Alcohol septal ablation in hypertrophic cardiomyopathy.

Alcohol septal ablation (ASA) has become an alternative to surgical myectomy in obstructive hypertrophic cardiomyopathy since it was first introduced in 1994 by Sigwart. The procedure alleviates symptoms by producing a limited infarction of the upper interventricular septum, resulting in a decrease in left ventricular outflow tract (LVOT) gradient. The technique has been improved over time and the results are comparable with those of myectomy. Initial concerns about long-term outcomes have been largely resolved. In this review, we discuss indications, technical aspects, clinical results and pa

Management of Neonates With Complex Left-Sided Obstructions Using Endovascular Flow Regulators and Ductal Stenting.

Surgical repair or palliation in neonates with complex left-sided obstructive congenital heart disease, especially with comorbidities or low birth weight, remains a high-risk intervention. We describe an initial transcatheter strategy involving the implantation of modified pulmonary vascular occluders and stenting of the ductus arteriosus to regulate pulmonary overflow and maintain systemic perfusion. Four neonates are presented: 2 low-birth-weight neonates with hypoplastic left heart syndrome and left ventricular-to-coronary artery connections; one significantly underweight neonate with aorti

Clinical Outcomes After the Surgical Treatment of Hypertrophic Obstructive Cardiomyopathy With Left Ventricular Apical Aneurysm.

BACKGROUND: Left ventricular apical aneurysm (LVAA) is a rare complication of hypertrophic obstructive cardiomyopathy (HOCM). This study aimed to illustrate the clinical characteristics and surgical outcomes of these patients. METHOD: Patients with HOCM and LVAA who underwent modified extended Morrow myectomy and surgical left ventricular reconstruction (SLVR) between October 2012 and March 2021 were retrospectively recruited. Patients with coronary artery disease were excluded. Clinical characteristics were summarised. Time-to-event outcomes were calculated using the Kaplan-Meier method and

Clinical profile and outcomes of pediatric hypertrophic cardiomyopathy in a South Indian tertiary care cardiac center: a three decade experience.

INTRODUCTION: Although much research has been done on adult hypertrophic cardiomyopathy, data on pediatric hypertrophic cardiomyopathy is still limited. METHODS AND RESULTS: The study enrolled all patients with cardiomyopathy who presented to us between 1990 to 2020 and were younger than 18 yrs. During the thirty-year study period, we identified 233 cases of pediatric cardiomyopathy. Sixty-three cases (27%) had hypertrophic cardiomyopathy. Out of the 63 HCM cases, 12% presented in the neonatal period and 37% presented in the first year of life. The median age of presentation was 7 yrs (Range

Univentricular heart: management options.

The term ''Univentricular Heart'' encompasses a wide variety of heart defects that functionally and physiologically constitute a single ventricular chamber. The terminology ''univentricular repair'' is frequently used in the surgical literature to include those biventricular hearts that are not amenable for a final two ventricle repair and need to go through the same surgical stages as with a functionally univentricular heart, culminating finally in a total cavo-pulmonary connection. Broadly, treatment is focused on controlling the pulmonary blood flow in early infancy, by means of aorto-pulmo

Rapid haemodynamic improvement with trametinib in Noonan syndrome-associated hypertrophic obstructive cardiomyopathy: a report of two cases with distinct genotypes.

BACKGROUND: Noonan syndrome is an autosomal dominant RASopathy caused by germline gain-of-function variants in the renin-angiotensin system/mitogen-activated protein kinase signalling pathway. Hypertrophic obstructive cardiomyopathy is a serious and potentially fatal manifestation of Noonan syndrome, most characteristically associated with RAF1 and RIT1 variants. Trametinib, a selective MEK1/2 inhibitor, has demonstrated efficacy in Noonan syndrome-associated hypertrophic obstructive cardiomyopathy across several genotypes; however, real-world data pairing distinct genotypes with clinical resp

Changes in left atrial function, left ventricle remodeling, and fibrosis after septal myectomy for obstructive hypertrophic cardiomyopathy.

BACKGROUND: This study aimed to investigate the impact of septal myectomy on left atrial function, left ventricle remodeling, and fibrosis in patients with obstructive hypertrophic cardiomyopathy. METHOD: From May 2012 to September 2016, preoperative cardiac magnetic resonance imaging of 507 adult patients who underwent septal myectomy at Fuwai Hospital was retrospectively collected. Until October 2019, 57 patients were followed up with postoperative cardiac magnetic resonance imaging at 11.9 months (interquartile range, 6.4-25.3). Preoperative and postoperative left atrium and left ventricle

Preoperative imaging of left portal vein at the Rex recess for Rex shunt formation using wedged hepatic vein carbon dioxide portography.

BACKGROUND: In children with extrahepatic portal vein obstruction (EHPVO), formation of a mesentericoportal bypass (Rex shunt) restores hepatopetal flow, relieves portal hypertension, and reduces variceal bleeding and hypersplenism. The Rex shunt is created by inserting a vein graft between the superior mesenteric vein and the umbilical segment (Rex) of the left portal vein within the Rex recess of the liver. The preoperative evaluation of a patient with EHPVO includes an accurate assessment of the venous inflow and outflow. The inflow portal vein is readily assessed by ultrasound and magnetic

Sequential Use of Alcohol Septal Ablation and Electrosurgical Leaflet Resection Prior to Transcatheter Mitral Valve Replacement.

In patients with increased surgical risk and hemodynamically significant mitral disease, a transcatheter strategy for mitral valve replacement (TMVR) may be suitable; however, is also not without procedure risk. Obstruction of the left ventricular outflow tract (LVOT) is one of the most dreaded complications of TMVR, requiring careful consideration of potential candidates with preprocedural imaging and ex vivo valvular fit simulation as part of risk assessment for postprocedure obstruction. In patients at high risk of LVOT obstruction, early studies have shown that alcohol septal ablation or e

[Anesthetic management of a patient with mid-ventricular obstructive hypertrophic cardiomyopathy treated with dual-chamber pacing].

A 64-year-old man complicated with mid-ventricular obstructive hypertrophic cardiomyopathy (MVO) was scheduled for resection of rectum cancer under general anesthesia with epidural block. Because of unexpected circulatory collapse at the induction of anesthesia, the operation was canceled. Therefore three weeks later, we inserted a temporary dual-chamber (DDD) pacing device before induction of general anesthesia, and we could maintain stable hemodynamics during general anesthesia. In the patients with MVO, blood outflow from the left ventricle to the aorta is decreased by the abnormal blood fl

Effect of mavacamten and disopyramide on left ventricular mechanical dispersion and life-threatening arrhythmia in obstructive hypertrophic cardiomyopathy.

AIMS: Disopyramide and mavacamten both decrease left ventricular outflow tract gradients in obstructive hypertrophic cardiomyopathy (HCM). Yet, their effects on myocardial mechanics remain unclear. This study aimed to compare the effects of mavacamten and disopyramide on left ventricular mechanical dispersion (LVMD) and its association with life-threatening ventricular arrhythmias. METHODS AND RESULTS: Consecutive subjects (n = 120) with obstructive HCM treated with either Mavacamten or disopyramide from 2018 to 2024 were identified. Modulating energy metabolism to treat non-obstructive hypertrophic cardiomyopathy? Insights from IMPROVE-HCM.

Hypertrophic cardiomyopathy (HCM) is the most common genetic heart disease worldwide and may present with or without dynamic left ventricular outflow tract obstruction (LVOTO). Significant advances have been made in the management of obstructive HCM. On the other hand, despite their significant symptomatic burden, patients with non-obstructive HCM (nHCM) (i.e., without LVOTO) still do not have evidence-based therapeutical options. The recent IMPROVE-HCM study, a phase 2 randomized, double-blinded trial, aims to place a first step in filling this gap in knowledge. The study assessed the safety

Aortic valvuloplasty under echocardiographic guidance in a minor infant at a national referral center in Peru: case report.

Aortic valve stenosis is a congenital heart defect that causes a fixed left ventricular outflow obstruction with a progressive course. Symptomatology in neonates and young infants resembles congestive heart failure. In addition, the diagnosis of this condition is made by imaging, through echocardiography. On the other hand, treatment can be surgical or interventional under fluoroscopic guidance, depending on the hospital in which it is performed. We describe the case of a minor infant patient who presented severe aortic valve stenosis; however, the fluoroscopy equipment was not available at th

Giant coronary aneurysm of Behcet's disease with sudden syncope: a case report.

Behcet's disease(BD) is a chronic inflammatory vasculitis that rarely affects the arteries, making myocardial infarction unlikely. We report a 28-year-old patient who was admitted to our hospital with multiple sudden syncope. Cardiovascular risk factors such as hypertension (HT), diabetes and obesity were not found in her. Preoperatively, imaging examinations suggested thrombosis of the inferior and superior vena cava and right heart combined with coronary artery aneurysm. The patient was finally diagnosed with a huge coronary artery aneurysm proximal to the left anterior descending artery. Sy

Successful Conservative Treatment of Cardiac Rupture Associated with Takotsubo Syndrome.

We herein report a 75-year-old woman who was diagnosed with Takotsubo syndrome (TTS) complicated by left ventricular outflow tract obstruction on admission. Treatment with beta-blocker and anticoagulant was started; however, her hemoglobin level decreased gradually, and computed tomography performed one week later revealed hemopericardium. Oozing-type cardiac rupture was suspected; therefore, we discontinued heparin treatment. Finally, she recovered uneventfully without cardiac surgery. It is noteworthy that cardiac rupture may occur with TTS, especially in patients treated with prophylactic a

Primary leiomyosarcoma of the atrium with heterologous differentiation.

UNLABELLED: We report a case of a 47-year-old female who presented with breathlessness and palpitations for two weeks. On clinical evaluation, bilateral pedal edema was noticed. A CT pulmonary angiogram showed a mass in the left atrium causing significant obstruction to cardiovascular outflow. After extensive work-up, the mass was surgically resected. Histopathological findings from the acquired specimen revealed a high-grade leiomyosarcoma with extensive necrosis and heterologous (cartilaginous) differentiation. The early postoperative period was complicated by cardiac tamponade and the patie

Long-term clinical outcome after alcohol septal ablation for obstructive hypertrophic cardiomyopathy: results from the Euro-ASA registry.

AIMS: The first cases of alcohol septal ablation (ASA) for obstructive hypertrophic cardiomyopathy (HCM) were published two decades ago. Although the outcomes of single-centre and national ASA registries have been published, the long-term survival and clinical outcome of the procedure are still debated. METHODS AND RESULTS: We report long-term outcomes from the as yet largest multinational ASA registry (the Euro-ASA registry). A total of 1275 (58 ± 14 years, median follow-up 5.7 years) highly symptomatic patients treated with ASA were included. The 30-day post-ASA mortality was 1%. Overall, 1

[2D and Doppler echocardiography after atrial switch operation for transposition of great arteries; a review].

Introduction of the atrial baffle procedure (Mustard, Senning) more than 30 years ago has dramatically improved the prognosis of patients with transposition of the great arteries. However, a range of late complications can occur even in clinically asymptomatic patients including: superior and inferior limb obstruction, pulmonary venous obstruction, residual baffle-leakage, left ventricular outflow tract obstruction, tricuspid valve regurgitation, right ventricular dysfunction and postoperative dysrhythmia. As an increasing number of patients has reached adulthood an accurate noninvasive techni
