Intraosseous Malignant Peripheral Nerve Sheath Tumor of 2 Consecutive Lumbar Vertebrae: A Case Report and Literature Review.

BACKGROUND: Malignant peripheral nerve sheath tumor (MPNST), which refers to any malignant tumor that originates from peripheral nerve sheath cells, is rarely found in the spine. Primary intraosseous spinal MPNSTs are extremely uncommon; in fact, such an occurrence has been reported in a very limited number of cases hitherto. We report here one case of low-grade intraosseous MPNST in the lumbar vertebrae and review clinical research related to this rare tumor and modus operandi to its treatment. CASE DESCRIPTION: A 51-year-old man suffering from lower back pain was initially diagnosed with lu

[A case of tuberculous myeloradiculitis with abdominal lymphadenitis presenting with symptoms of radiculomyelopathy].

A 30-year old man was admitted with right hip pain and gait disturbances. Neurological findings revealed muscular weakness in the lower limbs, hyporeflexia, dysesthesia in the sacral region, and bowel and bladder disturbances. Cerebrospinal fluid (CSF) examination indicated a white blood cell count of 371/μl (lymphocyte:polymorphonuclear leukocyte = 97:3), protein levels of 463 mg/dl and sugar of 20 mg/dl. Although CSF culture was negative, tuberculous infection was presumed. Magnetic resonance imaging revealed areas of enhancement in the intramedullary region surrounding the spinal cord and c

[Tuberculosis of bones and joints].

In developing countries tuberculosis of the bones and joints is a disease with an endemic character. In industrialized nations it is also gaining importance again as a result of social problems and of cases of immune deficiency. After the bacteriologic/histologic diagnosis of tuberculosis of the bones and joints, the primary therapy is non-operative with immobilization and antituberculous chemotherapy. Nevertheless, there are indications when an abscess and arthrodesis or spondylodesis must be cleaned operatively. An absolute and urgent indication for operative cleansing and stabilization is a

Beyond Kyphosis: A Case of Tuberculous Spondylodiscitis With L3 Vertebral Dislocation.

Spinal tuberculosis (TB) accounts for the majority of skeletal TB cases and remains a significant cause of morbidity in developing nations. While thoracic involvement is most common, lumbar vertebral dislocation represents an exceedingly rare and severe complication. This case involved a 22-year-old female who presented with a six-month history of severe back pain followed by the sudden onset of complete paraplegia with bowel and bladder incontinence. Despite initiation of antitubercular therapy (ATT) at another facility, her condition progressively worsened. Neurological examination revealed

Tuberculous spondylitis in adults.

We treated 107 adults with spinal tuberculosis. The average age was 41.8 years (range, sixteen to seventy-five years). Diagnosis was difficult: bone scans were negative in 35 per cent; gallium scans, negative in 70 per cent; and results of tuberculin skin tests, negative in 14 per cent. Five neurologically impaired patients had no discernible bone lesions when they were first seen but were found to have either intradural or extradural tuberculomas or tuberculous arachnoiditis. Our indications for a spinal operation were neurological impairment, spinal instability, or failure of medical managem

Typhoid spondylodiscitis mimicking tuberculosis in a teenage girl.

Salmonella Typhi cause a broad spectrum of human illnesses like gastroenteritis, typhoid fever, and bacteremia. It has also been recognized as a causative organism of osteomyelitis for more than a century but the incidence appears to be uncommon. Microbiological workup plays important role in the diagnosis of Typhoid spondylodiscitis as most of the time it mimics tuberculosis and misguide the clinician, especially in the developing world. Here, we reported an uncommon case of lumbar spondylodiscitis by Salmonella Typhi in an immunocompetent teenager, with the help of clinical, microbiological

Fibrosing mediastinitis as a cause of right claudication.

Fibrosing mediastinitis is a rare benign disorder caused by proliferation of acellular collagen and dense paucicellular fibrous tissue within the mediastinum. Its precise cause, pathogenesis and links to infectious (such as histoplasmosis or tuberculosis) and non-infectious (such as sclerosing cholangitis) diseases remain speculative. Affected patients present signs and symptoms related to obstruction of mediastinal hollow organs, such as large vessels, esophagus and airways. The present study reports the first case described in medical literature of medullar compression by a mediastinal mass

Tuberculosis: an uncommon cause of cerebral venous thrombosis?

Several infectious etiologies are related to cerebral venous thrombosis (CVT), but a review of literature showed only few cases related to tuberculosis (TB), and only one with neurological manifestations. We report an unusual case of CVT related to TB and mutation in prothrombin gene. A 38-man black presented abrupt right hemiparestesis, and hemiparesis. Investigations revealed CVT. Cerebral spinal fluid (CSF) examination evidenced an infection by Mycobacterium. He was heterozygous for G20210A prothrombin mutation. Probably, hypercoagulability mechanisms of TB, added to mutation of prothrombin

Salmonella osteomyelitis. An important differential diagnosis of granulomatous osteomyelitis.

A case of septicemia owing to Salmonella choleraesuis with localization in the lumbar spine and left knee is described. The spinal lesion is dominated by tuberculoid granulomas with or without central necrosis. The necrotic foci within some granulomas show heavy polymorph infiltration, whereas in others they simulate caseous necrosis and are indistinguishable histologically from tuberculosis, brucellosis, and fungal infections. As Salmonella osteomyelitis has a strong tendency to chronicity if antimicrobial treatment is delayed, inappropriate, or inadequate, this diagnosis should be considered

The radical treatment of tuberculosis of the spine.

The technique of radical treatment of tuberculosis of the spine at various levels is described. Eighty cases out of 158 of spinal caries underwent radical surgery over a period of 7 years. Caseous pus, sequestra, granulation tissue and discs were excised. Rib grafts were used in the dorsal spine and iliac strut grafts in the cervical and lumbar spine, through a direct anterior approach. Sound bony fusion was achieved in 90% of cases. The procedure is technically difficult but recovery is fast and the majority were fit to resume work within 6 months.

[Spondilodyscitis: five years of experience in a department of rehabilitation].

INTRODUCTION: Spondylodiscitis is a rare cause of spinal cord lesion. Although this kind of infection is known to have a low incidence; it reflects cases of major clinical interest, since early diagnosis and treatment are determinant reducing morbi-mortality and in improving functional prognosis. OBJECTIVES: To analyse demographic and clinical data; the diagnostic investigations, the rehabilitation programme and the outcome in hospitalised patients. METHODS: The authors performed a retrospective, descriptive study. They reviewed the medical records of all hospitalised patients in Centro Hosp

The MEG3/miR-133b regulatory axis in exosomes influences macrophage responses to drug-resistant mycobacterium tuberculosis.

Drug-resistant tuberculosis remains a major global health challenge, with spinal involvement causing substantial morbidity. Exosomes and non-coding RNAs (lncRNAs and miRNAs) are increasingly recognized as mediators of intercellular communication and immune modulation during mycobacterial infection, yet their specific roles in drug-resistant contexts are incompletely understood. To investigate the potential interaction between exosomal lncRNA MEG3 and miR-133b and its influence on macrophage proliferation and apoptosis in an in vitro model of drug-resistant Mycobacterium tuberculosis (MTB) infe
