Alcohol, cancer, and immunomodulation.

There is a great deal of epidemiological evidence indicating that chronic, excessive alcohol consumption is a major risk factor for cancers in humans. However, the experimental basis for the increased cancer risk associated with alcohol intake is not clear. Since it appears that ethanol alone is not carcinogenic, ethanol effects must be explained in terms of its modifying the actions of other causal agents. Current studies indicate that ethanol and its congeners may act as tumor promoters, thereby enhancing the effect of initiating carcinogens from the environment. Available evidence also show

[Liver damage in alcoholics].

The authors made a liver biopsy in a group of 42 persons chronically abusing alcohol. In the majority of patients the authors found various forms of hepatopathology: fatty degeneration--8 cases (19%), alcoholic hepatitis--10 cases (24%), chronic active hepatitis--11 cases (26%), and hepatocirrhosis--2 cases (5%). The authors found no correlation between the kind and degree of liver damage and the amount of alcohol drunk, which suggests the role of individual hypersensitivity in the development of hepatopathology in alcoholics. In some of the alcoholics liver damage was connected with infection

[Neurologic effects of alcoholism].

The alcoholism is no hereditary but genetic factors can be involved and racial differences in alcohol susceptibility depend on break down rate of ethanol. The alcohol adaptation and tolerance and physical dependence appearance with symptoms and signs display after abstinence on people suffers with chronic intoxication, it would be related to changes caused by alcohol over molecules of neuronal membranes, especially proteins and receptors. An account is achieved about the various neurologic manifestations related to alcoholism, nutritional in origin or unknown pathogenesis. The results obtained

Alcoholic liver disease.

Liver injury may develop in some people who consume alcohol. The pathogenesis of liver damage in such subjects remains obscure. Major histopathologic features of alcohol-associated liver injury include steatosis, steatonecrosis, and cirrhosis. The clinical manifestations of alcoholic liver disease are nonspecific and range from asymptomatic hepatomegaly to stigmata of portal hypertension with advanced parenchymal failure. The severity of the clinical presentation and the degree of aminotransferase elevation correlate poorly with the liver histopathology, particularly in patients who continue t

[Acute alcoholic hepatitis].

In a group of 1,025 cases of alcoholic liver the authors detected 28 cases (2.73%) of acute alcoholic hepatitis on the basis of the morphologic examination, 75% were males, mean age 45 +/- 18 years. The mean amount of alcohol consumed daily was 106.7 g for a mean duration of 19.78 years. The hepatic lesional substrate was hepatic steatosis and cirrhosis in 7 cases (25%) and steatofibrosis in the other 14 cases (50%). The triggering factors were the abuse of alcohol in 27 cases and a pneumonia in the last case. The disease was characterized by fever, encephalopathy, hepatomegaly, hepatocytolyti

[Alcohol-induced cardiomyopathy].

Because of increasing alcoholism the importance of alcoholic organ lesions is also increasing. The alcoholic cardiomyopathy, which is the result of toxicity of alcohol, but also of alcohol-induced hypertension, coronary sclerosis and toxic destroyed nervous system, is demonstrated with a the keep of case. Stages, diagnosis, therapy and prognosis are described. The most important consequence is abstinence.

Loneliness and adolescent alcoholism.

This paper first shows how the feelings of separation anxiety, hostility, and loneliness are intrinsically related. In pursuing this line of thought, the views of Fromm-Reichmann, Fromm, and Erikson are synthesized. The foregoing emotions are then discussed within the context of Freud's speculative postulation of the "oceanic feeling." All these conceptual connections, in turn, are then analyzed as essential factors contributing to, and indeed determining, adolescent drinking disorders. Traumatic issues and conflicts, initially centered in the infant's sense of separation from the mother, are

[Alcohol and endocrinologic homeostasis].

In addition to direct toxic effects on endocrine organs chronic alcohol intake affects regulation of endocrine systems by disturbed liver function. As a result in patients with alcohol-induced liver cirrhosis gonadal axis is characterized by low total and free testosterone, elevated estradiol. LH, FSH, and sexual hormone binding globulin and an enhanced conversion of testosterone to estradiol. Prolactin also is found to be elevated. The thyrotropic axis is characterised by low T3- und T4- as well as elevated rT3-values and normal TSH. STH is elevated, while somatomedin C is decreased. The cort
