major challenges are encountered in clinical heart transplantation. The first is monitoring immunosuppression for adequate prevention of acute rejection and surveillance for side effects. The endomyocardial biopsy remains the gold standard for rejection surveillance, but since it is an invasive procedure which can only be perfo

Cardiac transplantation.

Cardiac transplantation is now an accepted therapeutic procedure in the management of patients suffering from end-stage congestive heart failure. The advances in myocardial preservation, long-distance procurement, immunosuppression, improvement in the treatment of infectious diseases, and utilization of endomyocardial biopsy in the diagnosis of rejection have made its application widespread and resulted in improved survival.

Cardiac transplantation.

Cardiac transplantation has emerged as an acceptable treatment for patients with end-stage cardiac disease. As a result, it is important for the critical care nurse to understand the principles associated with this procedure and to anticipate complications for this population. This article has reviewed the historical evolution of cardiac transplantation, identified recipient and donor selection criteria, and described the operative technique. Principles of immunosuppression are addressed as compulsory knowledge for the nurse caring for these patients and common immunosuppression agents are del
