[Gynecological abnormalities in patients with rectovestibular fistula].

Patients with recto vestibular fistula may have gynecological malformations that could be unnoticed at the initial examination. The aim of this paper is to demonstrate the incidence of these malformations and propose a study method to help diagnose these malformations, avoiding unnecessary surgeries. We reviewed the records of patients treated with rectovaginal fistula (RVF) in the last 18 years and studied their gynecological malformations, time at diagnosis and treatment received. Of the 39 patients treated, 5 of them (13.1%) demonstrated 9 gynecological malformations: Hemivaginas (2), hemiu

Asymptomatic endometrial thickening.

OBJECTIVE: To formulate clinical recommendations for the assessment of endometrial thickening when it is found on ultrasound in a postmenopausal patient without bleeding. OUTCOMES: Ensure that women with asymptomatic thickening and endometrial polyps found on ultrasound are managed appropriately. EVIDENCE: Published literature was retrieved through searches of English language articles from the EMBASE, Cochrane, and PubMed databases for relevant peer-reviewed articles dating from 1970 to 2009, using appropriate controlled vocabulary (e.g., "asymptomatic endometrial thickness," "endometrial c

The management of nonpalpable testis with combined groin exploration and subsequent transinguinal laparoscopy.

PURPOSE: About 20% of all cryptorchid testes are nonpalpable. Although surgical exploration was previously the method of choice for management, laparoscopy from the inferior edge of the umbilicus has been established as a useful method of finding a testis that is nonpalpable. However, conventional subumbilical laparoscopy is unnecessary when the testis or its remnant is located below the internal inguinal ring. We evaluated the efficacy of transinguinal laparoscopy after inguinal exploration for a nonpalpable testis. MATERIALS AND METHODS: While 30 patients each had a unilateral nonpalpable a

Fibromatosis colli - a rare cytological diagnosis in infantile neck swellings.

Fibromatosis colli or sternocleidomastoid tumour is a rare cause of benign neck mass in infants. It is a self limiting fibroblastic lesion usually presenting with torticollis and a history of birth trauma. It is one of the few causes in which Fine Needle Aspiration Cytology (FNAC) is indicated in a neonate to confirm the diagnosis and to differentiate it from other congenital, inflammatory and neoplastic causes. FNAC provides a rapid, cost-effective, reliable, non invasive method of diagnosis resulting in conservative management of these lesions. We present two interesting cases of neck swellin

[Lymphatic mapping and inguinal sentinel lymph node biopsy in anal canal cancers to avoid prophylactic inguinal irradiation].

Thirty-five patients with clinically N0 cancers of the canal anal, 33 epidermoid carcinomas et 2 melanomas were histologically staged with inguinal sentinel lymph node biopsy (ISN). With the combined technique, blue dye and radiocolloid the ISN was identified in 100% of the cases. The ISN was invaded in 7 cases/33 for epidermoid tumors and 2/2 for melanomas. After 18 months of follow-up, no inguinal recurrence could be seen in ISN pN0 cases. In conclusion, ISN biopsy is a reliable procedure to stage anal canal cancers. It should prevent unnecessary prophylactic inguinal irradiation for pN0 ISN

Compliance with colorectal cancer screening in a high-risk occupational group.

A Detroit-area union offers an early detection colorectal cancer screening program consistent with that recommended by the American Cancer Society. Analyses compared men who have complied with the American Cancer Society recommendations with men who have not so complied. Older men, nonsmokers, and woodworkers were most likely to participate in all screening examinations. Primary reasons for nonparticipation were the belief that the examination was unnecessary, anxiety/affective concerns; and time/motivation problems. Particularly important to the success of such programs is a sensitive health

Application and recent advances in conventional biomarkers for the prognosis of papillary thyroid carcinoma.

Cancer remains one of the most common and deadliest diseases worldwide. Among endocrine neoplasms, the incidence of thyroid malignancies has been rising in recent years. Papillary thyroid carcinoma (PTC), the most frequently observed histological subtype of thyroid cancer (THCA), typically yields favorable clinical outcomes for affected individuals. However, this has raised concerns about the overdiagnosis and underdetermination of the prognostic factors in PTC cases. As a result, researchers now advocate for patient stratification and tailored therapeutic approaches for PTC cases, with the go

Hospital Participation in Federated Learning: Evaluating Sustainability and Clinical Utility.

Prostate cancer (PCa) diagnosis often relies on biopsies, which can lead to unnecessary procedures and complications. Federated learning (FL) offers a privacy-preserving approach for training predictive models across hospitals without sharing sensitive patient data. In this study, we evaluate the feasibility of FL for PCa risk prediction by benchmarking different training strategies, including local, federated models, as well as free-riding (FR) on federated models. Using real-world heterogeneous datasets from 19 hospitals, we analyze the impact of data diversity and consortium size on predict…

Repeated Detection of Rubella Virus IgM Antibodies in Two Pregnancies Without Evidence of Fetal Infection: A Case Report and Challenges in Serological Interpretation.

Serologic screening for rubella virus, as part of TORCH testing during pregnancy, remains a cornerstone in the prevention of congenital rubella syndrome. However, isolated detection of rubella-specific IgM antibodies may lead to diagnostic uncertainty, especially in the absence of clinical signs or supporting serological evidence. We present a case of a pregnant woman who tested positive for rubella IgM antibodies during two consecutive pregnancies, despite lacking any sonographic or serologic indications of fetal infection. Both pregnancies culminated in the delivery of healthy neonates with

A Rare Case of Sarcoidosis With Aggressive Bony Lesion in the Frontal Bone.

Sarcoidosis is a multisystem granulomatous disorder that can rarely involve bone, particularly the skull, where lesions may closely resemble malignant or infectious processes. We report a 60-year-old man with established multisystem sarcoidosis who presented with acute left-sided headache, diplopia, and periorbital pain. Neuroimaging demonstrated a poorly defined left frontal bone lesion with homogeneous post-contrast enhancement. Laboratory evaluation revealed elevated angiotensin-converting enzyme levels, and a supraclavicular lymph-node biopsy confirmed non-caseating granulomas. As systemic

Chilaiditi's Sign: A Rare Finding in a Patient with Concurrent Liver Cirrhosis and Chronic Lymphocytic Leukemia.

Chilaiditi's sign is a rare radiological finding characterized by colonic interposition between the liver and the right hemidiaphragm. It is a cause of pseudopneumoperitoneum which is often mistaken for true pneumoperitoneum. In this article, we present the case of a 73-year-old female with a previous history of pulmonary tuberculosis, who reported to our hospital with progressive abdominal distension associated with early satiety, bipedal swelling, generalized weakness, worsening fatigue, and weight loss. Following evaluation, she was diagnosed with liver cirrhosis and chronic lymphocytic leu

Immunoassay interferences: laboratory pitfall in the diagnosis of adrenocortical carcinoma.

A woman in her late 50s with recent onset of hypertension, diabetes, lumbar pain and unintentional weight loss was diagnosed with a cortisol and androgen-producing adrenal mass. Despite this, serum adrenocorticotropic hormone (ACTH) concentration was inappropriately elevated, which was investigated thoroughly. Investigations included a brain magnetic resonance imaging to exclude concomitant pituitary adenoma, a corticotropin-releasing hormone stimulation test and a gallium-68 DOTATATE and 18F-FDOPA PET scan, both excluding ectopic ACTH production. Considering the disparity between clinical pre

Comprehensive Update and Review of Clinical and Imaging Features of SMART Syndrome.

Источник пометил публикацию как отозванную

Stroke-like migraine attacks after radiation therapy (SMART) syndrome is a delayed complication of cranial irradiation, with subacute onset of stroke-like symptoms including seizures, visual disturbance, speech impairment, unilateral hemianopsia, facial droop, and aphasia, often associated with migraine-type headache. The diagnostic criteria were initially proposed in 2006. However, the diagnosis of SMART syndrome is challenging because clinical symptoms and imaging features of SMART syndrome are indeterminate and overlap with tumor recurrence and other neurologic diseases, which may result in

Idiopathic asymptomatic pneumoperitoneum in a patient with ureteric calculus and Crohn's disease.

A 46-year-old male referred to the Emergency Department with pain in the left flank. The patient suffered from Crohn's disease. He had not experienced any fever, and a urine strip showed signs of blood. A ureteric calculus was suspected and a computed tomography scan was performed, which surprisingly showed free intraperitoneal air (FIA) located along the ascending colon. The patient had no signs indicative of peritonitis and the spontaneous pneumoperitoneum was suspected. Pneumoperitoneum is often associated with severe intraabdominal pathology, such as perforation of the gastrointestinal tra

MR imaging features of multiple biliary hamartomas (Von Meyenburg Complex): A pictorial review and differential diagnosis.

Biliary hamartomas (BHs) are rare malformative cystic/cystic-like lesions of the liver affecting the biliary tree, named after Hanns von Meyenburg who described them for the first time and still known with this eponym to this day. They usually lack clinical symptoms, and abnormalities in liver function tests are unusual; thus, it is typically an incidental finding of liver imaging. Despite being benign lesions, BHs can pose clinical challenges; the first one is differential diagnosis with other more relevant pathological conditions. Therefore, knowledge of MR imaging findings of BHs is helpful

Humeral greater tuberosity osteolysis as a complication of intraosseous calcification migration: natural history depicted by imaging.

Migration of calcification within the bone leading to greater tuberosity osteolysis is a peculiar complication of the calcifying tendinitis of the rotator cuff. The case of a 38-year-old woman complaining of right shoulder pain, which had been going on for one year, is hereby described. The evolution of the infraspinatus tendon calcifying tendinitis leading to osteolysis of the greater tuberosity of the humerus is depicted by imaging and, particularly, by the MR and CT features changing over time. In this paper we focus on the importance of both MR and CT exams in the diagnostic process of the

Bone up on spinal osseous lesions: a case review series.

Spinal osseous neoplasms are frequently encountered and can be challenging when present as solitary lesions. Familiarity with the range of benign and malignant spinal pathology can help the radiologist formulate a comprehensive differential diagnosis. This article focuses on the spectrum of extradural spinal tumors, accounting for the majority of primary spinal tumors, by comparing the epidemiology, pathophysiology, clinical presentation, and characteristic imaging appearance of these lesions. The discussion includes the commonly encountered benign lesions, such as vertebral venous vascular ma

A Case for Abandoning Inpatient Fecal Occult Blood Testing.

Fecal occult blood testing (FOBT) is currently Food and Drug Administration (FDA) approved only for colorectal cancer (CRC) screening. There is now widespread off-label use of FOBT in the hospital setting as a diagnostic test. Here we present a brief case and a more detailed review of the literature arguing against inpatient FOBT. Inpatient use of FOBT is problematic for several reasons including failure to account for false positives or negatives, delays in appropriate consultations or endoscopy, increased costs, increase length of stays, unnecessary procedures, and test results that do not c…

Team Approach: Management of Brachial Plexus Birth Injury.

Brachial plexus birth injury is an upper-extremity paralysis that occurs from a traction injury to the brachial plexus during birth. Approximately 10% to 30% of children with a brachial plexus birth injury have residual neurologic deficits with associated impact on upper-limb function. Management of brachial plexus birth injuries with a multidisciplinary team allows optimization of functional recovery while avoiding unnecessary intervention. Early occupational therapy should be initiated with a focus on range of motion and motor learning. The need for microsurgical reconstruction of the brachi
